SUBJECT: REQUEST FOR PROVISION OF PROVIDENT FUND

Respected Sir.
With Due respect and humble submission, I Ali Raza working in SMS private Ltd since January
2011 in USE Embassy project as Assistant Manager Supply chain.
Recently I have facing seriously family problems at my home, my father is seriously ill from last
seven months, he had serious Psychotic and heart problems. My father’s panel did not cover
treatment of psychotic diseases. I am only son of my parents, I am paying all his treatment from
my salary. Due to this unfavorable condition I have spent all my savings on my father’s treatment
and I also taken loan to continue his treatment. I am only earner of my family, I am supporting my
wife, two children and my parents.

Under this circumstances, I therefor pray and hope that you would be kind enough to allow me to
have my provident fund (My contribution and as well as Company’s contribution). It is only hope
of paying of my loans.

I am always thankful for your kind help and support.
