AFFIDAVIT OF DISCREPANCY IN NAME

I, FREDERICO JOSE V. VELASQUEZ, of legal age, single, and with address at 9

Amethyst St. Severina Subdivision, Km18 Paranaque City, under oath, state:

1. THAT my name is FEDERICO JOSE V. VELASQUEZ, as stated

and registered in my birth certificate issued by office of the Civil
Registry of Manila. A copy of my birth certificate is hereto attached as
ANNEX “A”;

1. THAT the name FREDERICO JOSE V. VELASQUEZ appears in my

passport which was issued based on the data appearing in my birth
certificate. A copy of the page of my passport where my name and
picture appears is hereto attached as ANNEX “B”;

2. THAT my parents intended that the name be registered as Frederico.

However, based on the script writing/penmanship in my birth
certificate my parents were misled by the blurry and confusing hand
writing that the same was spelled FREDERICO (with an “R”) NOT
FEDERICO

3. THAT the same is used since childhood, as my friends and acquaintances

call me “FRED” from the name FREDERICO and thus since then
I have been accustomed to using the name FREDERICO my personal
records, school records transactions and communications;

4. THAT the name FEDERICO JOSE V. VELASQUEZ and FREDERICO

JOSE V. VELASQUEZ refer to one and the same person, the herein
Affiant;

5. THAT I am executing this Affidavit to attest to the truth of the foregoing

facts and to use the same for whatever legal purpose it may serve.
