Valid ID (e.g Government ID) of the signatory with 3 specimen signature

Latest Proof of Billing (under the Applicant name and address)
Business Permit
BIR/ COR
SEC

Note:

If you can't provide Proof of Billing under the Applicant Name but the service address is
same in the application form,
Need an Authorization Letter from the owner of Proof of Billing and also need the Valid
ID of the Proof of billing owner or Contract of Lease (notarized)

FOR GOVERNMENT ENTITY

1. Government issued ID or Company ID of Signatory/Representative (Front and Back

with 3 specimen Signatures and not Expired)
2. Notarized Certificate of Funds Budget Office
3. Proof of Budget Allocation signed by Municipal Treasury
