1.3. Definitions

Accident 1. Any person who requires medical attention greater than


that provided by on – site staff
2. Property damage involving an insurance claim or requiring
a police report
3. Near miss accident represents a close call. Any occurrence
that nearly resulted in an accident.

Accident Prevention The application of measures designed to reduce accidents or


accident potentials within a system, organization, or activity,

Approved Complying with written Employer Representative standards, or


where no such standard exits, complying with applicable
legislated standards.

Competent A Person who is qualified because of his or her knowledge,


training and experience to perform the work property and safety.

Confined space A space in which because of its construction, location, contents


or work activity hazardous gas, vapor, dust or fumes may
accumulate or in which an oxygen deficient atmosphere may be
created.

PM The person / Persons appointed by the Contractor., to act as


Project Manager.

Material Safety Date Manufacturer’s printed information that identifies the hazards,
Sheet (MSDS) proper procedures and emergency treatment for a particular
chemical.

Employer’s The Senior person assigned to the site responsible for


Representative representing the Employer.

Direct Supervision Where the Supervising worker is present at the job site and is
fully cognizant and in control of the activities of the less
experienced workers.

Hot Work Use of open flame, other heat sources and / or spark-producing
devices where there is a potential for explosion or fire.

Incident An Undesirable event which has the potential to cause a serious


accident.

Loss Prevention A before-the-loss program designed to identify and correct


potential cause of accidents before they result in actual injuries
or financial loss.

Non-Conformance A Failure to satisfy a requirement specified in the safety, Health


and Environmental plan. Applicable regulations or procedures.
Hazard The potential of a substance, operation or condition to cause
harm or damage.

Risk The likelihood of a hazard being realized

Risk Assessment A formal process by which hazard are identified, the associated
risks are evaluated and the necessary control to either eliminate
or mitigate the risks to an acceptable level are specified.

Regulation A rule, ordinance, law, legal regulation or such device by which


conduct or performance is controlled.

Site The entire area required for the performance of the work
including temporary working space.

Project Team The Consultant, The Main Contractor and the Sub- Contractor

Tool Box Meeting Tool Box Meeting will be conducted on-site by Contractor Site
Safety Officer with the Supervisors, Foremen and laborers to
discuss H.S.E. program issues and to provide training as
required.

Visitor Ant person temperately on the site who is not regularly involved
in the daily Site activities including delivery personnel and
guests. All Visitors shall be reported to Amana.

Worker Any person engaged in an occupation at the site,

2.0

GENERAL

2.1 Safety Policy

The contractor has a written Health and Safety Policy signed by the Engineer, which
gives out a broad out line of company’s commitment forwards the Health and safety of
its employees and to the society.

HEALTH SAFETY & ENVIRONMENT POLICY

General
This is to ensure that employees at all level are aware of their personal responsibility towards
and that the are giving the necessary information for carrying them out.

Management commitment
The management considers the safety of its employees a major part of their responsibility and its
committed in implementing an HSE system to fulfill the satisfaction of the clients/consultant.
Basically through:
• Putting safety first on the agenda of their priorities.
• Provide a safety work place, environment, facilities and equipment at site.
• Providing adequate resources for HSE programme.
• Ensuring effective supervision at work site
• Conduct site inspection to review and assess the safety performance by the senior
management Personal.
• Taking suitable disciplinary action for the serious cases / repeated violation of safety rules
procedures.

Supervisory
All level of site management must understand, maintain and improve health and safety
performance and act upon the HSE plan. They are accountable for the safety of employees
working under their supervision at all times.

Employees
• All employees will familiarized with relevant project requirements of the HSE plan and
they must co-operate with management and take responsible care to ensure that don’t
endanger themselves, follow workers and members of the public
• All employees will report any accident and damage to property or equipment to their
immediately supervisor irrespective of whether person are injured.

Work Site
• All area surrounding the work site to be maintained in a clean & hygienic condition. It will
be exercised to minimize the effect of dust, noise, vibration, chemical contamination,
water pollution, waste material segregation and disposal and protection of greenery and
trees.
• Hygienic facilities will be provided such wash room, sanitary facilities, cold water etc.
• All offices and working locations will be kept clean and tidy.

Insurance Policy
All the company, Employees is insured under workmen’s compensation policy in
accordance with country’s policy.

Responsibility for implementing this safety plan is as given below:

a) All Employees

• All Employees shall follow the site HSE Rules and regulations.

• All Employees shall use protective devices and clothing provided to protect
them from dangers of accidents and dangers of occupational diseases.

• They shall refrain from creating and hazards and from performing any act that
may hinder execution of these instructions.

• They shall not temper with any of the safety arrangements made at site

• All employees shall be responsible for the implementations of the relevant


requirements of this Health and Safety plan.

• Be familiar and show commitment to the implementation of this Health and


Safety plan.

• Alert their Manager, Engineer, Supervisor or Safety Office if any hazard are
observed in their work place.

• No worker shall carry out any work that will put themselves or other in danger.

b) Project Manager

• Responsible for the total compliance and implementation of this Health and
safety plan and held accountable for the project safety performance.
• Review and approve job procedures and risk assessments.
• Establish a good housekeeping standard and good operational practice.
• Monitor the implementation of this Health and Safety Plan on a day-to=day
basis in close coordination with the Safety Officer.
• Chair the site Safety Meetings.
• Provide the necessary resources required to implement this Health and Safety
Plan.
• Display his commitment to Health and Safety and address this site safety
issues on a daily basis.
• Recognize good safety performance from the individuals and team.

• Seek advice and assistance from the safety department and keep abreast of
changes in legal requirements and best practice.

C) Site Safety Officer

• He shall be responsible to the Project Manager for H & S support and advice,
and for promoting the continues improvement of H & S performance of the
company.

• He shall also be responsible for promoting H & S plan and procedure.

• He shall prepare a weekly Schedule of all safety activities of the project and it
will be strictly followed.

• Shall be familiar with current H & S legislation relevant to the project. Copy of
these will be made available on site for ready reference.

• Assist the Project Manager in implementing the guidelines given in the


document.

• Advice and assist all personnel on the project with the implementation of
Health and Safety plan and compliance with the relevant H & S legislation.

• He will give inspection reports indicating all the unsafe conditions noticed by
him at site highlighting the status of rectifying the same to the project
Manager.

• Plan the requirement of first Aid, Fire Fighting and Safety appliances as the
work progress.

• Investigate all accidents to find out the roof cause and to take necessary
actions to prevent its reoccurrence.

• Organize the inspection of scaffolds, ladders and power tools based on the
enclosed checklist at a frequency as mentioned therein and maintaining a
record of such inspection.

• Organize safety meetings, which will be chaired by the Project Manager, once
in every month and maintaining the minutes of such meetings.

• Providing induction programs for all the staff and workmen on their arrival at
site and organize refresher session whenever required. He will maintain record
of such program.

• Conducting risk assessment for major works based on the method statement
before they are taken up, to identify the possible hazards and precautions to
be taken.

• Organize and / or conduct safety training for all project personnel.

D) Site Engineers / Supervisors / Foremen

• Understanding this Safety Plan fully and following the same in their day-to-day
activities.

• Ensure that all personnel undergo H & S induction training before they are
deployed at the site

• Enforce adherence to safe job procedures and risk assessment controls.

• Identify weakness and deficiencies in job procedure, rectify these possible and
where necessary refer these to the Project Manager / Safety Officer.

• Incase of serious accidents or incidents, take part in the investigation team


and agree remedial measures.

• Ensure that all employees working under them implement the relevant
requirements of this Safety Plan.
• Giving Safety Instructions to their workmen on a daily basis as a part of the
job instructions. Highlighting the possible hazard of the days work and
precautions to be taken.

• Ensuring that the operative working with them are issued with necessary PPE
and they use the same.

• Keeping their work area neat and clean, especially at height, free of loose
materials.

• Promote the continuous improvement of H & S performance.

• Verifying third party test certificates while hiring cranes or any lifting
appliances for their validity.

• Preventing horseplay of workmen.

• Organizing and conducting toolbox talks to their workmen every week.

• Identifying and eliminating all unsafe conditions in their respective work area
by carrying out safety inspection everyday.

• Taking suitable corrective measures to rectify the unsafe observations pointed


by the safety officer in his inspection reports.

E) Store Keeper / Material Co-Ordinator

• Ensuring a minimum stock level of all safety appliances at stores.

• Issuing safety appliances on a daily basis to the technicians as authorized by


the site engineers, supervisors and Safety Officer and maintaining record of
the same.

• Obtaining test certificates while hiring cranes and while receiving and lifting
tools and tackles and maintaining record of the same.

• Ensure that proper housekeeping is maintained in site stores.

• Ensure that proper arrangements are made for storage of highly flammable
material. Appropriate warning and Safety signs will be ensured at such places.

• Ensure and maintain proper fire fighting equipment in store area.

• He shall arrange to remove all accumulated scrap and waste material from site
regularly.

4.0

SPECIFIC

ARRANGEMENTS

4.1 Safety Meetings

Safety Meeting will be conducted monthly. The Project Manager along with Project
Engineer shall chair the meeting.

Objective

The objectives of the meeting are:

• To review the implementation of this Safety Plan.

• To create a safe environment, to review the safety performance on site, to ensure


all work activities are carried out in a safe manner.

• To find solutions for any safety issues remaining unresolved during the previous
month.

• To identify the problem areas and to work out suggestions for the same.

• To brief on accidents occurred during the previous month, if any, to communicate


lessons learned.

• To decide any special safety requirements to be taken during the next month
arising out of the new activities to be taken up during that period.

Participants: -
The Project Manager and the senior staff member nominated by the Project
Manager shall attend the meeting.

Responsibilities: -
Site Safety Officer will be responsible to record the minutes and to follow up the
implementation of the actions agreed. The minutes will be circulated and it will
also be maintained at site for audit purpose. Copy of minutes will be circulated to
all.

4.2 Personal Protective Equipment (PPE)

• Company will issue the required PPE to all workmen employed, dispending on
their work activities. All the workmen employed at sites will be required to use
safety shoes, safety Helmets and coveralls (on which the name / Logo of the
company will be prominently displayed). It will be the responsibilities of the
Respective supervisor / Site engineer to ensure that the operative’s worker under
them is issued with necessary PPE.

Color coding for reflected Safety Jacket will be as follows:

Foremen & above : Orange Jacket

Lead Man : Yellow Jacket

Site Works / Operations will be periodically reviewed to determine the requirement of
any additional Safety requirements.

The Storekeeper will maintain a minimum stock level of PPE and an issue register will be
maintained.

Ear Muffs and Ear Plugs All noisy work areas and while working on noisy
machines.

4.3 Safety Violation

All site activities will be monitored regularly and any violation to this safety plan
shall be recorded. The site management shall deal with any safety violation noted. The
procedure shall be as follows:

i. First Violation The Violators shall be given a verbal Warning on the spot
itself and penalized. The violation will be recorded.

ii. second Violation If he repeats the violation, he will penalized double the
amount of First violation and will be made to attend the Safety Instruction
Training again

iii. Third Violation He will be given written warning and Dismissed / Suspended
from the site with a note to the AHO Safety Department and Personal
Department. He shall be given adequate safety training by HO before
deploying him again any site.

In case of a violation from staff, the procedure shall be as follows:

 First Violation Warning letter shall be issued to him the PM and copies to be
send to Safety Department
 Second Violation Appropriate fine shall be imposed upon him by the Project
Manager. Warning letter shall be issued to him with a copy to Personnel
Department.
 Third Violation He shall be sent to HO for counseling with Divisional Manager.

4.4 First Aid and Welfare facilities

First Aid

 Trained first-aides will be made available at site



 They will be identified with proper stickers on their Hard Hat.

 First aid boxes will be made available at site office and stores where workers
can have an easy approach.

 Injured persons at site will be evacuated with the help of facilities available at
the site.

 All major injury cases will be taken to the Govt. Hospital after necessary first
aid.

 Ambulance facilities of the Police Department will be called to handle major


cases.

 All first aid cases will be recorded for investigating.

Welfare Facilities

• Cool drinking water will be made available at site.

• Rest area provided at the site will be used for lunch, tea breaks and for taking
rest.

• Rest area will be adequately ventilated lit and cleaned on a regular basis.

• No food articles will be allowed in construction area.

• Appropriate bins will be used to dispose the food waste.

• Toilet and wash facilities available at will be used.

• Sleeping and taking rest in other areas will not be allowed.

• Separate smoking area will be earmarked if found necessary and provided with
adequate extinguishers after consultation with main contractor.

4.5 Work Control Measures

In a construction site there is a need for written safe systems of work in specific
circumstances. A Permit to work system should form part of that “Safe System” Permit
to work must only be issued by an appointed authorized person, using duplicate and
numbered forms. They must be signed by authorized person and by the competent
person who is not carry out (or be responsible for) the work described on the permit.

Permit to work system are essential to ensure safe working and freedom from hazard
where a high-voltage electrical supply, cables and requirement exits, particular in
installation, maintenance or construction work. Entry to such area will be strictly
controlled. All energized rooms will be have proper lockable doors. The following work
control measures will be followed at site:.

a) HV / LV – Plant Room Work Permit

While taking up any activity in energized electrical, plant or mechanical room, the
concerned engineer will issue a permit in the enclosed format to the concerned
supervisor / foreman (competent person) with a copy to the site safety Office indicating
all the precautions to be taken. LOTO procedure will be followed during such activities.
Appropriate warning signs will be ensured at all necessary places.

b) Working in Shaft

Any work in shaft will be controlled. The concerned supervisor has to fill up details of
work and ensure that the precautions mentioned therein fulfilled.

C) Hot Work Permit


Any gas cutting or welding work can be taken up only after obtaining a hot work permit
in the enclosed format from the site Safety Officer / Concerned section Engineer. It will
be the responsibility of the concerned supervisors to obtain the hot work permit from the
Site Safety Officer or from their Engineers and to arrange all the precaution detailed in
the permit before starting the work.

4.6.1 Housekeeping

Housekeeping is an important element of accident prevention. If should be the primary


concern of all supervisors and fill the end of the job they shall monitor it the following to
be ensured.

• Proper access and egress shall be maintained at all times at site and it shall not be
blocked.

Out with permission only. The removed protection shall be replaced after the work
is over.

• Information posters will be put up in appropriate locations including the emergency


telephone numbers and fire points.

• No smoking signs and hot work prohibition signs will be installed wherever required.

• Suitable safety posters and sticker will be arranged and displayed from time to time.

• Emergency telephone numbers will be displayed on all the notice boards.

• Emergency procedure will be displayed at appropriate places.

4.7 Safety Inspection and Safety Audits

• Project Manager shall conduct a site safety inspection at least once a month. This
may be undertaken in conjunction with an inspection by the site Safety officer.

• All the site supervisors shall be responsible for conducting routine H & S surveillance
and taking appropriate action as they go about their day to day work.

• The site safety officer will inspect the entire site weekly or as and when needed.
Report shall be given to the Project Manager.

• The respective engineer, supervisor or the foreman will attend to all unsafe
conditions pointed out by the site Safety Officer. Project Manager will be responsible
to ensure the corrective and preventive action on the inspection points.

• Sr. Safety Officer from The Company will inspect audit the site and incorporates any
additional requirements in the safety in the plan to strengthen the accident
prevention efforts.

• All the site supervisors will inspect their site and brief the employees of the hazard
related to the job.

5.0

TRAINING

5.1 Induction Training

• All staff and technicians will go through Safety Induction Training before their
deployment at site.

• The record will be maintained to this effect.

• It will be the responsibility of the respective engineers to inform Site Safety Officer
about arrival of any new employees.

• Subject to be covered during the induction sessions are as per the Annexure 11.1

• A “Zero Tolerance” culture will be imbibed during the induction to all the employees.

• All visitors will be required to meet the secretary or concerned person at The
Company site office, if any of the visitors have to be taken to site, i.e. construction
area, it is approved by the Project Manager and they will be required to give
undertaking (Annexure 2 ) before taken inside the site. Copy of the format will be
available with the Secretary or concerned person at the site office. They shall be
provided Safety Helmets and Safety Shoes. All visitors shall be reported to Amana
prior to enter to the site.

5.2 Tool Box Talk

• Tools box talks will be conducted at least twice in every week by all the site
engineer / supervisors to their respective workmen before the shift begins.

• Project Manager shall issue a schedule of the toolbox talk program. In addition to
this, depending upon the site activities in progress, job specific hazards and
precautions to be taken will be highlighted wherever required. Unsafe conditions
and unsafe acts noticed at site, lessons learnt from the accidents, safety
precautions to be taken in the next weeks work etc, will be the focus of discussion
during the toolbox talks.

• Site Safety Officer shall issue the necessary TBT subjects to front line staff.

5.3 Job Specific Training

• Any specific training needs of site based personnel for any specialized activities shall
be organized by the HO Safety Dept. on request from the project Manager.

5.4 Daily Work Instructions

• The daily work Instruction by the supervisors and site engineers will include the
existing hazards and their precaution. If a new job is being started, the engineer /
Supervisors must ensure that the toolbox talk about the job is carried out to all
operative.

5.5 First Aid Training

• Ho Safety Dept. will organize First Aid Training for the site nominated personnel
through certified and approved third party agencies. Necessary ID stickers will
provided to the trained first aides and will be identified to Co-Contractors also.

ACCIDENT

6.1 Reporting Procedure.

Accident Prevention is a part of planning and controlling functions and it will help
management to reach the goal of “Zero Accident”. The following procedure shall be
followed at site. It has been designed to satisfy both statutory and corporate
requirements.

• All accidents and near miss incidents at site will be recorded and all these cases,
including first aid cases will be investigated.

• The individual providing the first aid will maintain a first aid register at site.

• All lost time injured and property damage exceeding QR 500/- will be reported to
the Engineer / Project Managers and HO immediately through a fastest medium and
a detailed report will be sent within 24 hours of the incident (Annexure-1)

• A Suitable follow up report shall be sent to the main contractor and HO regarding
the status of the injured at a later stage getting necessary information from the
Hospital / Doctors.

• The site safety Officer will investigate all accidents including the near misses and
first aid cases. The investigation report, if required will be submitted to HO Safety
Officer.

• All serious accidents will be reported to the Govet. Authorities / Police and the scene
of accident will preserved until the Police investigations are completed.

• Incase of a serious accident the emergency procedure (Annexure – 12) will be


followed without fail.

• Project Manager, to ensure that the Emergency Procedure is followed.

7.0

HAZARD
ANALYSIS
&
PROCEDURES

7.1 General

Based on the Hazard Analysis and Generic Risk Assessment, the following “Safety Work
Procedure” has been developed. There control measures apply to general circumstances.
Site specific details have to be incorporated to these procedures before carrying out any
activities.

Utmost care has been exercised while conducting the hazard analysis. This is based on
the date currently available. Certain additional environmental hazards may be
encountered depending upon the actual field conditions while executing the work.

Depending on the progress of the project. Planning schedule such as monthly, weekly
and daily, all critical jobs will be analyzed to identify the hazards likely to be encountered
during the execution and the precautions to be taken will be decided.

Activity specific Risk Assessments for new activities will be prepared by the site Safety
Officers based on Method statement and the Safety Precautions suggested by him shall
be implemented by the site engineers while taking up the said activity.

Before taking up any major lifting operations, a detailed lift study will be carried out and
a method statement will be peppered incorporating all the safety requirements by the
respective senior engineers.

7.2 Setting up site Facilities: Officers, Welfare & Storage.

Hazards:

a) Fire

b) Trespassers.

c) Exposure to hazardous substances.

d) Collapse of structures

e) Hygiene related hazards.

Procedures:

• Setting of temporary building will be planned to ensure stability and availability of


utility services (Water, drainage & Electricity). Loading and off- loading of office
will be planned to ensure a suitable crane. Lifting equipment and trained
personnel are available. Fire extinguishers will be ordered as required prior to
occupation of facilities and supplied to site before work starts.
• First-aid facilities will be provided as required by assessments. Toilets, welfare and
hygiene facilities will be arranged suitable for likely numbers of employees using

requirements of the various regulations, especially in regard to control of
subcontractors and liaison with there contractors. Office workers and operatives
will be trained in emergency procedures.

• The area underneath the portable site officers will be entire closed or maintained
free from any disposable wasted that can cause fire hazard.

• Dustbins will be provided in the office, stores and rest areas to facilities the
disposal of waste material .

7.3 Working at Height

Hazards:
a) Fall of persons

b) Fall of materials

c) High wind hazards

Procedures:

• Work is planned to ensure a safe means of access is provided. All equipment


provided will be maintained properly.

• Suitable protection will be erected all opening and edges where fall of more than 2
mtrs could occur. Safety harness will be provided for all operatives who work at
height ( 2 mtrs. Or more) when no positive fall prevention can be provided.

• Where edge protection is removed for access, or is not practicable, operatives


working at or near the edge will wear safety harnesses.

• All equipments used will be checked to ensure iy is in good order, to correct


specification.

• Work will be monitored to ensure that additional precaution and equipment is


taken into use if edge protection is removed.

• Training and instructions must be provided to all operatives and supervisory staff
involved in the use of lines and harness and how to inspect and access PPE of this
type before use.

7.4 Use of Mobile Scaffold Towers

Hazards:

a) Fall of persons

b) Fall of Materials

c) Collapse of Tower

d) Overturning of Tower.

Procedures:

• Only authorized personnel will erect, modify or dismantle scaffolding towers.


Towers should be not be specified for in, the vicinity of overhead power lines,
Specification for use of towers scaffolds will take in to account, the site ground
conditions, height restrictions and obstruction.

• Tower will be erected by trained personnel in accordance with relevant standards


and manufacturers instructions. Ladder access should be internal and fixed to the
narrowest side. Maximum height to base ration will be no be exceeded 3.5:1
inside and 3:1 external use without ties. Ties will be exposed or windily conditions.
All tower platforms will be fully boarded and fitted with toe boards and guardrails.
Wheel will be locked when the tower is in use. Personnel and materials will be
removed before the tower is removed. Manufactures advice on maximum loading
will be adhered to.

• All scaffolding will be inspected before use. After alterations or adverse weather
conditions authorized personnel must inspect scaffolds. Tower over 2 mtrs in
height must be inspected every seven days. A compartment person will carry out
all scaffold inspections. Scaffold will be rechecked regularly to ensure their correct
use and that unauthorized adoptions have not been made.

• Person erecting and inspecting scaffolding must be adequately trained. Training


may be provided to such people and it shall be recorded scaffold-tag system will
be followed at site.

• Safety harness will be provided for all operatives who work at height (2 mts or
above where no positive fall prevention can be provided.

7.5 Use of Ladders

Hazards:
a) Fall of persons from ladder

b) Slipping of ladders

c) Objects dropped by ladder user.

Procedures:

• Tower ladders will be checked to ensure correct length, type and condition before
use.

• Ladders are subject to the planned maintenance program.

• Ladders work is restricted to light activities and for a short duration.

• The ground base for ladder use must be firm and leveled.

• The ladder must be sufficient length to extend 1.05 mtrs above the step off point
(loading Platform) when used as access to a scaffold.

• The correct angle of the ladder to rest is 75 degrees or a base to height ratio of
1.:4

• Ladders must be secured against slipping by tying at the top or at the bottom.

• Over-reaching from ladders will be avoided.

• Metallic ladders will not be used in the jobs where it is likely to come in correct
with live electrical parts.

• Ladders without non-slip base or hinge locks, where applicable will not be allowed
at the site.

• Supervisors must check ladders before use to ensure they are safe and in good
conditions

• Use of ladders will be monitored regularly, to ensure that the operatives are not
over-reaching.

• Damaged ladders will be removed from the workplace immediately.

• Painted ladders will not be accepted for use.

• All ladders brought to the site will be registered and appropriate number will be
given to ensure that they are inspected at regular intervals.

• All operatives will be trained for safe use of ladders and hazards during the
instruction training.

7.6 Working on Shafts

Hazards:

Procedures:

Entry inside any shafts will be authorized by the concerned engineer or supervisor after
he inspects the place of work by means of the checklist.

If the any of the specific requirements given in the checklist could not be complied for
any reasons, the matter shall be referred to the concerned sr. engineer or site Safety
Officer to find a suitable alternative for complying with the same.

Safe scaffolding / working platforms shall be ensured to carry out the work inside the
shaft.

Access and exit route to the working platform inside the shaft shall be clearly defined
and communicated to all concerned.

7.7 Use of Portable Electrical Equipment

Hazards:
a) Electrocution

b) Fire

c) Damage to operatives

Procedures:
• All portable electrical equipment will be indicated individually and is subject to
planned maintenance. Equipment supplied to site will be in suitable condition
for its purpose with regards to voltage, power and environment conditions.

• Equipments found to be detective would be switched off and reported


immediately. Visual inspection of the equipment will be carried out before the
use leads and extension cables are to be routed so as to minimize the like
hood of damage and trip hazards.

• Only molded socket (Industrial plug and socket) holders will be used on sites.

• Only equipment operating at 110 volts re 220 volts with necessary ELCB

• The use of electrical equipment will be monitored to ensure safe use.

• Site management will ensure that only trained competent person test repair
and maintain portable electrical equipment.

• Project manager shall a person to inspect / check all portable power tools
before releasing it to the site use.

7.8 Hand tools

Hazards:

c) Slipping and falling due to the out tools.

Procedures:

• It is to be ensured that the tools issued are suitable for the purpose and in
accordance of environment in which they are used.

• Eye protection is to be provided and used wherever work is done using cold
chisels, drills, grinders or other tools where there is a risk of flying particles of
pieces of tools breaking off.

• Open bladed knife screwdrivers and other shape tools are to be carried and
used with precaution so as not to cause injury to the user or others.

• Insulated tools must be used where there is possibility of live electrical work.

• Site management will monitor hand tools which can deteriorate with use and
also ensure that the correct tools are being used properly.

• Specific checks will be made as follows:

place. Supervisors. Will ensure the use of safety guards of the tool during
operation.

• Home made, make shift hand tools fabricated at site will not be allowed. Any
such tools of site will be removed and destroyed.

• Trays or bungs will be provided where necessary beneath containers to


prevent ground communication. Handrails will be fitted to storage areas where
persons could fall more than 2m

• Stockpiles and storage areas will be inspected regularly to ensure stability and
to ensure that the above physical precautions are in places. Verbal instructions
and training will be given to operatives as necessary to ensure good
housekeeping standards are maintained on site.

7.13Material Handling

7.147.13.1 Manual Handling

7.15 Electricity At Work

Hazards:

Procedures:

ALL CIRCUITS TO BE WORKED ON WILL BE TREATED AS LIVE UNTIL VERIFIED DEAD,


THERE IS NO EXCEPTIONS TO THIS REQUIREMENT; EXPERIENCE OF EMPLOYEES IS
IRREVERENT.

Before any operation or work at site commences ad during progress, the contractor shall
take all practicable steps to prevent danger to persons employed from any live cable and
apparatus.

Wherever possible, live work is to be avoided. Where love work is required, a safe
system of work is to be devised, preferably written and that would be by use of permit to
work system.

Necessary protective devices such as Earth leakage Circuit Breakers shall be provided in
the circuit to avoid risk of shock. Fire and burns.

Access to live conductors is to be controlled and appropriate signs are to be in place.


Written information and instructions will be required for work on complex systems
(control, meeting & parallel circuits). A clear access of 1m, gloves and matting are to be
provided for live working. Electrical test equipment will be insulated, fused and

calibrated. Electricity supply authority sends will not be broken and final connections will
not be given without authority.

Live work is only to be carried out by authorized competent electricians under direct
supervision of nominated supervisors. Electricians will not be permitted to work
unaccompanied on live connections above 125 volts unless specifically authorized to do
so and good communications are in place.

Adequate PPE, first aid and qualified first aides are to be made available at workplace
where the live work is to be done, if live work is not covered by a permit system, then
switching off must be accompanied by a physical lock – off and appropriate signage.

Electrical Section In charge to ensure that only qualified and competent persons carry
out electrical work.

7.15 Work In Confined Spaces

Under the confined spaces regulations, a Confined Spaces” can be either.

• A Place which is substantially, through not always entirely, closed or

• A place where there is a reasonably foreseeable risk of serious injury from


hazardous sustenance’s or conditions within the space or nearby

Some confined spaces are easy to identify, such as closed tanks, vessels and ewers.
Others are less obvious but may be equally dangerous, such as open topped tanks and
vats, silos or constructions that become confined spaces during their manufacture. Some
places may be confined space only occasionally, perhaps due to the type of work to be
undertaken, such as a room during paint spraying.

Hazards:

a) Poisoning from toxic gases b) Asphyxiation (Lack of Oxygen)

b) Explosion d) Fire

e) Excessive heat f) Drawing

Procedures:

Eliminate need for entry or use of hazardous materials by selection of alternative


methods of work or materials.

Permit to work system will apply in confined spaces. Adequate ventilation will be present
or arranged. The area will be tested before entry and continually during the presence of
persons in the confined space. Emergency rescue harness. Lifting equipment,
resuscitation facilities, safety line shall be provided where necessary.

A communication system with those in the confines space must be established.


Necessary PPE and hygiene facilities will be provided for those entering sewers.

Site management is responsible to decide on nature of the confined space and to put a
safe system into operation including checking the above. Flood potential and isolation
must be checked.
Apart from the above, site specific method statements will be prepared before allowing
such works.

7.16 Work in and With Excavation.

a) Collapse of sides.

b) Persons falling into excavations

c) Plant and materials falling into excavations.

d) Flooding of excavations.

e) Presence of hazardous atmospheres

f) Presence of contaminated soil.

Procedure:

Sufficient numbers of trained operatives and competent supervision must be available


before work start. Sufficient and suitable plant must be available for trench support
before work start. Suitable monitoring, equipment and personnel trained in its use will be
required where known exposure to toxic substance or lack of oxygen may occur.
Location of existing service must be complete before work start, also information
obtained on ground conditions.

Sides of excavation greater that 1.2 meters in depth, must be supported. Where flooding
risk exists, cofferdam / caissons will be installed with pumps of suitable capacity.
Sustainable barriers will be erected around excavations greater than 2 meter deep.

Stop barriers will be used to prevent vehicle entry. Soil and materials will be stacked at
least 1.5 meters from the edge of excavations. Ladders will be provided for safe access /
egress. Cable location and local authorities’ drawings will be used to trace buried services
prior to commencement of work. Suitable sign and barriers will be provided to warn of
work

Supervisors to ensure safe system of work provided taking account of prevailing


conditions including weather, traffic and existing structures. Provide suitable PPE as
required and ensure correct use. Inspect excavation daily or before each shift over 2
meters deep. Ensure personnel selected are capable and experienced unless under direct
supervision.

7.17 Working inside Manhole

Some manholes may become confined space only occasionally, perhaps due to the type
of work to be undertaken or the location of it. In such cases the following procedures
shall be follows.

Hazards:

a) Poisoning from toxic gasses

b) Asphyxiation (Lack of oxygen)

c) Excessive heat.

d) Falling men / materials

Procedures:

Surrounding areas of the hole has to be barricades. Proper signage / warning lights shall
be placed warm. If required a guard shall be placed with flag to warn vehicle.
Proper and correct key shall be provided and used to open / remove manhole covers.
Both portion of the manhole cover (if manhole cover is of two parts) shall be held with
key to prevent one falling inside the manhole while other portion is being removed.

Operative to follow the material handling procedure to prevent any lack injury while
removing the manhole cover.

The manhole has to be kept open for sometime to let fresh air in. if required fans shall
be used to give fresh air.

Eliminate entry into manhole if the work can be carried out from outside.
Necessary PPE and hygiene and hygiene facilities shall be provided for those entering
sewers.

Apart from above, area specific method statement will be prepared before allowing such
works. Work notification shall be obtained to carry out such work wherever necessary.
Necessary TBT shall be given to those operatives who have to perform work in manhole.

Supervisor shall be preventing during the operation.

Necessary communication facility shall be made available if required depending on the


nature of work.

All materials and equipment will be removed from the work area once the work is over
and all barricade and signage shall be removed.

7.18 Use of Disc cutters and Abrasive Wheels

Hazards:

a) Flying particles during a grinding.

b) Bursts or disintegration of grinding wheels.

c) Contact between a revolving wheel and a person’s hand or some part of the
body.

d) Contact with power.

Procedures:

Many of these injuries are relatively minor nature but some prove fatal. Strict compliance
with the requirement of the Provision and use is essential in reducing the risk of these
injuries.

Sufficient operatives trained in use of abrasive wheels and discs will be available at the
workplace. The correct wheels for the type of machine, speed and material to be cut will
be ordered and supplied.

A nominated electrician will inspect power tools once in a month. He shall check the tools
as well as power cord, plug and socket for correct fitting.

Supervisors shall ensure the required PPE i.e. ear, eye, head and foot protections as
appropriate for the work and machine are issued and used by the employees. Operatives
will not wear loose clothing. Disc cutters will only be used when standing on a firm and
level base. Operators will visually check equipment and discs / wheels for damage before
use, ensuring operating speed is indicated. Users shall not use undue pressure on the
equipment.

Suitable storage facility will be available at the workplace for spare discs and wheels.
Equipment and spare wheel are to be checked for visible signs of damage before issue.

Selection may be required for operatives who have experience of the work and are
physically fit.

7.19 Control of Substances Hazardous to Health

In construction, use of limited number of group of materials are made, which are in the
COSHH category of hazardous substances, including:

• Solvents ( Paints)

• Fibers ( Glass fiber Insulation)

• Dusts ( Silica from aggregates)

Procedures:

Prior to starting work with LPG or any compressed gas cylinders, quantities will be
estimated to ensure adequate storage facilities will be available. Liaison with main
contractor and co-contractors will be maintained to ensure storage is adequate. The
minimum required quantity only would be help on site.

Cylinders will be stored upright in an open mesh, lockable container away from the
buildings, drains and excavations. Only cylinders connected to equipment. Will remain in
work areas. Cylinder will be kept away from flammable materials and heat sources.
Adequate ventilation will be provided in areas where LPG is in use. Where LPG is used, a
dry power fire extinguisher will be available. Empty cylinder will be treated as full, except
in storage where they will be segregated. Storage will be signed with appropriate Safety
signs and warnings. It will be stored away from direct heat.

Managers / Engineers / Supervisor / Storekeeper will ensure that storage facilities are
adequate and are maintained to the specified standard. During their site walk around
they will check to ensure LPG equipment is being used property, cylinders not is use are
removed from the workplace, fire extinguisher are present and storage areas are in good
order. Hot work using LPG must be inspected at the end of work in order to minimize the
risk of fire.

Operatives shall be provided with training on fire precautions and use of fire
extinguishers.
