This is in reply to your Management Observation Memorandum No. 003 s. 2018. I sincerely
apologizing for the non- submission of my Certificate of Trainings as required in the
Memorandum dated April 25, 2018. I failed to submit such documents because I assumed that
my sole training last March 2-31, 2013 for Wellness & Livelihood Skills Training for Women is
not acceptable because the said training is over 5 years older. But now, I attached the certificate
for such training for your validation. I promise that this won’t happen again. And I want also to
let you know that I am very much willing to undergo trainings if you required me for the
betterment of our office. I am hoping for your usual kind and consideration. Thank you!
