Collision Between Vehicle Controlled
by Developmental Automated Driving
System and Pedestrian
Synopsis
 The accident happened in the 18th of match 2018 along
Tempe , Arizona, U.S.A at exactly 9:58 pm (National
Transportation Safety Board Aircraft, 2018)
 The vehicle which conducted the accident was under the
ADS control (National Transportation Safety Board Aircraft,
2018).
 It involved a pedestrian who was crossing the road
 The accident was occurred when the vehicle was at 39 miles
per hour

Analysis
The accident was caused by a skill based error.
The ADS had already detected the pedestrian5.6
seconds before the accident.
 The driver failed to control the speed instantly.
 Action was taken0.02 seconds before the accident
and the SUV was at 39 miles per hours
 The video recorded showed the driver trying to
contact the console.
 These actions show delays in action as a result of
poor skills

 The theory articulates that the brain takes time to respond to various
automatic processes
 In the case of this accident, the driver was faced by numerous
concurrent processes.
 The ADS system automatic feedback also slowed down her mental

Collision of Heavy Lift Vessel Hawk with
Unnamed Barge and Destroyer Delbert

Synopsis
 The accident happened on the 29th of March in the year 2019 at
exactly 1012 (National Transportation Safety Board Aircraft,
2020).
 The scene of the accident was in Pascagoula River at a point close
to the shipbuilding site in Mississippi (The global position of the
location is 30°20.67’ N, 88°34.24’ W).
 Some injuries were experiences while 15 gallons of BCP mineral
oil were spill on the waters
 The damage caused by the accident was$15–$20 million.

Analysis

 This accident was caused by a decision-based error


 The Hawk’s pilot wanted to dock at the Deep Hole.
 Based on his pictures of the vessel, he rejected the use of for tug
boats top control the vessel.
 This factor led to the exposure of the vessel to collision with the
Destroyer.
 When the Hawk pilot was close to the Destroyer, the pilot
decided to increase the turn speed.
 This move caused the collision.

 Divided attention causes bad decisions which may cause
accidents
 Hawk pilot exhibited divided attention. By considered the photos
and advise from the co-pilots at the same time.
 As a result, he decided to increase the turn speed.
 This factor made the collision happened.

Recreational Vessel Silver Lining

Synopsis

The accident took place on 23rd of July 2019 at 1500 hours in the
southwest part of the Hood Canal Bridge (National Transportation
Safety Board Aircraft, 2020).
It involved the sinking of the Silver Lining vessel after the pilot hit a
rock
The estimated damage was estimated to be $500,000.
There was no environmental pollution or injuries reported

 The accident was caused by a perceptual based error.


 Despite the challenge being on the port engine, the owner of this
vessel, the owner concentrated on the alarm
 After the loud band, he decided to restart the engine but it was so
late
 As a result, the vessel sunk
 This shows that he had no profession perception

Reasons Model

 The accident is well justified by the reasons theory


 This approach articulates that a person is lime to make perceptual
decisions based on the lack of understanding of a specific concept
 The owner lacked professional perceptual skills
 Thus, he ended up mitigating literal challenges and forgetting the
internal errors within the system of the vessel.

Loss of Control While Maneuvering
Pilatus PC‐12/45, N128CM
 This accident happened in Montana when the PC-12/45, N128CM
craft crushed on its way to land on the BTM runway.
 the plane had travelled from Oroville in California and was headed
to Gallatin Field in Montana before the accident (National
Transportation Safety Board Aircraft, 2011).
 The plan dropped for 2100 feet from the air and injured 13
passengers together with the pilot
 The injuries were caused by a fire explosion as well as the impact
force

Analysis

 The plane crush happened as a result of challenges in the fuel


tank (National Transportation Safety Board Aircraft, 2011).
 . Before traveling it was stated that the plane required a good
icing inhibitor to prevent the oil from condensing when the plane
had set off in the atmosphere.
 However, an analysis of the radial information revealed that the
pilot never added the inhibitors
 As a result, the plane landed with a thud while the impact caused
the fuel to catch fire. 13 people were injured by the fire and
13 impact. The plan also got destroyed
Error Chain
 The accident crashed as a result of the lack of anti-
icing in the fuel tanks.
 Notably, the plane drew the fuel from all the two
wings
 The fuel was not able to clean the wing propellers
before being drawn to the main tank
 As the oil entered the main tanks, it never gave
the plane and centrifugal force to propel it

 The plane crushed at a height of 2100 feet.
