Derailment at NJP: KIR Div
• On 14.06.2023 at 11:10 hrs, NJP central panel reported
that BOXN L no 22072215331 (28th from Train Engine)
of train no. DN CHLT Bolder got derailed front trolley all
wheel near point no 262 at NJP yard.
• At 11:35, front portion cleared at NJP on line no 12.
• At 12:32, UP line clear
• At 13:25, restoration work started and completed at
14.16 hrs
• Prima Facie Cause: Uneven loading

Derailment at SNCA: RNY Div
• On 07.06.2023 at 14.45 hrs, while the train UP STAR
Coal through pass SNCA Station, it got derailed at KM
111/4-5 after passing Advance Starter between SNCA-
BOKO by 21 wagons blocking main line.
• No Causality and No injury.
• Track fit for Line No. 1 given at 08.30 hrs of 08.06.2023.
• PF Cause: Track Defect.
• JAG Inquiry under progress

Derailment at AZA: RNY Div
• On 08.05.2023, the train no. UP BOBYNL stock was stable at line
no. 03 and it was programmed to shift the load from line no. 03 to
line no. 07 at AZA yard.
• As per shunting programmed, the loco no. 70142/70162 hauled 25
loads and move forward KYQ end and during pushing back the
load towards AZA end the wagon no. 701208/01870
ECoR/BOBYNHS and 780321/10684 NR/BOBYNHS (14 th and 15th
from train engine) got derailed at leading portion of point no.
139.
• The wagon no. 701208/01870 ECoR/BOBYNHS derailed by all
trolley all wheels while wagon no. 780321/10684 NR/BOBYNHS
derailed by one trolley all wheels.
• Dragging about 62 metres.
• PF Cause: Uneven loading.

• On 18.05.2023 at 02:25 hrs, at Jorai (JOQ) Yard Siding line,
during shunting of BOBYN E/S (Load-54, Loco No 12477) all
wheels of wagon no. NCR BOBYN 78132010747 (12th from
SRPB end) got derailed ahead of point no. 17/17X on line no 5
(JOQ Yard Siding line) and subsequently 17/17X point
obstructed.

• Rerailment completed at 06:23.

• TC given by SSE/P Way/KAMG at 06.50 hrs

• Prima facie Cause: Uneven unloading

• On 24.05.2023 at 15:10hrs, during shunting of Loco No
40090WDP4D of Train No 05694/93. from line No.1 to line
No.2, the said Loco got derail near crossing body of Tumbler
Point (Front trolley 2 wheels).

• Loco no 40090 WDP4D re-railed at 20.48 hrs.

• Prima Facie Cause: Point No K-1 was in favour of L/1. During


shunting from L/2 to L/1, the engine overshoot the point
without setting in favour of L/2. As a result, two wheels of front
trolley derailed over point due to wrong shunting.

30079383803 SE/BCN (1st from Loco), rear trolley LHS axle box housing disk
24-05-23 displaced & grease oozing out and it is unsafe to run. Said wagon detached at

KIR hrs as rolling out staff reported that long pull rod found hanging condition. On

broken and dropped down & also brake beam broken one side and touches

01-06-23 B/Van), automatic twist lock defective which temporary rectified with M.S.
wire and train fit to run up to NJP with 30 km/h speed restriction.

Train no. DN BCN E/S, Loco no. 32087, train detained after departed at
NJP yard from 08:20 hrs as rolling out staff noticed that CBC yoke
supporting plate of rear CBC hanging condition in w/no. 30079660768
SE/BCN(19th from Loco) which rectified by C&W staff/NJP after back to

controlled at TIHU at 23:46 hrs as SM/KTCH reported sparking. On
checking found c/no. 145266 NFR/LWSCN(8th from Loco), one steel
mounting bracket of rear trolley RHS Bio toilet was bent and
RNY 13-05-23 touches rail on run which secured by LP, GD and P/man. [P/M-
DBRG][S/M-LGH]. [BPC no. 1223376, issued by DBRG/NFR, Date-13-
05-2023]

 Train no. 15656 UP (SVDK-KYQ EXP.), train detained at NOQ from


06:51 hrs as passenger reported that smoke came out from a coach.
On checking found c/no. 205257 NF/LWACCN, smoke noticed from
APDJ 09-06-23
HOG transformer of Net-1 feeder was burnt. Net-1 feeder was
disconnected and power supply was given through Net-2 feeder.

28-05-23 wheel of some BCN E/S are messed up with the RE wire. Track clear

 Train no. 15604 DN (LEDO-GHY EXP.), train stopped at km 243/3-7


LMG 30-05-23 between RXR-DSR at 23:55 hrs as RE wire hanging over the track &
wire in contact with train engine. Track clear at 02:03 hrs.
 Train no. DN JRNA/SC, train detained at NKMG as RE wire found
05-06-23 hanging condition between KGBC-NLBR. TC given at 05:50 hrs.
 Train no. 07527 UP(, train detained at PNB as OHE wire in hanging

Buffalo Dashing Case: RNY Div
• On 01-05-2023 at about 09.05 hrs, LP of 15930 DN reported to
SS/BDLN that three numbers of buffaloes knocked down by the
said train at km 353/5-4 in between DMC (Dhemaji) – BDLN
(Boginodi).

• Front feed hose pipe of the Train Engine had cracked along with
brake pipe of two coaches, NFLS 195715 (2nd from Train Engine) and
LSCN 197979 (6th from Train Engine), broken.

• After rectification, train cleared section at 10.23 hrs.

• Resulting, 15896 DN detained at DMC for 01 hour 3 minutes.

• The above site is not identified as CRO site.

• On 14.05.2023 at 00.35 hrs, ESM/JMI informed that vertical weld
failure occurred at km 320/8-9 (LHS) over main line of Jamuguri
(JMI) yard.

• TC given at 01.45 hrs with speed restriction of non stop 30 kmph.

fracture occurred at NMY IOC Siding line at Km 16/2-3
between NGC – NMY.

• Year of Laying: 2016

Brake Binding & Wheel Skidding Case: RNY Div
• On 03-05-2023, Train no- DN DCCK CONT (Loco- 28267/27714, Load- 45+01)
arrived NBQ at 02.20 hrs and while conducting rolling in examination by C&W
staff noticed unusual sound from one wagon & sparking.
• After stopping the train, on duty JE/C&W/NBQ along with staff attended and
found W/no. 62259971647 CNCR/BLC (17th from Loco)both trolley in brake
binding condition, metal deposited in all wheel & all wheel in skidded condition
& DV previously in isolated condition.
•  Accordingly detachment memo served to SM/NBQ at 03.25 hrs after deposited
metal removed from wheel.
• Train finally left with 41 loads from NBQ yard at 11.25 hrs or 09 hrs 05 mints
detained at NBQ

Chip (ex- DTC, loads 21+1) at BPB, the train
manager served memo to on duty Dy.SS/BPB
at 17.00 hrs that suspected flat tyre of
wagon no. SR BCNAM1 300698/65050.
• SSE/C&W staff attended and found that flat
tyre over wheel no. 01 (65 mm) and wheel
no. 2 (70 mm).
• The flat tyre may be due to the train running
with invalid BPC due to breach of rake
integrity by more than 4 wagons.

• On 06.05.2023, train manager of train no. DN BXG Fly Ash (Loco
no. 49131/49144) ex- PKB, served memo at about 13.10 hrs that
wagon no. ECR BOXNL 70027 was having inflated body and
also was in tilted condition.

• TXR staff attended the said train at PKB at about 14.40 hrs and
after checking served memo to detach the culprit wagon.

• Train finally left PKB at 17.47 hrs.

Tree Fallen Case: LMG Div
• On 10.05.2023 at 13.20 hrs, train manager of train no. 22503
reported that that tree fallen over track at km 171/9-172/0
between Lamsakhang (LKG) – Hawaipur (HWX) due to heavy
storm.

• The fallen tree got entangled with the OHE wire and post which
were also in hanging condition.

• Track fit certificate given at 18.36 hrs.

• Resulting 11 (eleven) trains detained.

Improper Rail Loading Case: APDJ Div
• On 22-05-2023 at 12.30 hrs, train no. DN GLMA Rail (Loco- 12179,
loads-14, BPC-22.05.2023 EE, Loaded ex-NBQ) arrived Dangtol
(DTX) and on checking found that flap doors were not available at
both ends and rails were also not properly secured.
• At 16.50 hrs, C&W Staff from NBQ attended and verbally allowed
to run the train in same condition.
• Train left DTX at 17.45 hrs and again controlled at KOJ for further
checking and proper lacing of the rails.

● Date & Time : 29.05.2023 at 11.35 hrs.
● Section: Gomoh - Dhanbad
● Location: Between Nichitpur - Tetulmari stations; at Km 283/14-16.
● Brief: RVNL contractor's labourers were working in mid section on down line
between Nichitpur and Tetulmari stations without taking any block and without
informing railway officials. During Mast erection work, suddenly OHE mast fell
down on OHE wires near Engineering Gate No. 7C/E, at km 283/14-16. As a
result, six persons were electrocuted badly. No train was involved in this
unusual incidence.
● Casualty/ Injury: Killed -06 (All labourers of the contractor)
● Restoration time: UP line Dhanbad - Gomoh OHE fit at 13.05 hrs; DN line
Gomoh - Dhanbad OHE fit at 13.43 hrs.

duty Pointsman reported that its one Coach (GS-SR 081208; 3rd from rear)
caught fire.

● Brief: While train no. DN DDL Goods was passing through Khajraha Home
signal, Guard reported smoke in wagon. Same informed to Loco Pilot and
Dy.SS/Khajraha. Train controlled at Khajraha loop line. Checked by crew and
found wagon floor sheet caved in & was rubbing with wheel in trailing trolley of
4th wagon from Brake Van. Guard demanded TXR.

Manned Level Crossing Accident in Adra Division /SER
● Date & Time : 06/06/2023 at 16:45 hrs.

● Section: Adra - Gomoh.

● Location: At Manned Level Crossing Traffic Gate No. AM-20 of Santaldih


station yard.

● Train No: Dn 22812 (New Delhi - Bhubaneswar) Rajdhani Exp.

● Load: 17 Coaches (LHB)

● Engine No.: 30211/WAP-7/GZB

● Brief: While Dn train No. 22812 (New Delhi - Bhubaneswar) Rajdhani Exp.
was passing through Santaldih station, the train engine and its adjacent 3
coaches got scratched with the rear portion of a tractor trolley which was stuck
on Dn line inside the MLC Traffic Gate No. AM-20. This MLC Gate was in
closed position. No derailment took place.

● Casualty/ Injury: Nil. 91


Manned Level Crossing Accident in Adra Division /SER (contd..)
● Final cause and responsibility:
i. While closing the gate, the Gate man on duty failed to observe that the
tractor trolley was trapped inside the gate boom, and transmitted the key
to SM on duty for lowering Dn Home Signal.
ii. The Gateman failed to take any action like putting back of Dn home
signal, placing of banner flag across the down track of approaching train
and running towards the approaching train with the red flag to draw the
attention of engine crew for stopping the train, as per GR 16.07.
iii. Gateman failed to observe the provisions of para 7 of Gate Working Rule
(GWR)of Gate No. AM-20.
iv. The driver of tractor trolley fled the spot leaving his tractor trapped inside
the gate without informing any Railway personnel, violating section 153,
174 (b), and 159 of Railway Act.

● Action taken: Gateman removed from service with immediate effect. 92

Unusual Incidence of Rolling down of Monsoon special in Khurda Road
Division / ECOR
● Date & Time: 07.06.2023 at 16.30 hrs.

● Train No: Monsoon Special (Stabled in Iron/Ore Dust Siding)

● Load: 33 BOX/N (Loaded: 31; Empty: 2)

● Brief: Train No. Monsoon Special, a load of 33 BOXN, was stabled on


Iron/Ore Dust Siding, at Jajpur station from 09.06.2022. Suddenly, the rake got
uncoupled by 15 wagons & rolled down towards dead end. In this unfortunate
incidence, some trespassers who were taking rest below the rake, ran over by
this train and some sustained injuries. No obstruction to any running line. No
derailment took place.

● Casualty/ Injury: Killed-06; Injured-02 (All trespassers) 94


Unusual Incidence of Rolling down of Monsoon special in Khurda Road
Division / ECOR
● Prima facie cause: There was a thunderstorm at 16.15 hours. There was one
stabled rake of monsoon reserve on ORE siding. Some contract labourers
were unloading panel rails on UP main line. To take shelter from rain, they
moved underneath the stabled wagons. Slum dwellers residing adjacent to the
siding used to go underneath the wagons to do their morning toilet activities. It
is presumed that they operated the handle of 14th wagon by which the train
got uncoupled and due to wind and vibrations of unloaded panel rails, moved
towards the dead end at a speed of 5 to 7 kmph. No derailment occurred. Due
to labourers taking shelter unauthorisedly and unsafely under the wagons in a
cramped space, when wagons rolled down they were unable to come out as
broken concrete pieces are lying adjacent on one side. The labourers tried to
move out from the opposite side only which created further congestion.

Derailment of Passenger Train on Narrow Gauge over Salem

coach is detached and traction is changed from Diesel Engine to Steam
Engine. Due to heavy rain, passengers were deboarded from the coaches.
After attaching Steam Loco, empty rake without passenger was moved to
check for wheel slipping. Meanwhile, one empty coach got derailed by rear
bogie two pair of wheels.

got derailed by rear truck leading pair of wheels. No obstruction to through
traffic.

● Date & Time: 10.06.2023 at 21.29 hrs.
● Section: Midnapore-Kharagpur.
● Location: At Kharagpur station Yard at km. 116/12.
● Train No: 38830 (Midnapore - Howrah) Dn EMU Local.

● Brief: While train No. 38830 (Midnapore - Howrah) Dn EMU Local was
entering Kharagpur station yard, its one coach No. SE 12079 (6th from rear)
got derailed near Inner Home Signal no. S-42, at Km 116/12.

● Restoration time: 01 hour & 39 minutes (23.08 hours of 10.06.2023).

● Prima facie cause: Five Nos. grazing marks found on north side rail table and
also the hitting marks on the top of the trolley frame and coach, there might be
some foreign metal object which hit the coach trolley and came between the
north side wheel and it caused a lurching of coach by which leading wheel off 98

Wagon and passed the Advanced Starter signal No. S-8 of Sihora Road
station at 'On' position and stopped at km 1026/17-19.

Container Derailment Case in ECR
• On 22.05.2023 at 03.45 hrs, while train no. UP Goods
MJCG (Loco- 27076) was running on 3rd line between DCX
– KMS stations, it was stopped suddenly at km 639/21 at
KMS Station due to 02 wagons got derailed at km 640/5-
7 and Container fallen down at Km 633/29 in R/Line.
• DN line movement started from 7.34 hrs and R/Line at
08.47 hrs.
• No casuality, no injury.
• Prima Facie Cause: Central Automatic Twist lock of
Culprit wagon (no. BLC – 61401821533) was not in
position on both side, i.e., not in position to enable
locking. This resulted in shifting of both containers from
center position of bogie platform during run and led to
the derailment.

SPAD resulting to Side Collision in ER
• On 10.05.2023, there was a side collision in ER near Bardawan
station between 37784 Memu & goods train resulting
derailment of two coaches of Barddhaman - Bandel EMU local.
• As per initial reports, Dn Barddhaman - Bandel EMU local while
entering Shaktigarh (SKG) station at about 21:16 hrs, two of its
coaches (from front i.e.Howrah end) got derailed.
• The Goods train was diverted from DN M/L to DN HBC at SKG
Home signal. As per motorman of 37784, the train was
diverted from DN HBC to DN/ML at SKG home signal but he
could not follow home signal properly due to obstruction of
the goods train. As a result EMU local derailed at the crossing
point and side collided with the goods train.
• No injury or casualty.
