We are Grade 11 HUMSS students of Camarines Norte National High School

(CNNHS) who are conducting a research with the title Evaluating Senior High
School (SHS) Teachers and Grade 11 Students toward the First Rizal
Monument. This research is a partial fulfillment in our Practical Research I
subject.

In this light, may we humbly request for your permission to allow us in conducting
a survey to the SHS teachers and Grade 11 students of your school on March 13.
In addition, we will be conducting an interview session with at least 3 Grade 11
students and 2 SHS teachers on the same date.

Rest assured that we will follow all the necessary protocols within the said school
and that the information, which will be gathered from the data collection, will only
be used for our research.
