327 Masigasig St. Bagong Nayon, Baliwag, Bulacan, after being sworn to in accordance
with the law hereby depose the following:

1. That as of the present time, I am not employed in any capacity at any corporation,
company, and individual person;
2. That I am applying for an entry level teaching position in a Junior High School at
Teodoro Evangelista Memorial High School;

foregoing and to support my application for the teaching position in Teodoro
