Tuberkulosis – A Continuing Problem In Indonesia

Tuberculosis (TBC) is an infection disease that happen in human respiratory tract, which
is caused by bacteria. The history of TBC in Indonesia started since pre-independence era which
is in the colonial government of dutch era. The government forming a specialized institution to
handle the problem about TBC called Stichting Centrale Vereniging tot Bestrijding der
Tuberculose (SCVT). Post-independence, on the 1969-1970 period as the marker of modern era
that controls TBC in Indonesia, namely the revision of TBC patient management guidelines and
the establishment of a special government agency that handles the problem of TBC (National TB
Manager). On the 1977-1995 introduced nationally 2 regimens for TB therapy, that is conventional
therapy and short-term therapy (Short- course chemotherapy). On the 1995, DOTS strategy model
(Direct Observed Treatment Short-course) began to be introduced in Indonesia. On the 1995-1999,
DOTS strategy was done gradual expansion to every puskesmas in Indonesia. On the 1999, formed
a national integrated movement (Gerdunas TB) which is has a main policy, is to implementation
of DOTS strategy throughout Health Service Unit. On the 2000-2005 period, strategic plan of TBC
was arranged as reference for district and province in the planning of TB programs whose focus is
intensification of DOTS strategy with quality improvement for the first time. Meanwhile, on the
second strategic plan of TB period on the 2006-2010 was focused on consolidation and
implementation of innovation of DOTS strategy. Despite various strategies to overcome
tuberculosis, TBC remains a continuing problem in Indonesia.
Tuberculosis has become one of the most dangerous diseases in the world. In 2007,
globally there were 9.27 million incident cases of TB, 13.7 million deaths from TB in HIV-
negatives and 0.45 million deaths in HIV-positive persons. This data always gets increase time by
time. Now in the worldwide, each year we can find 8 million new cases of TBC. In 2015, the cases
of TBC reach 10,4 million increase from before just 9,06 million cases. As a global health
emergency, TBC is find in Indonesia too. Indonesia was claimed have the second most cases of
TBC in the world after India. Until now, TBC still become the most important problem of health
in the world. Based on the data of Health Minister of Indonesia, TBC cases in Indonesia on 2003
is 627.047 sufferers and on 2004 there is 539.000 new cases and 101.000 deaths from TBC.
Between 2013-2014, new cases of TBC in Indonesia was 403/100.000 inhabitant, while the
prevalance number was 660/100.000 inhabitant. Because of all data, Tuberculosis was claimed as
the first infectionus disease that declared as a global health emergency.
In indonesia the problem faced by society about tbc (tuberculosis) disease is the way of tbc
treatment and tbc prevention. but very difficult because in indonesia it self tbc patient rate
uncertain in every agency so very difficult to determine medication pattern and prevention of
disease. but there are two supporting things help the community in the first treatment and
prevention efforts that is by searching tbc patients in the community and treat it.but a new problem
arises that patients feel healed when they take their drugs immediately dismiss the treatment so
that there is drug resistance and tbc relapse when tbc drug should be in consumption for 6 months.
Because of this problem people find thinking about home treatment with the supervision of his
family so that the treatment can run well, effective and efficient. Family also indirectly prevent the
spread and infected tbc disease so the patient can recover and tbc disease in Indonesia can be
reduced from the previous number of patients.
Indonesia as a developing country is being active in planning the prospects for sustainable
development. The consequence of this health problem is less attention because of government
focus to the development. Tuberculosis is a serious disease that should not be underestimated
because the number of deaths from the disease is classified as many in Indonesia. But in reality
the handling is still very less and not evenly distributed.The lack of government attention to
Tuberculosis sufferers is still very low. Viewed from health services provided either preventively
or curatively. The services provided by the government itself are still less evenly distributed. One
of the causes is the extent of the State of Indonesia to reach. Equity itself is still less when in terms
of conditions where inland communities are difficult to reach, it is itself more need more adequate
health services.Public awareness to pay attention to health is also seen less in Indonesia. The lack
of education gained by the people of Indonesia makes them unable to distinguish what is good and
what is bad. Indonesian people's mindset is still very narrow, so they do not want to develop
themselves from various aspects. But the government is less aware of this with the lack of holding
extension activities. Though counseling is very important to be done evenly in order to reach all
Indonesian society.The government itself focuses on the welfare of society to realize Healthy
Indonesia. The government's response to high mortality rates in Tuberculosis itself is to ensure
access to diagnosis, effective treatment, stop tuberculosis disease, reduce injustice in social and
economic burdens, promote health, and cure every tuberculosis patient.
Indonesia is a country that is in a region where climate change often occurs. Climate change
can cause tuberculosis germs to survive in cold and humid temperatures. Tuberculosis germs love
to live in air so they can infect the lungs and living in the lungs with optimum temperature 370C.
TB germs will quickly die when exposed to sunlight containing ultraviolet. To be able to cope with
TB germs, we can prevent it by making air vents in our home environment. Try to keep the room
temperature is not too humid and is at 22-30 degrees so that TB germs can not develop.
Tuberculosis is strongly influenced by lifestyle. the longer the life is the more modern that
makes people easy to access everything and make people live a sedentary life. Lifestyle changes
start from the food, have started many new foods and trends in this age. The trend is not a pride
because the food popularized is not a healthy food but junk food is very dangerous. When humans
are satisfied with the food of man is a sport because this lazy becomes a thing that is not good for
the body. In addition, the start of the development of the world of the night that makes humans do
sex free sex with people who are not known so that it can cause disease hiv / aids. These three
things of course make people experience a decrease in the body's immune system so that
tuberculosis disease is a contagious infectious disease very quickly attack the human body. The
best solution for maintaining and strengthening the human immune system is by eating nutritious
foods (cooked by frying or boiling), avoiding junk food. Exercise regularly to strengthen the body
at least 15-30 minutes per day and avoid free sex to prevent hiv / aids disease or get married in
order not to change couples.
