STEP 1
 Present your Form 138- A / Card to
the Enrollment In-Charge in the Table
 Fill-In the Forms

STEP 2:
 Proceed to the School Clinic In-
Charge Ma’am Heddy Arendain for
determining your Height, Weight and
BMI.

STEP 3
 Proceed to the Prefect of Discipline
 Present your Brigada Slip
 STEP 3-A
(GRADE 7 and Transferees only)
 See Ms. Lorraine Joyce Lagos for
 Reading Status

STEP 4:
 Submit your enrollment form
and pertinent documents to the
EMIS-In-charge Maam Moth Sapac
with your documents HS Card
(Certification of Good Moral
Character) or GMC for Transferee.
