    1 August 2012 Ms. ARLENE P. MANCERA JOMED Gen. Merchandise Naga City Dear Madam: We are happy to notify you to that your Bid dated 27 July 2012 for the procurement of Medicines of the LGU of Camaligan, Camarines Sur for the Contract Price equivalent to Ninety-Nine Thousand Eight Hundred Ninety-Nine Pesos and fifty centavos (PhP 99,899.50), as corrected and modified in accordance with the Instructions To Bidders is hereby accepted. You are hereby required to provide within ten (10) days the performance security in the form and the amount stipulated on the instructions to bidders. Failure to provide performance security shall constitute sufficient ground for cancellation of the award and forfeiture of the bid security. Also, please accomplish and sign CONTRACT of AGREEMENT form in four (4) copies found in your Bidding Document and have it duly notarized. Very truly yours, EMMAUEL T.     The head of the procuring entity, through the Procurement Unit/Office, immediately upon approval of the recommendation for award shall issue the Notice of Award to the bidder with LCRB.
