1. SCOPE

 The Contractor shall provide materials, labor, equipment and supervision required to
accomplish the requirements at the chipping and restoration works activity. This
scope of work is to check area/ location and size of concrete block out/ pipe to be
chipped based on the construction plan and approved shop drawings.

3. WORK METHODOLOGY

 Prepare al the approved materials and equipment needed for chipping and
restoration works.
 Preparation of area concrete to which mortar will be applied shall be newly exposed
concrete free of loose and unsound materials.
 Verify field measurements is as shown on the drawings:
 Execute work in strict accordance with the best practices in a through
substantial workmanlike manner by competent workman.
 All installation shall meet or exceed minimum requirements of the standard
and codes.

 FIRST PROCEDURE – Mark the slabs/ walls to be chipped using chalk.


 SECOND PROCEDURE – Cut the marked slabs/ wall using concrete cutter.
 THIRD PROCEDURE – After cutting, chipped off the cut portion manually using
chipping gun and grinder.
 FOURTH PROCEDURE – After chipping the desired size, install utility box/ junction
box for power and lighting lay outs.
 FIFTH PROCEDURE – Restore the perimeter of the sleeve or floor drain body by
concrete pouring at the floor level.
 SIXTH PROCEDURE – Clean the area/ location of activities including areas where
the concrete/ debris fall.
 SEVENTH PROCEDURE – Hauling of concrete debris and dump it properly.

4.0 MAJOR PLANT, TOOLS AND EQUIPMENTS

 Grinder with concrete cutter


 Scaffolding/ Ladder
 Safety Signage or Caution tape and other safety requirements
 Materials Specification
 Concrete cement and sand ( as per concrete specification )

5. SAFETY PRECAUTIONS

 Safety Education
 The workers will be advised to wear and/ use PPE’s, hard hat, vest, safety
shoes, eye spectacles, googles, gloves and dust mask prior to the
chipping works. Appropriate signage will be posted on the areas covered
by the chipping works in order to inform the other workers doing their own
work on the specified areas
 During Inspection
 Contractor’s safety engineer will visit the site frequently and check that all
precautions are taken prior and during working hours. Proper
housekeeping is also observed before and after the activity. Safety officer
will ensure that all personnel, equipment, work permit, PPE’s are in
compliance with the safety standards.
 Scaffolding and Ladder
 Scaffolding are generally classified by the loading of work platform with
light, medium or heavy design platform. Platform load 25 (122), 50 (224)
and 75 (336) psi – (kg/m2). The requirements are applicable to all scaffold
work platform:
a) A guard shall enclose all working platforms more than 10 ft. (3m)
above the ground or floor level.
b) Guardian shall be located 42 in(1050mm).
c) Work platform shall be completely decked with no openings.
d) Adequate access to each work platform shall be provided.
e) If ladder are used they should be securely tied to scaffold.
f) Planks should be fastened, secured and kept clean.
g) All materials use for scaffolding shall be periodically inspected to
confirm condition satisfactory for confirm used.
h) Suitable footing shall be provided to prevent uneven settlement and
loading of vertical support of scaffolding.
i) When erecting or using rolling scaffold, caster wheels shall be
locked in the position if adjusting screws are used. They should be
installed only at the base plate any tool or materials on scaffold to
move shall be secured.
j) Straight or extension ladder shall be use at an angle of seventy five
(75) degrees with horizontal of four is to one (4-1) and the top
extending at least 3 ft. (900mm) above the points of top support
and securely tied at the point.

 DUST CONTROL
a) Implement a suitable program for dust control in and around the project site,
designed to reduce dust generation/ distribution to reasonable levels.
Coordinate with environmental program.
b) The principle of dust control is to reduce dust generated at source.
c) When carrying out activities which will generate dust, isolation of work area is
preferable.
d) Demarcate the work area and put up warning notice at dusty zone.
e) Reduce time exposure inside the dusty zone. If not possible compulsory
wearing of breathing apparatus is needed.
f) Frequent watering is required on site for dust suppression.
g) Materials (e.g. cement) which will generate dust should be handled carefully
and proper dust mask must be used.
h) Supplementary equipment such as water facilities and dust absorbers should
be used in activities like grinding, drilling etc. which generate dust; workers
should wear approved breathing apparatus.
i) All kinds breathing apparatus should be in proper type.
j) For the sake of environmental protection and public concern effective dust
control and monitor programmed should be adopted to maintain the dust level
at an acceptable standard.

6. SAFETY PRECAUTIONS

 It shall be implemented to protect the public from hazard, connected with


construction operation and activities.

REQUIREMENTS:
 Standard Warning Sign
 Safety Net/ Inverted Umbrellas
 Exhaust/ Ventilation fan for dust control

All these items shall be installed as required by the government regulations and by
construction standards.

 Inspection shall be conducted and advice to proceed when all requirements are
complied with the following requirements will be examined but not limited to: warning
sign and barricades, exhaust/ ventilation fan fire extinguisher, PPE’s and etc.
 Removes damaging using for function other that informing the public towards
ENVIRONMENTAL HEALTH and SAFETY be subjected for disciplinary action
stated in penalties and violation.

7. QUALITY ASSURANCE / CONTROL

 When the chipping and restoration has been completed, the MDC QA/ QC Engr.
Inspector shall be invited for Visual Inspection of the areas. The chipping and
restoration shall be made according to procedure mentioned above.
 If the chipping and restoration results show that the finished output is not well done
in a satisfactory manner and providing the requirements, the MDC inspector decide
whether this is due to incorrectness or faulty work by the contractor and if this be the
case the contractor shall, when called upon, carry out at this own expense such
alternations, replacements and adjustments as may be requires by MDC inspector
and complete satisfaction. The MDC inspector and QA/QC engineer of MDC
decision as to what constitutes a satisfactory test shall be final.

8. RESPONSIBILITIES

 The MDC QA/QC Engineer/ Inspector shall be invited for Visual Inspection of the
areas. The chipping and restoration works shall be made according to approved
methodology of works.
 The contractor’s Project in charge, Field Engr., Safety officers will communicate
always to the MDC Personnel/ Inspectors regarding on the work activity on site for
them to check or monitor if it is done properly.
 The QA/QC Engr. Will invite the MDC QA/QC Engr. To inspect the activity (chipping
and restoration) if it is done correctly. When it is found out that the contractor did not
follow the proper and approved procedure/ methodology, the expenses on the
alteration, replacement, and adjustment as may require by the MDC Personnel will
automatically followed and it is charges to the contractor.
