1.0 PROJECT QUALITY STATEMENT

The PMC shall ensure that the CONTRACTOR complies with the following:
 Achieve a quality service by meeting customer requirements.

 Define and document the Quality System for the project’s functions to comply
with the requirements of ISO 9001:2015

 Develop and use qualified personnel, trained with the necessary skills and
experience to perform their duties.

 Perform in accordance with documented Quality System with the aim of error
free performance

 Ensure that processes and products provided by CONTRACTOR are in


accordance with the contract, safety and international standards.

 Evaluate the Quality System’s suitability continuously and monitor its operation
to improve its effectiveness.

2.0 PURPOSE

The purpose of this Project Quality Plan is to have a planned quality control program
in order to achieve full conformance of all the project works in accordance to the
contract requirements and applicable codes, standards, specifications, drawings and
other related International Standards.

This Plan shall include a description of the procedures, instructions and measures to
be used to assure that all activity carry out by the CONTRACTOR is controlled in
accordance to specified or contract requirements.

The Plan is also directed towards implementing and enforcing the quality assurance
and quality requirements for safety related structures and components is in
compliance with specified requirements.

PMC shall conduct all quality control activities in accordance to the procedures
contained in CONTRACTOR Work Method Statements to ensure it meet the
requirement of the standard.

PMC shall have access at all times during each activity stage to inspect any part of
the job in progress or completed by the CONTRACTOR.

3.0 SCOPE

This Project Quality Plan is designed to explain the quality system to be


implemented by PMC to the CONTRACTOR during the execution of the project;-
Project Management Consultant (PMC) Services For The Civil Construction
Works For WACT Terminal Upgrade Project At Onne, Nigeria.

The proposed work to be executed under this tender shall cover; Procurement,
Materials supply inspection and handling, civil work construction, fabrication of mild
steel materials (if required) etc.

7.0 PROJECT QUALITY POLICY

7.1 Quality policy


The policy for this project is in compliance with the PMC Quality Policy as defined in
our Quality System Manual.
This Quality plan has been drawn up based on the project requirements and in
compliance with the requirements of International standard ISO 9001:2015.
Quality Manual guidelines.
In particular, PMC Quality Policy for this project to the CONTRACTOR is as follows:
 Document all Quality system activities by implementing procedures issued in
conformance with applicable standards.

 Assure by means of adequate planning that all services are of high quality and
in compliance with contractual standards and specifications.
 Use experienced personnel in all areas relevant to quality during project
execution.
 In addition, all personnel should work in accordance with the Quality Plan,
Procedures, Standards and work instructions.

7.2 Quality Objectives


All Quality objectives align themselves with the Safety Management System
objectives to be applied to this project and will be maintained by the project QA/QC
Engineer and distributed in accordance to the project requirement.
The objectives stated below are those to be realized by the development and
implementation of PMC Quality Management System requirements by the
CONTRACTOR during the Project.
 Implementing the quality system during works and provide final products/or
services which are fit for purpose and meeting the operational requirements,
functional performance and reliability.
 To demonstrate by means of documented quality records that the work has
been carried out to the required level of quality and was inspected.
 To ensure that the work is properly planned and organized.
 To ensure tasks and functions are correctly assigned within the project
framework.
 Continuously improve the performance of the Company by adhering to its total
commitment in implementing the Quality Management System.
 Establishment of both internal and external organizational interfaces for issue,
during the execution of the project work.
 Detection of non-conformances and omissions and the reduction of waste
during the period of the construction.
 To emphasize and ensure that all interface activities, including the critical areas
of vendors, suppliers, are conducted in an environment of “best practice” in
accordance with the Quality plan.

 To ensure by planned audit that the work is being executed in accordance with
the established work procedures and instructions.
 To verify by inspection that the work is being carried out to the required level of
quality.

8.0 PROJECT QUALITY SYSTEM STRUCTURE


This Project Quality System is derived from PMC quality management system to the
CONTRACTOR by selecting applicable corporate management system procedures
against the project specification and authorising these, with adaptations as required,
for use on the project.

The project quality system elements are defined as follows:

8.1 Project Quality Plan


The project quality plan describes in details the project quality system to be adopted
by the PMC to the CONTRACTOR during the execution period.

 Reflects project quality objectives and responsibilities.

 Describes the structure of the project quality system and the interrelation of
specific system elements.

 Defines specific scope requirements.

 Defines the project quality audit requirements and implementation.

8.2 Project Practices


Practices describe how an individual activity identified in a procedure is performed
(such as forms, specifications, codes and Standard etc.) to be utilized and may
include technical details of the work to be performed.

The current version of applicable Company practices is modified to meet PMC


requirements and shall be used as project practices. Project specific practices shall
be developed (if required) and controlled as part of the Project Quality Plan.

8.3 Project Standards


Standards are the available tools, which are to be used for effective and consistent
execution of the individual tasks and assignments and are referenced from the
associated practices. A master list of all the procedures is maintained and updated
as required.

8.4 Document Control


Documentation and data developed, received and/or implemented during Project
execution shall, be controlled in accordance with Document and Data Control
Procedure. Further controls, relevant to interfacing with the Project contract
requirements, are described in this Quality Plan.
Document Control Registers shall include all Project documentation, specifications,
reports and all sub-contractor submissions, to meet the Project/contract
requirements.
Only the Project Director or representative shall approve the issue of the Project
Control Procedures.

9.0 MANAGEMENT RESPONSIBILITY


PMC already has in place a quality system established to implement and maintain an
effective means of ensuring that services and products provided by the company
conform to specified requirements. For this scope, a project organization will be
established in compliance with the contract demands. For Administrative purposes,
the project management team will work directly with the CONTRACTOR during the
project execution.
The activities of the team shall consist of but not limited to:
 Project administration
 Quality Monitoring, Control and assurance

 Supervising operations at all level

9.1 Project Organization and Responsibility


The Project Organization describes the responsibilities and authority levels of the
personnel performing quality activities for the project.

The responsibility, authority and the interrelation of all personnel who will manage,
perform and verify work affecting quality shall be defined by the CONTRACTOR,
particularly for persons who need the organizational freedom and authority to:
 Initiate action to prevent the occurrence of contract non-conformity.
 Identify and record any work quality problems
 Initiate recommend or provide solutions through designated channels
 Verify the implementation of solution
 Control further processing, delivery or installation of non-conforming product
until the deficiency or unsatisfactory condition has been corrected.

The PMC QA/QC Team performing quality assurance with the CONTRACTOR,
quality control activities shall have sufficient authority, access to work areas and
organizational freedom to prevent and identify quality problems. They also shall
initiate, recommend and provide solutions to quality problems through designated
channels and verify implementation of solutions.

The QA/QC Manager shall ensure that any conflicts, which may arise, are
satisfactorily resolved and that the appropriate quality instructions are issued. The
QA/AC Manager has the support and co-operation of the Project Director or its
representative, in exercising his duties.

The QA/QC Manager has functional responsibility in liaison with the Project Director
or its representative, for developing and maintaining the Quality Plan and for issuing

management of the Project.

9.1.2.2 QA/QC Manager

The QAQC Manager Reports directly with Engineering/Project Director or its


representative and ensure that the CONTRACTOR carried out the following:
 Development of Quality Management System for the project.
 Development of Project Quality Plan.
 Development of proper Work Procedures and Method Statements
 Ensures compliance with approved Work procedures.
 Ensures that requisite Inspection and test are carried out as per QA/QC
requirements.
 Ensures that material resources for the project are in compliance with contract
specifications.
 Ensures that only qualified personnel handle specific work items.
 Audits the quality system with a view to correcting defective areas.
 Provides data for development of As-built documentation.

9.1.2.3 QA/QC Engineer

The PMC QAQC Engineer ensures that the CONTRACTOR implements the Quality
plan during the execution of the project. He represents all Quality related activities
during the project execution. His duties shall include but not limited to the following:
 Coordinating QA/QC related activities.
 Ensure that the CONTRACTOR are complying with the quality Plan,
 Ensure that the CONTRACTOR has QA/QC Procedures.
 Evaluation of Vendors / Subcontractors QA/QC Systems.
 Coordination and Supervision of all activities regarding establishment of the
final “As-Built Package”.

9.1.2.4 Document Controller

 Receives and keeps records and logs all documents / drawings.


 Transmits and keeps records and logs/drawings.

personnel/departments.
 Maintain library /register for documents and drawings.

10.0 RESOURCE MANAGEMENT


The PMC Personnel assigned to the Project are fully supported by the service
departments during the project execution.

The terms of reference for all Project personnel will be held on file and available for
review, to ensure all phases of work during the execution of civil Construction in
accordance with the Project quality requirements and by personnel with the requisite
competence.

The implementation of an induction process for all Project personnel will ensure that
the information communicated is consistent, accurate and provided in a timely
manner to facilitate a smooth introduction into the Project.

10.1 Product Realisation

10.1.1 Planning
Procedures implemented on the Project shall be developed to satisfy the
requirements of the Project/contract in line with the Corporate Management System
Controls; Project Control Procedures shall be integrated within Project Manuals and
traceable to the individual Manual through the Project Master Document Register,
which details all individual documents utilised on the Project.

Individual Procedures shall clearly identify the responsibilities and controls specific to
each activity.

Documents shall include and as a minimum, comply with the requirements of the
Project System as defined in the Project Document and Change Control Procedures.

Project Procedures, Drawings and Specifications, when appropriate, shall be


assigned a project-specific sequential document number to reference the individual
documents (as client requirement).
The Project Control Procedures may as a minimum encompass the following work-
scope and activities:
 Safety.
 Quality.
 Planning and Reporting.
 Equipment.
 Joining and testing.
 As-Built Records.
 Drawings.
Only the Engineering/Project Director or its representative shall approve the issue of
the Project Control Procedures.

10.2 Training
The CONTRACTOR shall carried out training (as appropriate) throughout the Project
duration to ensure that all personnel are familiar with the scope of work, equipment
function/operation and quality requirements to ensure that problems/queries are
resolved in a timely and efficient manner.

10.3 Project Review


PMC with CONTRACTOR shall Regular carry out Project Review Meetings during
the execution of the project. The Project Director or its representative shall specify
the frequency of the meetings and issue where necessary, the agenda and minutes
of meetings. Records of the review meeting shall be maintained and the following
points shall, as a minimum, shall be reviewed/discussed.
 Project progress.
 Non-conformities and corrective actions.
 Incident reports and preventative actions.
 Site Queries.
 Clarification or additional information needed.

 Resource requirements/issues (sufficient to meet Project requirements).

10.4 Interface Co-ordination.


It is the role of all Project personnel to be aware of interfaces and contribute to their
management activity by providing the requisite information in a timely manner

The following phases of the Project shall be considered in all interface controls:
 Procurement.
 Fabrication of Installation Aids.
 Construction.
 Grounding resistance test
 Cable acceptance Testing
 Material quality (Fitting, Wire etc.,)
 Preparation of the foundation and reinforcement

10.5 Competence.
It is the policy of the PMC to the CONTRACTOR that personnel whose roles are key
to the safety of the Project and also have technical expertise in the specific work in
which the Company is involved, shall be determined as competent prior to
progressing work.

10.6 Quality Improvement/Feedback.


During the Project, non-conformity shall be dealt with as described in the Non-
conformance Control Procedure so that analysis of the records generated, provides
the corrective action mechanisms to subsequently propagate improvement and
prevent recurrence.

The initial reporting of a non-conformance is the responsibility of the CONTRACTOR


personnel involved in the activity to which it relates. It is their responsibility to inform
the PMC supervisor, when appropriate, who shall be responsible for recording the
incident and the corrective actions.

If the action to be taken is to raise a deviation, the number of the application shall be
referenced on the Non-Conformance Report.

Deviations shall be raised on a Technical/Site Query Report and be submitted to the


relevant PMC person for approval as appropriate.

10.7 Supplier Quality Assurance.


Suppliers shall be required to operate and maintain a Quality System, which complies
with the requirements of ISO 9001: 2015

Suppliers shall be capable of compliance with the relevant quality assurance


requirements prior to the award of an order/ contract.

Dependent on the scope of supply, suppliers shall be required to submit the relevant
QA details associated with their work scope.

The prime responsibility for the provision of materials or services in accordance with
specified requirements rests with the supplier and the unnecessary duplication of
efforts will be avoided

Where warranted by the criticality of materials or services, appropriate levels of audit,


surveillance, and inspection shall be employed to monitor and verify the effective
implementation of the supplier’s quality plan(s). The results of these activities shall
be formally documented.

10.8 Control of Purchased Material, Equipment and Services


 Use of the Procurement Control procedure to assure that the applicable quality
requirements are included in procurement document; and/or

 Implementation of Procurement Control procedures that provide for the


appropriate pre-qualification of surveillance and periodic assessment of
effectiveness of the suppliers/subcontractor’s quality assurance/quality control,
as required. These procedures also provide for evaluation and monitoring of
suppliers/subcontractor procurement control to assure the
supplier/subcontractor adequately controls their purchases, materials and
services; and/or

 Implementation of Material Control procedures which provide for quality control


receiving inspection for conformance to procurement requirement, including
verification that applicable quality documentation provided by the supplier are
correct and available at the manufacturing facilities/shop or the site prior to use
or installation of the material or equipment.

The Project Director or its representative shall prepare the purchase requisitions in
reference to the material take-off and to the specific material specifications.

The specification of materials contained in the requisitions and in the purchase


orders shall conform to the requirements of the contract.

The Procurement personnel will serve the purchase orders to the selected
vendor/supplier.

Documentation of PMC quality activities related to receiving inspection and quality


assurance of CONTRACTOR shall be retained in accordance to the Document
Control procedures to assure that documentary evidence of action taken to verify
conformance to procurement requirements is placed in record files.

The required supplier documentation is controlled by implementation of Material


Control procedures and filed in accordance to Document Control procures to assure
that a record of conformance to procurement is kept.

The purchased materials must have the required material certificates.

PMC shall be responsible for the quality of the products, materials and services
supplied by CONTRACTORS.

10.9 Handling, Storage and Preservation of Materials


 The Material Controller is responsible in handling, storing and preservation of
materials, equipment and instruments.

 Material and equipment in storage is controlled by the use of Material Control


procedures which implement the handling, storage and instructions for the
preservation of material, equipment and instruments from design specifications

from suppliers, to assure that material and equipment are protected from
damage and deterioration and that special protective environment are
maintained when required.

 The methods of handling shall prevent damage to the materials, equipment and
instruments.

 The materials, equipment and instrument shall be stored in their designated


storage areas or stock rooms with provisions for preventing damage and
deterioration.

 The delicate equipment and instruments shall be stored in a clean and dry
place, and preserved in accordance with the instructions from the
manufacturer/supplier.

 PMC with CONTRACTOR controls material and equipment from suppliers by


special handling, storage and shipping procedures or by requiring instructions
from suppliers to prevent damage or deterioration during field operations which
will be incorporated in accordance to the procedures for instructions, Drawings
and Procedures.

 Receipt inspection includes verification of the required control to prevent


deterioration or damage during shipping and verification of receipt of the
required handling and storage instructions from the supplier.

10.10 Control of Inspection, Measuring and Test Equipment


The QA/QA Manager or its representative shall prepare the list of Inspection,
Measuring Equipment and Instrument that are deemed necessary for the job.

The QA/QA Manager or its representative shall verify if the Inspection, Measuring
Equipment and Instruments intended for the job are calibrated and controlled as per
the specific procedure.

The Equipment and Instrument Calibration Technician will send the equipment and
instruments to the certified testing laboratory for calibration. He may perform the
calibration of some equipment and instruments at site using the “Master Calibrated
Instrument”; witnessed by the Client representative.

The Equipment and Instrument Calibration Technician maintains the calibration


records, provide the identification of calibrated equipment and instrument and control
the issuance.

Defective or damaged instruments and those instruments with expired calibration


shall be immediately retrieved from the work sites.

11.0 INSPECTION AND TESTING


The PMC shall ensure that the inspection and testing prepared by the
CONTRACTOR shall be performed relevant to the project activities and site works.

The material receiving inspection will verify if the contractor purchased materials and
the supplied materials conform to the specifications.

The in-process inspection and the final inspection and tests shall be
performed/evaluated in accordance with the specific quality control procedures,
method statements and to the applicable codes or standards.

The inspection activities for site works shall be defined in the Inspection and Test
Plan (ITP) that shall be developed detailing the specific work activities, the inspection
points and the planned inspection surveillance such as hold point, review or witness
point.

The inspections and tests must be recorded in the specific inspection forms and must
be signed by the PMC.

12.0 QUALITY SURVEILLANCE AND METHODS


The Quality Surveillance by the PMC or by its appointed shall be controlled through
the Quality Control Plan.

The Quality Control Plan shall contain the detailed list of activities involved in the site
works especially civil activities.

The Quality Control Plan also includes the corresponding inspection activities, the
reference inspection procedures or method statements and the indicated inspection
interventions such as “review, hold point or witness point”.

The Quality Control Plan shall be reviewed by the PMC and any revisions or
additions shall be incorporated prior to final approval. The PMC shall indicate on the
Quality Control Plan their “witness points and hold points”.

The Quality Surveillance shall be part of the routine inspection activities by the PMC
with the CONTRACTOR.

CONTRACTOR Quality Control Engineer shall prepare the inspection and test
reports and submitted to the PMC for review and shall be signed by PMC before the
project commence.

12.1 Project Report.

12.1.1 Work Progress Reporting


In order to keep the PMC abreast of the progress of work done, the CONTRACTOR
shall submit the following reports on weekly or monthly basis:

 Weekly Progress Report,

 Monthly Progress Report,

 Recovery Plan

These reports will state progress achieved on a particular activity/milestone,


difficulties encountered and method for solving these problems, equipment and
personnel involvement in the project for the week or month as regards to the work
scope.

12.1.2 Project Close-out Report


The Final Project Close-out Report(s) shall be prepared by the CONTRACTOR and
submitted to the PMC to reflect all available information established and collated as
defined in this Quality Plan.

The purpose of the Project Close-out Report is to ensure a collective data record of
Project activities performed during the implementation of the Project Scope of Work.
The Project Close-out Report shall contain all relevant data produced during the
implementation of the Project or reference the relevant data compiled to date.

The PMC shall ensure that the Project Close-out Report is collated and compiled with
all relevant statistical and verifying documentation and when appropriate, shall report
the relevant progress to the Project Director.

13.0 MEASUREMENT ANALYSIS AND IMPROVEMENT

13.1 Quality Audits


The QA/QC Manager shall perform the Internal Quality Audit on the project quality
system to monitor and to measure the effectiveness and adequacy of
implementation.

The scheduling of Project Quality Audits, when appropriate, shall include


CONTRACTOR audits where the scope of work warrants it.

The results of quality audit shall be recorded and brought to the attention of the
personnel having responsibility to the activity audited.

The QAQC Manager shall take timely Corrective Action on deficiencies found during
the Audit.

Follow-up audit activities shall verify and record the implementation and effectiveness
of the corrective action.

13.2 Non-Conformance Reporting.


Any non-conformance or non-compliance identified during the project shall be
documented and managed in accordance with the NCR procedure.

The PMC shall raise a non-conformance report to the CONTRACTOR. Generally,


however, they will result either from surveillance by the Quality Engineer or QAQC
Manager or from the findings of internal audits.

Corrective action to cure a non-conformance shall be planned and carried out by the
corrective discipline.

All non-conformance reports and supporting documentation will be retained as quality


records.

No work shall be corrected, rejected or used as is, without prior approval of the PMC.

13.3 Corrective Action/Preventive Action


The aim of corrective action is to prevent recurrence of non-conformances and
deficiencies.

Corrective action covers a structured and systematic handling, through all phases of
the project, of all non-conformances identified by chance, reviews, and audits, the
tracing and analysis of their causes, and the planning and implementation of
appropriate action.
