    SAFETY ALERT 12
    Fire Incident
    Where: MAC #081, Pad #04 (Man camp)
    Extent: Cabin total loss.
    A fire was noted by passing contractor employees in which the alarm was raised via radio. The camp fire brigade responded
    extinguishing the fire. No injuries occurred in this incident but it could have been more serious if it occurred at a different time of
    the day. Because of the extent of damage possible causes point to careless smoking and unauthorized electrical appliances with
    improper extension cords.

    Root Causes:
     Careless smoking by room occupants and not disposing of cigarette butts properly.
     Improper use of electrical appliances and extension cords for the design of the accommodation.
    Corrective Action:
     Training .with all camp occupants for all contractor firms. There is to be no smoking inside living accommodations. “This is
    to be covered in tool box talks, & safety orientations for new employees”.
     There will be strict safety action for such unsafe acts.
     Remove all unapproved electrical appliance & extension cords.
     Contractors to monitor and enforce this requirement for living accommodations.
