1. Introduction
1.1. General
It is with great satisfaction that we present the successful HSE journey undertaken during the P&A activities at the
KNDP-A platform, executed by Tanjung Offshore Services Sdn Bhd under the vigilant oversight of our valued client,
Hibiscus Malaysia (HML).

Our unwavering commitment to safety has yielded commendable results, with an impeccable record of 123,528
manhours logged over 224 incident-free days. This accomplishment stands as a testament to our steadfast dedication
to fostering a safety-first culture, ensuring the well-being of our workforce.

A crowning achievement in our safety narrative is the realization of 100% compliance with the total plan versus
program outlined in our Project HSE Plan. This exemplifies our meticulous approach to safety planning and execution,
reflecting our pledge to go above and beyond industry standards.

As we transition to the HSE closeout phase, we extend our gratitude for the collaborative spirit maintained throughout

1.2. Summary of Contract Scope of Work


The scope of work for this contract is the P&A and workover activities at KNDP-A platform and it is for 2 wells, which
is 119 (Workover) & 111 (P&A). This campaign will be executed using a Hydraulic Workover Unit 502 (EMAS 502
HWU) with assistance of marine spread vessel under provisional of HML.
The HWU and the P&A associated equipment and services will be rig up and located on the platform throughout the
campaign.

The operations will be taking place within the PM3 North, PM3 South and PM305 South Angsi fields. The below figure
shows an overview of the operational areas:

The HSE Closeout Report for the KNDP-A P&A Project by Tanjung Offshore Services Sdn Bhd is a comprehensive
document designed to encapsulate the entirety of our Health, Safety, and Environment endeavors throughout the
project's lifecycle. This report aims to provide a transparent and detailed overview of our safety performance, lessons
learned, and future recommendations. The key components of the HSE Closeout Report include:

Personnel (IP), Mohd Syakirin Bin Jusoh, reported experiencing back pain, with discomfort extending to his mid-
chest region after performing the Maghrib prayer in the KNDP-A switch gear maintenance office.

Tanjung Offshore's response to this incident was prompt and effective. At 20:05, the T7 HSE personnel observed
the IP and, recognizing signs of fatigue and illness, immediately contacted the platform medic. The medic arrived

promptly at the switchgear with the necessary medical equipment to attend to the IP's condition. Subsequently,
the IP was swiftly transported to the sick bay on the offshore KNDP-A platform for a thorough assessment and
required medical care.

Further medical evaluation was conducted, including an electrocardiogram (ECG) at 20:21, which revealed a
normal ECG with a sinus rhythm. At 20:59, the medic consulted with the onshore Occupational Health Doctor
(OHD) to determine the appropriate course of action. Following the consultation, it was decided that the IP, Mohd
Syakirin Bin Jusoh, should undergo evacuation via helicopter (MEDEVAC) for further health evaluation and
treatment at the onshore General Hospital.

This incident report demonstrates Tanjung Offshore's commitment to the health and safety of its personnel,
showcasing a well-coordinated emergency response that prioritized the well-being of the individual involved. The
detailed timeline and actions taken highlight the effectiveness of the emergency preparedness and response
measures in place.

5. Recommendations and Continuous Improvement


As part of Tanjung Offshore's commitment to continuous improvement in Health, Safety, and Environmental (HSE)
practices, several recommendations have been implemented onboard KNDP-A. These initiatives stem from lessons
learned in various aspects, emphasizing proactive measures to enhance safety protocols and operational efficiency.
a) Crew Situational Awareness:
• Regular training sessions have been conducted to reinforce the importance of crew situational awareness,
particularly during lifting operations, manual handling, and fishing activities.
• Emphasis has been placed on clear communication, defined roles and responsibilities, and the need for
continuous vigilance to prevent accidents during critical tasks.
b) Risk Assessment:
• The risk assessment process has been enhanced by incorporating a comprehensive review of potential
hazards before the commencement of new tasks.
• Additional training has been provided to personnel involved in risk assessments, ensuring they can identify
all possible risks and implement effective control measures.
c) Heat Stress and Fatigue Management:
• Work schedules have been optimized to address heat stress and fatigue, considering factors such as
adequate breaks, hydration, and rest facilities.
• Regular awareness programs on the importance of managing heat stress and fatigue have been conducted,
emphasizing personal responsibility and proactive measures.
d) Deck Management and Congestion:
• Work processes and equipment placement on the deck have been streamlined through improved planning
and coordination.
• Clear pathways and signage have been implemented to prevent congestion, tripping hazards, and facilitate
efficient task execution.
e) Slip and Fall Prevention:
• Housekeeping practices have been enhanced with a focus on preventing slip and fall hazards.
• Anti-slip surfaces, appropriate footwear, and regular crew awareness programs have been implemented
to mitigate potential accidents.
f) Internet Connectivity:
• Backup communication methods have been explored to address intermittent internet issues.
• Troubleshooting protocols and contingency plans for scenarios where internet connectivity is compromised
have been developed.
g) Effective PTW Audits:
• The PTW audit process has been continuously improved by refining procedures and ensuring effective
communication among personnel involved.
• Identified issues from audits are promptly addressed, and lessons learned are integrated into future audit
protocols.
h) Lessons from Fishing Activities:
• Procedures related to fishing activities have been reviewed and updated based on lessons learned.
• Safety protocols, equipment handling, and the sharing of experiences have been emphasized to prevent
incidents during fishing operations.
i) Regular Training and Briefings:
• The effectiveness of crew training and pre-job briefings has been enhanced through continuous
improvement initiatives.
• Crew members are regularly informed and educated about safety protocols and task-specific
considerations through ongoing training sessions.

j) Emergency Response and Communication:


• Lessons learned from emergency response drills have been incorporated into crisis management
strategies.
• Emergency response plans are regularly tested and updated, with a focus on improving communication
protocols and ensuring crew preparedness for various scenarios.
These recommendations represent Tanjung Offshore's proactive approach to safety culture, ensuring continuous
improvement and the highest standards of safety onboard KNDP-A.

6. After Action Review


The HSE statistics for the KN campaign are satisfactory. Some feedback received during the session that includes:

I. To enhance the existing heat reduction height in the work basket area, a reduction shield was constructed at the HWU
work basket, addressing the high level of heat exposure caused by its proximity to the platform flare. This measure aims
to minimize the heat impact on personnel.
II. Adding extra heavy-duty blowers on every level of the HWU and platform deck to improve ventilation.
III. Personnel should stay mindful of their water intake levels and ensure adequate hydration at all times.
IV. Continuously implement the Buddy System to prevent Heat Stress and Stroke symptoms, while maintaining a constant
awareness of Fatigue Management among everyone
V. Facilitating the most effective interaction and discussion during the TBM involves the OIM taking the lead in steering the
session to drive the daily mission. During the Toolbox Meeting (TBM), the OIM plays a crucial role in overseeing the Plan,
Brief, Execute, and Debrief phases. This encompasses thorough observation during the TBM to ensure a comprehensive
understanding of the tasks at hand and fosters a proactive approach to safety and mission success.
VI. Conducting brief TBT sessions in between activities to prevent any communication gaps.
VII. Despite having personnel from multiple nationalities working on the same projects, there is no evidence of communication
barriers, as the team demonstrates effective communication.
VIII. Credit is due for successfully managing 5 simultaneous operations (SIMOPS) during the campaign without any incidents
or accidents.
