AFFIDAVIT OF DENIAL

I, _______________, Filipino, of legal age, married and a resident of

Candulawan, Talisay City, Cebu, after having been duly sworn to in accordance
with law, hereby depose and say:

1. That I applied for a Globe Telecom connection sometime year 2018, but it
did not materialize because the Globe Telecom did not connect its internet
connection nor approved my application;

2. That on April 15, 2019, I received a text messages from a collecting agent
that I allegedly had an account under Globe Telecom, under Innove, that I
have a due and demandable amount under the said account in the
amount of Php 8,106.85;

3. That I hereby unequivocally deny that I have an existing account with

Globe Telecom;

4. That I am executing this affidavit in order to attest to the veracity of the

foregoing statements and for whatever legal purpose this may serve.
