 Injuries from Dropped object ( RRWC)

On 2 Feb’23 at 4.15 pm. Stud bolt, sizing 1-1/2
inches, length 250 mm., weight 2.55 kg., fell from the
Platform at the second manhole of tower A-3401,
approximately 27 meters high from ground while box
up to closing the manhole by insert a new bolt, The
operator closing the manhole does not see the
inserted old bolt because it was the bolt that the
previous operator had locked the wooden ring ,The
new operator then pushes the new one bolt goes into
the flange. suddenly the old bolts that hold fall from
the flange Falling into the space between the grating
and the column to the bottom flew over to hit the
scaffolding and crashed into the worker's helmet the
worker he standing as below. The distance from the
tower was about 6-7 meters. He was injured had
wound at the head and left shoulder and then the
Supervisor taken him to Rayong Hospital. The doctor
performed 3 stitches on the head. Xray results
showed no fracture.

 Root Causes :
 1.Does not takeout the old bolt because this bolt for locked wooden
ring.
 2. The new operators team to perform to close the manhole and
previous team not communicate during hand over task before change
to new location.
 Preventive measures :
 1. Set up operator team 1 team per 1 location.
 2. Provide check sheet for inspection during disassemble and assembly
control by Supervisor.
 3. Close space or gap between grating and column by net or canvas.
 4. Use rope to tie the wooden ring instead bolt.
 5. Provide barricade with sign and do not allow unauthorized persons
into the area during working at height.

 Hazard Elimination:
 When workers are finished with the tools and materials that they are
using, they should be properly stored and put away. This is one
simple way to keep dropped object injuries or fatalities from
happening. If there is a way that you can easily keep people safe,
you should take advantage of the opportunity. All hand tools shall be
use safety lanyard.
