    AFFIDAVIT OF LOSS

    I, Eleuterio Baccol Mabao, Jr. 20 years of age, single, out of school youth and a resident of Sampaguita Extension, Green Heights New Town, Mayamot, Antipolo City, after having duly sworn to in accordance with law, hereby depose and say; That, I graduated at Mayamot National High School and issued a Good Moral Character and form 137 for purposes of my plan to enroll in college, 2 years ago, but due to financial problem I was not able to do it, instead I look for a job in order to earned money . That, at present I am prepared to enroll college, but only to find out that my documents are no longer available. That, I and my parents had exerted efforts to locate the same, but no to avail. That, I believe the said documents was misplaced when we transfer our other personal belongings to our second residence at Pasig City last September 2010. That, I executed this affidavit to attest the truthfulness of the foregoing facts and to prayed for the school authorities to re issue the said documents needed in my desire to enroll college education. IN WITNESS HEREOF, I affix my signature hereunder this______ day of April 2012 at Antipolo City.
