TRANSPORTATION CONTRACT

That, I am contracted to transport participants of the Municipality of Libona, Bukidnon during the
End-of-Term Assessment and Planning of the Legislative Department in the amount of

I, AURELIO B. LOPEZ, Vice Mayor of the Municipal Government Libona, Bukidnon herein referred
to as the Second Party,

That, I hire the services of a hired vehicle/van operated by ________________________________

for the above-mentioned activity with a contract amount reasonable for the award.

That, the contract amount shall be paid by the Municipal Government through the Office of the
Treasurer, Libona, Bukidnon upon compliance.

This contract becomes valid and binding upon approval of the Municipal Mayor in accordance
with the existing rules and regulations.
