This has reference to your project: Construction of Two(2) Storey Four(4) Classrooms
Workshop Building(Regural Laboratory-2), Kalinga National High School, Tabuk City, Kalinga.
Processing on your request regarding the resumption of work at site as well as time
extension in lieu of the suspended days of above captioned project must be strictly in
accordance with Department Order No. 142 Series of 2017: Implementation of the
Document Tracking System (DoTS) for Civil Works. In complying with this department order,
all requests with lacking documents will not be entertained. Hence, attach all documents as
per approved checklist under D.O 142, Series of 2017.

Relative documents including checklist for the said request are hereto attached for your
reference.
