I am Rey Vincent R. Coladilla from SE 3202 of the Batangas State University – Main II Campus, under
the program of Bachelor of Science in Sanitary Engineering, and I am currently doing an assessment
paper regarding the PD 856: Chapter 19 – Nuisances and Offensive Trades and Occupations of any
establishments / businesses / and occupations as per accordance to the Sanitation Code of the Philippines
(PD 856). This is to comply with the final requirement of our subject course: EnSE 304: Public Health
Engineering.
In line with this, I am seeking for your help in conducting the assessment by allowing me to observe and
to ask queries regarding to your establishment’s:
1. Sanitary Permit;
2. Requirements of your personnel
a. Health Certificate;
b. Personal Health and Hygienic Practices;
c. Personal Protective Equipment;
3. Sanitary Facilities Requirement; and
4. Safety Requirements;
Moreover, may you also allow me to take pictures for the purpose of documentation. Rest assured that the
information and data that will be gathered will be of educational purpose only and will be treated as to the
highest confidentiality.
