JOINT AFFIDAVIT OF CONSENT

We, FERDINAND SANTOS MAGBITANG and MARIE PAZ DE LEON

HERNANDEZ-MAGBITANG, both of legal age, Filipino, married, and presently
residing in Barangay San Juan, San Ildefonso, Bulacan, after being duly sworn to
in accordance with law do hereby depose and say:

1. That, we are the parents of FRINZ MARTIN HERNANDEZ MAGBITANG

born on March 1, 2015 in Barangay San Juan, San Idefonso, Bulacan;

2. That, we exercise joint parental authority over our son FRINZ MARTIN
HERNANDEZ MAGBITANG;

3. That, we both consent to the application of Philippine passport of our

son FRINZ MARTIN HERNANDEZ MAGBITANG;

4. That we are executing this affidavit in connection with our childrens

application for passport at the Department of Foreign Affairs and/or
travel clearance from the Department of Social Welfare and Development
for their travel abroad.
