• Key of meningitis
1. fever
2. stiffness of neck เกิดจากมี meningeal irritation
3. signs and symptoms of increased intracranial pressure (มีมาก
น้อยขึ้นกับเชื้อ)
4. symmetrical cortical involvement; ในกรณี ที่เป็ น complication of
meningitis สามารถมี secondary stroke จาก arteritis เชื้อที่ common
คือ TB, syphillis เลยอาจจะมาเป็ น asymmetry ได้ แต่ใน meningitis ธรรมดา
มักจะมาด้วย symmetrical symptoms

Terminology
• 2. encephalitis, cerebritis
คือการอักเสบเข้ามาใน brain parenchyma สามารถมาได้ท้ งั focal, multifocal,
diffuse ขึ้นกับเชื้อและตำแหน่งที่ involve ถ้าการอักเสบมาถึงชั้น meninges ก็จะ
เรี ยกว่า meningoencephalitis อาการ increased intracranial pressure
ขึ้นกับเชื้อว่าตัวไหนมีอาการมากน้อย
Terminology
• 3. brain abscess
กลุ่มนี้มาเป็ นก้อน มีการ form capsule ดังนั้นจะใช้เวลานานกว่า meningitis,
encephalitis แต่ในบางเชื้อที่การดำเนินโรคเร็ วก็สามารถมาแบบ early acute ก็ได้
เช่น staph., gram negative สามารถมาได้ท้ งั แบบ solitary mass,
multiple ก็ได้ แล้วแต่เชื้อเช่นกัน

1. Increased intracranial pressure; ปวดหัวโดยเข้าได้กบั secondary headache

Seizure อาจจะมี increased intracranial pressure และถ้าก้อนอยูท่ ี่ cortex ก็ชกั ได้
• ในคนไข้ที่มี brain abscess จะต้องถามหา risk อื่นๆ ที่ท ำให้เกิด abscess เช่น otitis media,
previous skin infection or brain surgery, immunocompromise

Acute meningitis and encephalitis
• Bacterial meningitis is most common
• Caused by hematogenous spreading
• Spread of organisms through veins from infected sinuses,
middle ears or mastoid
• Surgical procedures, head trauma
• Causative agents; most common; group B
streptococcus( streptococcus pneumoniae)
• Other organisms; gram-negative bacilli, listeria
monocytogenes, Neisseria meningitides (มองหา hemorrhagic
purpura)
Acute meningitis
• Duration < 4 weeks
• Acute; bacteria ( strep., N. meningitidis, H.
influ, L. monocytogenes)
• Acute; virus (enterovirus, herpes, arbovirus,
HIV)
• Aseptic meningitis (autoimmune SLE, toxin;
IVIg, CA; lymphocytic carcinomatosis)
Acute meningits
• History of meningitis with sinusitis, otitis or
mastoiditis; strep sp. Anaerobe, staph. Aureus, H.
influenzae, enterobacteriacae
• Post neurosurgical procedure and head trauma;
staph.aureus is most common, gram neg bacilli,
anaerobe
• Post splenectomy; S. pneumo.
• Neutropenia; pseudomonas. Aeruginosa and gram
neg bacilli(E.coli, Klebsiella sp.)

NSAIDs

acute febrile neutrophilic dermatosis (Sweet's
syndrome)
– Acute phase of viral infection

• Acute lymphocytic meningitis; viral infection, Listeria
monocytogenes
• Viral meningoencephalitis is the most common cause
• WBC count 10 to 1000 cells/mm3, but it is usually less
than 300 cells/mm3 (mononuclear cells).
• PMN leukocytes during the first 48 h of disease,
particularly with infections caused by enteroviruses,
herpes viruses, and arboviruses
