Issue details:
For the installation of gully (domestic drainage) activity an area was excavated ahead of schedule, during the excavation the KAHRAMA
MV cable was partially exposed. On 06 January around 4PM the site foreman assigned a carpenter to mark the center point of the gully
with a nail. While marking the center point with a nail the worker hammered a nail in the cable which resulted in damaged to the
KAHRAMAA MV cable.

Consequences/Impact:
The consequences that may be incurred are as follows:
• Immediate consequence is the damage to underground utilities which has led to work stoppage.
• Possible commercial impacts due to the rectification of the damages

Lesson Learned:

 As per the QCS 2014 Section 1 Part 10.5.4 & 10.5.5- For all excavation work a competent
excavation supervisor will be appointed and Prior to excavating in any area and the location of
existing utilities shall be confirmed from as-built drawings, contact with utility authorities and trial
holes.
 As per the QCS 2014 Section 1 Part 10.5.4 -Marker posts or blocks that clearly indicate the nature,
depth and path of all underground utilities.
 Safe working procedures around exposed utilities need to be communicated in a manner that the all
workers and foremen understand.
 A supervisor/foreman should always be in attendance to ensure a continuous guidance at site while
working near exposed utilities.
 Competence of the operatives must be reassessed to ensure that they clearly understand the
requirements of such an activity.
 Disciplinary actions to be taken if the human error/factor is obvious during accident root cause
analysis.
 Ensure that approved INR is available at site prior to start any activity.
