BƯỚC 1: XÁC ĐỊNH TẦN SỐ TIM VÀ TÍNH ĐỀU ĐẶN
• Tần số tim
Nhịp tim bình thường: 60-100 lần/phút; nhịp tim nhanh >100 lần/phút; nhịp tim chậm < 60 lần/ phút

Dương ở DI, II, avF, V4-V6; âm ở avR; hai pha ở V1 DII

0 đến 75 độ, cách xác định tương tự trục QRS
BƯỚC 3: KHOẢNG PR
Tăng khi tần số tim chậm và giảm khi tần số tim tăng
Sóng S: lớn từ V1 đến V2 sau đó giảm dần từ V3 đến V6. Diễn tiến này có thể thay đổi do phì đại
thất hay nhồi máu cơ tim
BƯỚC 4: HÌNH THÁI PHỨC BỘ QRS
• Thời gian
0,07-0,11s (0,06-0,09s). Kéo dài khi block nhánh,
phân nhánh, chậm dẫn truyền trong thất
Thời gian nhánh nội điện: là thời gian dẫn truyền từ
nội mạc đến thượng mạc, đo bắt đầu phức bộ QRS
đến đỉnh sóng R; < 0,045s ở V5-V6 Thời gian nhánh nội điện
Cách tính trục nhanh: chọn chuyển đạo nào có phức bộ QRS gần triệt tiêu, trục QRS sẽ vuông góc chuyển
đạo đó. Sau đó để ý phần dương hay phần âm ưu thế sẽ cộng hay trừ 10-15 độ là ra được trục QRS
BƯỚC 5: ĐOẠN ST
ST: pha tái cực thất (tái cực sớm pha 1,2), bắt đầu từ cuối phức
bộ QRS (điểm J) đến bắt đầu sóng T, bình thường ST đẳng điện
ST có thể chênh lên hay chênh xuống nhẹ <0,5mm so với đường
đẳng điện, trừ chuyển đạo V2-V3 có thể chênh lên < 2mm ở nam
>40 tuổi và <2,5mm ở nam < 40 tuổi và <1,5mm ở nữ.
ST có thể chênh lên 1-2mm ở chuyển đạo V3-V5 ở người trẻ như
trong trường hợp tái cực sớm, ngực lõm, phức bộ QRS dạng rSr’ Điểm J
ST có thể chênh xuống dạng chếch lên < 0,5mm ở người lớn tuổi
phụ nữ mãn kinh, khi gắng sức hoặc stress tình cảm. ST chênh
xuống khác nhau ở người bình thường và người bệnh mạch vành
khi làm trắc nghiệm gắng sức.
Theo định nghĩa toàn cầu lần thứ 3 về nhồi máu cơ tim, đo ST
chênh lên hay chênh xuống từ điểm J
