I, JUN GARCIA SANTOS, of legal age, married with residence and

postal address at Brgy. 3rd District, Jalajala Rizal having been duly sworn to
accordinace with law, do hereby depose and state:

1. That, our MANILA WATER is being register under my name JUN

SANTOS with Contract Account No. 16407755 and Service Address at
80 G. Borja Third District (POB.) 9993 Jalajala Rizal;

2. That, I would be much obliged if you will transfer the Account Name
from JUN SANTOS to JHOANA SANTOS BAYOGO;

3. That, I may request to the MANILA WATER COMPANY INC, to please

transfer the Account Name under the name of JHOANA SANTOS
BAYOGO;

4. That, I am requesting this affidavit to attest to the truth of the
