1. Permit to Work
• Defines scope of work
• Identifies hazards and assesses risk
• Establishes control measures to eliminate or mitigate
hazards
• Is authorized by the responsible person(s)
• Communicates above information to all involved in
the work
• Ensures adequate control over the return to normal
operations

2. Working at heights
• Company operations require personnel to work at heights where the risk of
falling is inherent. As such, BP's Golden Rules of Safety prohibit working at
heights of 2 meters (6 feet) or higher above the ground unless:
• A fixed platform is used with guard or hand rails, verified by a competent
person(s), or
• Fall arrest equipment will limit free fall to 2 meters (6 feet) or less.
• Person(s) are competent to perform the task.

3. Confined space entry
• BP's Golden Rules of Safety states that entry into any
confined space cannot proceed unless:
• Permit is issued with authorization by a responsible person(s).
• Permit is communicated to all affected personnel and posted, as
required.
• All persons involved are competent to do the work.
• All sources of energy affecting the space have been isolated.
• Testing of atmospheres is conducted, verified, and repeated as
often as defined by the risk assessment.
• Stand-by person is stationed.
• Unauthorized entry is prevented.

4.BP Driving Policy
Driving is the biggest single risk to personal safety in
the E&P Business
• Vehicular accidents are a major cause of serious injuries and loss resulting in
unnecessary costs both to personnel and property. Although several factors
can contribute to vehicular accident causation, they can be grouped into:
• Operator (driver) competence and attitude
• Roadworthiness of the vehicle
• A safe road system
• BP's Golden Rules of Safety state that vehicles will not be operated unless:
• Vehicle is inspected and confirmed to be in safe working order.
• Drivers are trained and certified to operate the class of vehicle.
• Passenger number does not exceed manufacturer's design specification for
the vehicle.
• Seat belts are installed and worn by all occupants.

• Hand-held cell phones and radios are not in use by driver.

5. Energy isolation
• To protect personnel from the harmful effects of
sudden/unexpected release of hazardous energy
during servicing or maintenance of machines,
equipment and other facilities..

6. Ground Disturbance

• Work that involves creating a cut, cavity,


trench, or depression in the ground by earth
removal requires the assurance for the safety
of employees, facilities, and the community at
large.

7. Lifting Operations

To ensure safe working conditions and


practices during lifting operations carried out
within Company work sites, either by
Company or Contractor employees.

8.Management of change (MOC)
Development of a work plan that clearly
specifies the timescale for the change and any
control measures to be implemented regarding:
• Equipment, facilities and process
• Operations, maintenance, inspection procedures
• Training, personnel and
communication
• Documentation

HITRA (Hazard identification and
task risk assessment)

• three basic stages:

• Hazard identification – identifying hazards which could


cause harm.
• Risk assessment – assessing the risk that may arise from
hazards.
• Risk control – deciding on suitable measures to eliminate
or control risk.

assessed.”
