Greetings!

This is to inform your good office that the recently opened business of Miss

Glenda B. Rivera with the trade name G.B. Rivera Enterprises was previously

presented at Bureau of Internal Revenue last February 2016 before the tax

mapping of businesses occurred. I disclosed this to the Tax Division Office in the

person of Miss Rose Ann Quinto and discovered that there is no lease of contract.

Lacking the knowledge that it is one of the requirements, she returned my

application for COR and authority to print receipts.

I inquired with the caretaker of the building to issue a lease of contract for

my application, but unfortunately they told me that the owner is now on vacation

abroad and they need to send the contract there for his signature. Thats why my

application was delayed.
