SUBJECT : Notice for the Negative Slippage

1stWarning (Early Warning Stage)

Dear Mr. Carpe:

This refers to the contract ESTABLISHMENT OF FIRE-FIGHTING SYSTEM

(INSTALLATION OF FIRE SUPPRESSION SYSTEM) IN CAPITOL COMPLEX, ALABEL,
SARANGANI PROVINCE in the amount of Php 4,689,009.10. Please be informed that as of
June 10, 2019, you have incurred a negative slippage of (-) _7.61% due to your fault caused
by slow project implementation of items specified in the attached sheet of paper, which now
falls under the “Early Warning Stage” pursuant to Department Order No. 193, Series of
2016 – Administrative Action on Contract with Negative Slippages in Accordance with the
Revised IRR of R.A. 9184.

Based on the approved PERT/CPM Network, the planned accomplishment is _1.08%_ % while
your Actual accomplishment is only _8.69_ %.

In view hereof, you are hereby directed to submit a detailed “catch-up” program on a
fortnightly (two weeks) basis to eliminate the slippage.

In the action program being required, specify therein the additional input resources such as
manpower, materials, equipment, and management to cope up and meet the desired result
accordingly.

Your immediate compliance on this matter is hereby sought.
