On 11th April at 17:30 Mabrook SW2. Circulating & Reciprocating string at 4770 M. Prior to Cut Core.Driller informed Ch Mech & Tool Pusher, a
DW crown block saver engaged. Checked Driller side upper limit crown block saver sensor *OK*. Noticed DW Toggle valve tilted. Clearance bet
Toggle valve and drum wire wound had sufficient clearance & there was no chance to hit the Toggle. Observed Fast line had come out from gro
Toggle valve. Immediately set Slips in Rotary & slack off WOS 189 Tons.

Noticed Drill line fast end came out, first 4 wraps became loose on Drum

Corrective and Preventive Action (To be uploaded in INTELEX): CA or PA Responsible Person Department
Apply Baker Lock on end of Fast line, installed Additional Clamp on end. PA TP Drilling
Stopped application of Dope on Drill line spooler guide bars, instead using PA TP Drilling
light coat of lubricant oil.

Initial Learning from :

1.Do not ignore any kind of alarm / notification
2..Install Additional retention clamp on fast line end
3.Follow the routine checks on each equipment as instructed in the Check list.
4.Do not apply dope on drill line spooler guide bars, instead use Thin coat of lubricant oil.
5.Take immediate action by following HSE procedure.
