AFFIDAVIT OF LOSS

I, Sunday Liasu Joseph, Male, Christian, Nigerian of 22, Reverend Olutayo Street,
Balogun Bus Stop, Ago Palace Road, Isolo, Lagos State, hereby depose to this
affidavit and state as follows;

1. I am the above named person.

2. I work with God’s Care international School at Okota Road, Isolo.
3. On December 14, 2018, I lost my phone on my way back from work
4. The lost phone contained my Simcard
5. This affidavit is deposed to for record purposes and so I can process
replacement for the lost items.
6. That I make this affidavit in good faith, believing same to be true in accordance
with Oath Law of Lagos State 2004.
