1.0 INTRODUCTION

1.1 Project Overview

The purpose of this project HSE Plan is to define project's HSE Management System,
organization and respon The Provision of Lean Gas Pipeline Phase-1 segment consist of a
new 36” pipeline (including Associated pigging facilities and pipeline accessories)
starting from Jangkrik Onshore Receiving Facility (ORF) and generally heading
northward in parallel with the existing line within PHM’S ROW from KM 7 to KM 21.65
(tie-in to 20” Handil – Nilam Pipeline location) for a distance of about 15 km. Final
destination of Jangkrik treated gas is to the Bontang LNG Plant and local market. This
route passes through primarily hilly terrain and some swampy areas of PHM’S ROW. It
also consists of modification work at ORF (KM 7), modification work at KM 21.65 and
provision of metering and piping, HIPPS, pig launchers, pig receivers, and integration
system with existing

1.2 Scope

This document describes the Emergency Response Procedure in the event of


emergencies on the following: Pipe line area, ORF, Workshop Fabrication and KM. 21,5

This procedure defines specific emergency situations and describes the response
necessary to maintain control of situation and to overcome any impact to the health and
safety, security and environmental of PT. Agcia Pertiwi staffs, sub-contractors, client's
personnel, visitors and guests, damage or loss to assets, environment and
reputation of the Company

1.3 Purpose
 Described the conditions in the Emergency Preparedness to be taken and carried out
in the event of all emergency situation in all workplaces under the supervision of
PT.Agcia Pertiwi.
 Ensuring measures and appropriate responses, fast and effective in combating the
face of life-threatening events, environmental or assets in order to reduce losses and
severity can inflict.
 To ensure the Emergency Response Plan is available and implemented if any
Emergency situation such as fire & explosion, earthquake, bomb attack, rupture,
illness and injury or death, property damage during project activity

2.1 RESPONSIBILITIES OF Site EMERGENCY RESPONSE TEAM


Incident Commander
 Coordinate all emergency respond activities.
 Inform/report to Company Representative.
 Coordinate with company, Contractor Management Response Team and third
party team.
 Upon being notified of an emergency, he shall immediately go to emergency
site together with ERT and regularly update to Company.
 Always make a regular communication with Company Emergency Response
team or On Call when on the way to site or when leaving the site.
 Activate ERT When emergency case occurred.
 Establish the Control Point, Attempt to evaluate and investigate the extent of
an emergency and report to OC the level of Emergency, request for more
additional personnel at site if situation getting worse or beyond control.
 To instruct ERT to control site such as:
 Control access and road nearby that could be danger to public.
 Control fire from spreading to adjacent areas without endangering
themselves
 Activate Search and Rescue (SAR), if there is a possibility of injured
people. Without endangering themselves
 Advice OC of a safe approach to site, number of causalities, Rescue
Team at site, logistic and other support required.
 Provide information and advice to other Rescue Team arriving at site.
 Updating emergency situation to Company Reps.

On Scene Commander
 Establish ERT & allocate adequate personnel at ERT & emergency site.
 Provide full support to Incident Commander at site and proceed to site once
Emergency situation is fully activated.
 Ensure to assign a person to take charge of the ERT.
 Coordinate with Incident Commander on duty to have access of facilitation
needed to evacuate the injured to hospital
 When situation under control, assist accident investigation process.
 Safe and Secure the Emergency Event and area and keep coordination with
Third party to have the assist.

Fire and Rescue Team


 Must be Supervisor area or Selected Worker and already Trained for Basic
Firefighting training.
 Immediately go to emergency site when being informed of emergency. Make
regular contact with OSC.
 If reached emergency site first, he shall confirm the emergency to on scene
commander.

 Extinguish the fire if safe and implement the Fire Protection system (cooling,
separating, etc.)
 Check Lower Explosion Limit (LEL) at surrounding area and determine the safe
area to set up the Control Post.
 Eliminate all hazards at site, if not jeopardizing his safety and the safety of
the people nearby.
 Coordinate Fire Team and stretcher during emergency situation.
 Muster checker at muster point and report to On Scene Commander
 Assist all crew to proceed the muster point and

Logistic
 Provide adequate logistic support at site e.g. machineries, spare part, Vehicle,
initial & expedite any urgent procurement.

Liaison Officer

Medical Team
 It must be paramedic and personnel who trained and appointed as first aider
 Responsible to Medical Equipment at site to ready to use
 First aid treatment during emergency situation
 Assist on scene Commander for IP Condition

Oil Response Team


• The personnel who involve in team and will train for oil response procedure
 Ensure spill equipment is ready for use at all times

 Immediately go to emergency site when being informed of emergency. Make
regular contact with ER Coordinator
 If reached emergency site first, he shall confirm the emergency to ER Coordintaor
 Check Lower Explosion Limit (LEL) at surrounding area and determine the safe

 Eliminate all hazards at site, if not jeopardizing his safety and the safety of the
people nearby.

 Conduct inspection activities and ensure triggers of emergency events do not

Supervisor/Foreman
In the event of accident supervisor shall stop the work and inform or contact
immediately On Scene comannder, to ask for the emergency team, fire team or
doctor/medic to come to the location immediately.
Report all detail of incident occurrence:
 Name of person giving information
 Location of incident. Name of casualty.
 Brief details of incident and patient condition.
 Description of accidental event (accident, fire, explosion. etc.} Advice of
any special condition
 Time of incident occurrence.

Ensure the area of accident, fire or explosion is isolated so it will be accessible for
fire/Evacuation in this location.

Except of doctor/paramedic or trained person no body is allowed to handle the


Injured.

Log Keeper Administration Assistant


The Log Keeper records the time of all events and actions taken. He is also in
charge of keeping track of missing persons on-site and those involved in search
and rescue activities. He is responsible for compiling all records and logs from
each emergency responder and for preparing an Incident Report

4.0 PROCEDURE

4.1 Level of Emergency shall be declared by categories as below :

1st Level of Emergency (TIER 1)

An event that can be dealt with by on-site PT. Agcia Pertiwi and or subcontractor’s personnel
and resources. PT Agcia Pertiwi Site Emergency Response Team will respond the emergency
on site. Company shall receive information about the emergency situation from PT. Agcia
Pertiwi immediately.

2nd Level of Emergency (TIER 2)

An event which may be dealt with on-site PT. Agcia Pertiwi and or subcontractor’s personnel
and resources, but requires involvement of wider Subsidiary support and external resources.
PT. Agcia Pertiwi Site Emergency Response Team shall respond to the emergency on site and
will need support from Company Emergency Management Team in Jakarta in responding the
emergency.

3rd Level of Emergency (TIER 3)

A major event, on or off site which requires the involvement of wide range of organizations
and other international resources and external agencies. PT. Agcia Pertiwi Site Emergency
Response Team will respond the on-site emergency and will need support from Company
Emergency Management Team in Jakarta in responding the emergency. Company Emergency
Management Team in Jakarta will need support from ENI E&P division to resend the situation
on site

4.2 EMERGENCY SCENARIO

Fire
1. In the event of fire, the foreman / supervisor commands all workers (other than fire
fighter and first aider) to stop working, and to the prescribed safe point and notified
in the meeting toolbox.
2. The location of fires is isolated, electricity is turned off and other fuel sources at work
a. Sites close to the point of fire should be kept away.
3. Fire fighter officers perform their duties to extinguish hotspots, and first aider officers
provide first aid when injured.
4. Foreman or other worker notifies On Scene Commander and Incident Commander
who responsible for the project that there has been a fire.
5. Incident Commander reports the incident along with any damage that has occurred to
the Project Manager and ENI Representative on site. The information provided is:
 Location of fire
 Actions taken related to fire
 There is a victim or not

 If any, who is the victim and the injury suffered

6. Incident commander instructs the Site Emergency Response Team to carry out its
duties in handling emergency situations.
7. If the fire is out of control, On Scene Commander contacts the local Fire Department
and the police based on coordination with Incident Commander
8. Incident commander reports emergency work to superiors.
9. Secure the scene as much as possible for any personnel and for incident investigation
purposes
10. If the fire already extinguished incident commander report to Company and incident
investigation process raise

by ENI

Medical Evacuation
In the event of an accident causing the worker to be injured, the foreman / supervisor of
the work instructs the first aider to perform his duties. Location of work carried out
isolation.
1. First Aider shall classify the severity of the injury in order to determine of actions to
be taken. If the injury suffered by the worker could not be handled by the first aider,
the injured person will be taken to a medical facility by Emergency Car that available
on every spread.
2. The first medical facility available on site is located in Site Office. The injured person
will be taken to the first aid room and will be treated by paramedic that have been
stand by. This facility will be used only if the patient can be treated by internal
paramedics without being referred to an external medical facility.
3. If the injured person should be referred to external medical services, the Emergency
Car from each spread directly takes the victim to the location of the medical service
that has been agreed previously. If the injured person has a burn, Incident
Commander immediately contacts the Hospital that can handle the nearest Burns
Level.
4. If the injured person is already in the site office and the paramedics decide to refer to
the external medical service, the injured person will be taken directly by using an
ambulance available at the site office
5. Foreman / supervisor reports the incident to the OSC and Incident Commander, and
the Incident Commander reports this incident to the Project Manager and ENI
Representative on site. The information provided is:
 Location of the incident
 Workers are injured
 Injuries that occurred
 First aid provided
 Referral medical services.
7 PT. Agcia Pertiwi should also prepare if the patient should be referred to a superior
medical service if the local medical service is unable to handle.

Handling of Traffic Accidents

1. Traffic accidents are accidents that occur due to driving a task. This accident has a
high probability due to the high mobility of personnel in the Company.
2. Handling Traffic Accidents is the same as Medical Evacuation above steps 1 through
7.
3. Liaison officer shall coordinate with the police.
4. All outside information (mass media, electronic media, and authorities) is provided
only by ENI.

Oil Spillage

1. All persons found the emergency situation (chemical leaks, fuel/oil spill) shall report
to Foreman/supervisor/HSE Officer/Site Manager/Oil Spill Response Team
2. Before handle the oil spill, the Oil Spill response team must make sure its Material
Safety Data Sheets (MSDS) already available on site and read the MSDS carefully
3. Prepare oil spill kit such as absorbent, shovel, rag, and hazardous waste bin
4. Notify the emergency response team / on scene commander to attend the scene for
cleanup.
5. Use PPE properly and keep objects that can cause fire
6. Prevent the oil spill enter the gutter, drainage and seep into the ground by applying
sorbent around the spill.
7. Assess the situation and ensure that the steps were taken to stop the leak or spillage
or inform to on scene Commander that the leak point /the drainage point to be
plugged and isolate the spillage.
8. Announce all the workers and inform them concerning of the condition and situation.
9. Consider the suspension of any activities, which may be affected by the leak or
spillage
10. The emergency response team members (other than oil Spill Response) shall assist in
cleaning up the leakage or spillage. Where possible the spillage liquid should be
collected.
11. Dry the spill with absorbent material.
12. Perform soil dredging using shovels for soil that already contaminated.
13. If there is still any residue, clean with rag and put in a sealed container.
14. Place the container in a temporary storage place of hazardous waste (TPS B3)

Resident Complaints

The grievance occurrence of residents is when a group of people (who are mass) come to
work units or work locations that exist in the Company's operational areas, such as
complaining about the impact of the project, etc.

1. Coordinate with Company Security and Liaison Reps on site


2. Things to consider in dealing with resident complaints are the calm of thinking and
avoiding emotional attitudes;
3. Complaints handling of residents requires the ability to hear with empathy, respect
for others, and provide an opportunity for them to make suggestions;
4. Liaison officer should be escorted by security guards if the condition heats up;
5. Call police or army assistance if needed

4.3 EMERGENCY EVACUATION

This step below is taken because of the following emergency situation. Special emergency
instruction will be made for site. The following steps should be done:

 Identify the type of hazard/emergency on the happening situation


 Identify wind direction to determine Muster Point and crossing evacuation procedure.
 Assembly Points located in two area (in front of the building and in the back)
 Provide Portable Announcer (portable loud speakers) to help giving direction.
 All personnel shall be gathering at Muster Point that had been determined.
 The Emergency Response team will count all people gathered at Muster Point.
 All electricity shall be shut down from center panel located inside room beside stairs on
ground floor
 All vehicles are parked at the place that will not interfere when the emergency vehicles
entered the venue.
 Do not back to work station until “All Clear” sign is announced or rings
 Asembbly Point (Muster Point) will be given “M” alphabet, with the green board and
white color in the center. Muster point always be placed above the wind from
emergency area / emergency venue
 There are no information that can be release to media or other party without the
approval of Project Manager.
 Establish parking rule that regulates all parking vehicle must be park in reversed
position
 If there is any injured person and needed medical treatment, ERT and first aider shall
take the person to nearest (appointed) hospital.

6.0 REPORTING

A comprehensive incident report must be prepared to evaluate the actual emergency


response and the effectiveness of the emergency response plan. Each member of the
emergency response team is responsible for keeping his/her own log. The Log Keeper is
responsible for compiling all logs and preparing an incident report. An incident report should
include the following :

 Date, time, location of the emergency


 Organizations which responded to the emergency
 Emergency response logistics
 Debriefing
 Consequences:
 Number of injuries and fatalities
 Environmental impact
 Cost of the emergency
 Cause or nature of the emergency
 Safety measures
 Analysis of the emergency
