1.0 POLICY STATEMENT:

The Company will provide mobile phones as tools of trade to eligible employees.

2.0 SCOPE

This Policy applies specifically to employees of the Company or to Consultants upon the
recommendation of the BU Head.

3.0 GUIDELINES

3.1 Entitlement and Limits


The following are the plan limits for entitled employees/consultants:

The Department/Business Unit Head will recommend which employees require the use of
mobile phones and decide which employees deserve to have a higher plan limit depending
on their level of job difficulty and responsibility.

Employees who are authorized to use company mobile phones but opt to use their
personal mobile phones, can have their official business calls reimbursed up to the limit
prescribed by their plan type. If the employee’s personal phone is under a postpaid plan,
original bills should be submitted to the Department/Business Unit Head for review. All
business calls should be properly identified and accounted. For a prepaid plan, official
receipt with a valid date of prepaid load will be required for reimbursement in which the
load is still subject to the above Plan Limit.
All employees, with the exception of Job level 16 and up, are required to comply with the
limits stated above. Anything in excess of the limit will be shouldered by the employee
unless it is justified otherwise.

4.0 PROCEDURE FOR ISSUANCE OF COMPANY MOBILE PHONES:

4.1 Request for Company Phone

4.1.1. An employee who is entitled to a company mobile phone should fill out the
Request for Mobile Phone (RMP) form. The RMP is attached to the request forms
provided by the Service Provider and is endorsed by the employee’s immediate
superior/manager and the Department/Business Unit Head to the Human
Resources Department.

4.1.2. The Human Resources and Admin Department will indicate/review the plan type
and entitlement.

4.1.3 The Admin Department will be the one to initiate the request to the service
provider and process the mobile phone according to the RMP specifications.
Once the request has been granted, it will then be released by the same
department.

4.1.4. As a receipt of mobile phone, the employee will fill out the Material Asset
Transfer Document (MATD) form which includes the whole item’s description,
quantity and accessories that match/comply to the conditions specified therein.
The original copy of MATD is to be kept by the Admin Department.
4.1.5. Any transactions such as share–a load, non-office related downloaded applications
or local calls not relevant or connected to the company’s interest and transaction
will be charged to the employee.

4.1.6. If the employee wishes to upgrade the phone handset that comes with the Plan,
the employee must pay outright for the said upgrade. However, if the employee
resigns within the lock-in period, the unit must be surrendered without any
reimbursement.

4.2 Roaming Request

4.2.1. The employee who will be subject for travel must inform the admin beforehand not
exceeding to 3 days so as to prepare the technical team of the service provider of the
details such as tracking of the employee’s country to be visited and charging of the total
expenses either texts or calls within the period of the business trip.

4.3 Resigned Employee

4.3.1.The resigned employee who will be able to complete the two years lock-in period may
get the absolute ownership of the mobile phone package. However, if the resigned
employee who will not be able to complete the two years lock-in period may surrender the
phone package in good condition or may complete the remaining month/s or year of
payment to get its sole ownership.

4.3.2. For resigned employee who has not been able to complete the lock in period for two
years and lost the phone in any circumstances it may possibly be in, he/she has to replace
the whole phone package with the same unit/model and brand. Otherwise, it should be
replaced to another existing unit and brand with exact same amount.
*Admin to add provisions on resigned employees who completed lock-in periods.

*Resigning employees’ option to buy phone.

* Provisions on loss and damages

* Provision on replacements for resigning employees

THIS POLICY SHALL BE SUBJECT TO PERIODIC REVIEW, AMENDMENTS OR


REVISIONS WHEN NECESSARY, WITH THE ABSOLUTE DISCRETION OF THE
MANAGEMENT.
