Entry Level Assignment

Directions
For the introduction of this lesson, I would like you to fill out the survey
on this webpage. On the first page of the survey, it asks you to fill in
your email which is optional, but for this assignment, please fill in your
school email. This will show me that you have completed the survey.
There are only 7 questions to fill out with the first 5 are multiple choice.
The last two questions require more thought than just a yes or no answer.
I would like honest and meaningful answers to the last two questions.
The word count for the questions are a minimum of a 100 words. If you
are not sure how long that is, please write in a Word or
Google document, and then copy and paste.

Grading Criteria 
These questions are to be filled out on your own only.
Since this survey is straight forward, this is a credit/no
credit assignment. Which means you will receive 10
automatic points for completing, or you will get a Zero for
non-completion. This is another reason for putting your
email in on the first page, so I know who to assign grades
for.
