0 EXECUTION METHOD STATEMENT

Preparatory Works

1. A site visit shall be performed to understand the scope and to identify any issues that may
come up during the repair works (Operations & and MIS Team).
2. Perform level II Risk Assessment; & obtain approval from Petrofac.
3. Perform detection of underground cables/ facilities routed in the area where excavations are
to be carried out. Identify cables routed below the area to be excavated.
4. Barricade area with adequate signage.
5. Obtain Permit for the work. Ensure all materials and arrangements are in place for the work
to commence.
6. PA shall conduct Tool Box Talk. This shall include a briefing on the TRA; the days work
scope, roles and responsibilities of the crew. The discussed points shall be recorded.
7. Confirm all control measures identified are in place prior to start of work.
8. Prepare 3 sample cubes (150 X 150 mm) for the mixed concrete for testing purpose.
9. QC documentation shall be maintained for all repairs.
10. The following documents shall be attached to the permit application.

1 In case of a gas leak stop all work & equipment, move personnel out of the
area, inform control room and proceed to muster point.
2 Radio communication to Control room
3 FIT (first intervention team) shall be informed by AA
4 Any emergency stop work and evacuate area
5 In case of adverse weather (heavy wind/ rain), stop work.
