GUIDE TO COMPLETING THE FORM
All equipment to be used at the installation must be listed together with their characteristics of supply which includes
operating voltage; operating current; equipment rating; single/three phase.
Additional Guidelines: -

1. If the time frame for installation of a meter exceeds six (6) months, the job will require reevaluation before
proceeding.
2. If a power rating (kVA) is available, then this should also be stated.
3. For Air Conditioning equipment, the rating in Tons or Btu should be stated.
4. The number and type of lights must be stated (e.g. compact fluorescent; LED; bay lighting; perimeter lighting)
5. The number of outlets for general use must be stated. Note: Outlets utilized for equipment separately stated
should not be included here.
6. For multi-metered premises, the equipment contained in each installation must be separately listed. This should
be done for both existing and proposed meters.
N.B. – Actual equipment ratings and NOT circuit breaker sizes must be submitted in the load detail.
