    Dear Sir,
    I am an Advocate and a Tax-Payer, based in
    Multan, practising throughout country. I have applied for
    a new telephone connection vide Registration No.
    MTI/5720 dated 27.06.2000 at my newly constructed
    house, 6-Ahsan Colony, Multan. Photocopy of the receipt
    is attached for ready reference. I wish to shift my family
    from my previous residence to above-mentioned one.
    It is therefore, requested that your goodself may
    direct or order to the concerned staff for installation of
    telephone at my new residence.
