D. Monitoring Findings/Issues Encountered & Actions Taken


No supply or available supply for the spices or condiments. We teachers provided the
ingredients for the foods.

E. Procurement Process
The school head is the one who task to get the food that we are going to cook and he kept the
remaining foods in his ref.

F. Good Practices or Lessons Learned


Because of the simple living of the community, we noticed that most of the children did not use
spoon and fork in eating so we prepared water and soap for them to wash their hands first
before eating. We also observed that most children have a lot of spill out food each time they
ate, so we taught them the proper posture in eating. We taught them also on how to use spoon
and fork during eating.
School based feeding program(SBFP) helped the children who supered in severely wasted
children to have a normal weight and height corresponded at their age. Likewise, the program
teaches us how important the proper nutrition in the limestone of the children.
G. Personnel Involved
The personnel involved are School Head, teachers, parents and the wasted and severely
wasted pupils.

L. Monitoring Findings/Issues Encountered & Actions Taken


Some of the parents who were assigned to cook the lunch of their children did not come to
school so it’s us teachers who cook for the pupils who are the recipient of the feeding program.

M. Procurement Process
Upon learning of the budget allocated by the Division for our school, we teachers planned what
food to cook, and assigned who will purchase and when and where to purchase the vegetables
and ingredients.
