BAC RESOLUTION NO. 04-S-2015

WHEREAS, the Barangay Government of Barangay Mabacan, Calauan, Laguna post the Invitation to
Bid for the procurement of Repair and rehabilitation of Barangay Health Center Roofing, Barangay,
Mabacan, Calauan, Laguna twice at Philippine Government Electronic Procurement System (PhilGEPS) and
in a conspicuous place at the premises of the Barangay Hall of Barangay Mabacan, Calauan, Laguna
continuously for 10 days;

WHEREAS, in response to the twice (2) postings and notice, no prospective bidder submitted to the
Bids and Awards Committee (BAC) neither a letter of intent and application for eligibility nor purchase a
bidding documents;

WHEREAS, the members of the BAC, RESOLVE to declare a FIRST FAILURE of bidding on
September 14, 2015 and a SECOND FAILURE of bidding on October 7, 2015, and RESOLVED
FURTHER to recommend the Negotiated Procurement of Repair and rehabilitation of Barangay Health
Center Roofing, Barangay, Mabacan, Calauan, Laguna;

WHEREAS, the Procurement of Repair and rehabilitation of Barangay Health Center Roofing,
Barangay, Mabacan, Calauan, Laguna will not only ensure the continuing operation and delivery of basic
health services, but even address the required facility for a better health services thru safe and comfortable
health center;

WHEREAS, considering the above premises, the pressing needs of the Barangay Government of
Barangay Mabacan, Calauan, Laguna to immediately implement the procurement of Repair and
rehabilitation of Barangay Health Center Roofing, Barangay, Mabacan, Calauan, Laguna in accordance
with the desire of its elected officials to attend the delivery of health services;

NOW, THEREFORE, We, the members of the BAC, hereby resolve as it is hereby RESOLVED to
recommend the Negotiated Procurement of Repair and rehabilitation of Barangay Health Center Roofing,
Barangay, Mabacan, Calauan, to any contractor and enterprises engaged in the business of construction and
delivery of materials and whose offer would be most advantageous to the Barangay Government of Barangay
Mabacan, Calauan, Laguna;

RESOLVED, at Barangay Hall of Barangay Mabacan, Calauan, Laguna this 7th day of October 2015.
