MECHANISM OF ACTION
INDICATION
CONTRAINDICATIONS
ADVERSE EFFECT insomnia, urticaria, tachycardia, bradycardia, leukopenia, thrombocytopenia, gynecomasti a, hepatitis
SIDE EFFECTS CNS: vertigo, malaise, headache. EENT: blurred vision. Hepatic: jaundice. Other: burning and itching at injection site
NURSING RESPONSIBILITIES
Histamine H2Antagonists inhibit action of H2 BRAND NAME: receptor Zantac which are found on the gastric CLASSIFICATION: parietal cells and prevent Histamine-receptor gastrin antagonist secretion, this Anti-ulcer decreasing acid production. DOSAGE: 50 mg
Post surgery antacid to prevent ulcer formation while the patient is on NPO.
Contraindicated in patients hypersensitive or drug or any of its components and those with acute porphyria. Use cautiously in patients with impaired kidney function, gastric ulcers and immunocompromised patients
1. Assess patient for epigastric or abdominal pain and frank or occult blood in the stool, emesis, or gastric aspirate. 2. Check drugs dosage properly before giving to be accurate in administering drug and ensure the safety of the client. 3. Before giving the drugs, obtain the history to previous used and reaction to drug. It is contraindicated to the patient hypersensitive to the drug. 4. Monitor vital signs. 5. Obtain history of existing diseases. This drug is contraindicated to pts. with acute porphyria. 6. Explain the purpose of the drug to the patient and its side effects so that the patient will be oriented as well as to gain trust and cooperation. Inform the patient that it may cause drowsiness or

dizziness. 7. Give drug on time as prescribed for better drug effect and to be able to assess drug action. 8. Instruct the patient to report any unusualities will occur to give immediate intervention. 9. Ask patient other prescription medications, nonprescription medications, herbs, and supplements you're taking, like antacids, diazepam, warfarin, Procainamide etc. Combining certain medications may alter their actions or produce unwanted effects. 10. Instruct pt. to don't smoke because it may increase your stomach acids and worsen the disease. 11. Give the missed dose as soon as possible. If it's almost time for the next dose, skip the missed dose and take the next dose as scheduled. Don't double the dose. To continue treatment as physicians doctor, and avoid over dosing of drug. 12. Check IV patency to ensure the drug is properly administered and to prevent discomfort in administering the drugs. 13. Ensure patients safety to avoid further complications or injury. DRUG
MECHANISM OF ACTION Synthetic compound with direct trichomonacidal and amebicidal activity as well as antibacterial activity against anaerobic bacteria and some gramnegative bacteria
INDICATION
CONTRAINDICATIONS
ADVERSE EFFECT CNS: seizures, dizziness, headache GI: abdominal pain, anorexia, nausea, diarrhea, dry mouth, furry tongue, glossitis, unpleasant taste, vomiting Hematologic : leukopenia Skin: rashes, urticaria
SIDE EFFECTS Dry mouth with strange metallic taste, Nausea and Vomiting and Diarrhea
NURSING RESPONSIBILITIES
GENERIC: Metronidazole
Contraindicated in patients hypersensitive to the drug. Should only given for life threatening infections when other antibiotic treatment is ineffective.
1. Check drugs dosage properly before giving to be accurate in administering drug and ensure the safety of the client. 2. Before giving the drugs, obtain the history to previous used and reaction to drug. It is contraindicated to the patient hypersensitive to the drug. 3. Monitor vital signs.
4. Monitor neurologic status during and
after IV infusion. Inform physician if seizure, dizziness or headache occur. 5. Explain the purpose of the drug to the patient and its side effects so that the patient will be oriented as well as to gain trust and cooperation. Inform the patient that his urine will be darker than usual. 6. Give drug on time as prescribed for better drug effect and to be able to assess drug action.

7. Instruct the patient to report any unusualities will occur to give immediate intervention. Advise patient to report onset of black, tarry stools; fever, sore throat; diarrhea; dizziness; rash; confusion; or hallucinations. 8. Give the medication exactly as directed evenly spaced times between dose. Do not skip doses or double up missed doses. If a dose is missed, give as soon as remembered if not almost time for next dose. 9. Check IV patency to ensure the drug is properly administered and to prevent discomfort in administering the drugs. 10. Ensure patients safety to avoid further complications or injury.
