Affidavit of Two Disinterested Persons

We, MECHE ALFECHE, Filipino citizen, of legal age, and with residence address
at Barangay Tuyan, Malapatan, Sarangani Province, and EARL RAY-NONN L. ICAT,
Filipino citizen, of legal age, and with residence address at Barangay Poblacion, Kiamba,
Sarangani Province after having been duly sworn in accordance with law, hereby depose
and state:
1. That We personally know Ms. Helen Ysah a. Paidumama being the Election Officer II
of Commission on Elections - Kiamba, Sarangani Province;
2. That We worked as staff at the Office of the Election Officer - Commission on
Elections - Kiamba, Sarangani;
3. That on May 19, 2013, the Office of the election Officer Kiamba, Sarangani
Province was robbed by unidentified person/s and the Computer Monitor with serial
no. CNC913PWEV and CPU with serial No. SGH9390047 that serves as Voters
Registration Machine were among the items taken from said office in the course of
that robbery;
4. That until at present the lost computer monitor and CPU Voters Registration
Machine remains unrecovered;
5. That We are not related by consanguinity or affinity to Ms. Helen Ysah A.
Paidumama; and,
6. That We are executing this Affidavit to attest to the truth and veracity of the foregoing
facts and in compliance with law.
