1.0 INTRODUCTION

SNOC, hereafter referred as COMPANY, awarded Petrofac, hereafter referred to as the


CONTRACTOR, the Works for LPG Back Flow and Import Project in Sharjah UAE.

2.0 SCOPE

The CONTRACTOR’s Emergency Response Plan (ERP) will, always, conform to the COMPANY’s ERP
and Emergency Procedures. This procedure is applicable to the emergencies resulting out of
accidents occurring during construction site to employees, subcontractors, and project site
visitors.

3.0 PURPOSE

The purpose of this procedure is to provide for prompt and effective response to, and control of,
emergency situations to reduce losses and the consequences of natural and man-made.
disasters. This procedure aims at:

• Emergency situations are identified


• Emergency plans are established
• Persons responsible for handling emergency situations are identified
• Equipment required to handle emergency situations is identified and available
• Emergency plans established are tested for responses
• Effectiveness of the responses is ensured

4.0 EMERGENCY RESPONSIBILITIES

The Company is responsible for overall managing the Brown field emergencies.
The CONTRACTOR is responsible to assist and report to Company in crises, disaster and Emergency
situations as per “ COMPANY’S” Emergency Response Procedure which shall be followed within the
project scope of work. The SUB-CONTRACTOR will adhere to the Company’s ERP as well as
Contractor’s Construction ERP.

5.0 CONTRACTOR EMERGENCY RESPONSE STRUCTURE & PROJECT INTERFACES

Contractor establishes plans, resources and procedures in line with the Emergency Response and
Crisis Management Standard. This standard includes:

• Structure for the implementation of the documents as regards Emergency Response and
Crisis Management
• Clear expectations and performance requirements for Emergency Response and Crisis
Management arrangements and plans
• Accountabilities, roles and responsibilities for management of Emergency Response and
Crisis Management
• Compatibility and consistency of response planning
• Continuous improvement in performance

• Assurance to internal and external stakeholders that Contractor has effective crisis and
emergency response systems in place
Contractor has a three-tier response system, designed to manage worst-case situations. Standard
processes and well-trained personnel enable us to mobilise quickly and commence managing
priorities. Thereafter, the response organisation can be modified to suit the incident.
This establishes a hierarchy of crisis management; each tier retaining specific responsibilities
within its own sphere of influence within a structured system able to provide successive levels of
support to an incident (See Figure 5.1 below).

Tier 1 emergencies are handled at site. An incident is escalated from Tier 1 to Tier 2 by the onsite
IMT. An emergency incident may be re-categorized at any stage. Escalation from Tier 2 to Tier 3 is
decided by the Tier 2 BST Lead located in Sharjah.

Contractor’s contract Representative will act as the conduit of communication between COMPANY
and Contractor’s BST teams.

A brief outline of the COMPANY / Contractor interface for emergencies is outlined in the Figure
5.2 below.

The complete Contractor Emergency Response and Crisis Management structure for Tier 1 is as
follows Figure 5.3:

5.1 Role of the IMT

The IMT is the group that comes together to deal with a live, or anticipated, Incident, Emergency,
Crisis or other abnormal threat. It consists of the senior staff at the project location. The IMT
Leader shall be the most senior Contractor manager at the Project location. The IMT should not be
at the scene of the incident. It should work as a management team from the site offices. IMT
Manages the response, downwards, sideways, and upwards. Supports the On Scene Commander
and the ERT.

Reports to the Business Support Team (BST) in Sharjah and COMPANY BST.

5.2 Role of On Scene Commander (OSC)

The OSC is the senior person at the scene of the incident. He manages the overall scene, the
response and is the link to the IMT. As part of ER planning senior site personnel must be appointed
as OSCs. Incase of Emergencies in brownfield area OSC will be from COMPANY.

5.3 ERT Membership

5.9.1 Emergency Response Coordinator (ERC)

• Leads the ERT


• Works for the OSC
• Ensures all activities are conducted in a safe manner.
• Updates the OSC on the recovery/response.

5.4 First Aider

• First Aider / Medic name & contact number shall be communicated to every crew.
• Works as per Instruction of ERTL
• Provide Necessary First Aid to the victim.
• Help the rescue team to Carry the victim and secure in stretcher.
• Transfer the victim to the Emergency Vehicle / Ambulance.

5.5 Muster Point Controller

• Coordinates the returns from the different Muster Points across the site

6.0 EMERGENCY RESPONSE CREDIBLE SCENARIOS

6.1 Emergency response

In the event of an emergency:


• The person discovering the incident should immediately call on the project Emergency
response Number (06 526 3263) and or Contact Project Emergency Response Coordinator on
radio and report nature of emergency situation at detail (location and time)
• In case of fire should take initial measures to extinguish the fire, if it is safe to do so. Also,
if it is safe to do so, ensure equipment is switched off and in a safe condition, notify the
Immediate Supervisor or Site HSSE
• Communication shall be done talking slowly and clearly, giving the status of personnel in
the area and describing what has happened. Exact point of the incident shall be provided
by witness.

• If the incident is related to injury of the personnel, Site medical Team shall be informed
immediately. The condition and situation of injured personnel shall be assessed for further
action.
• If the incident is not related to injury, then the HSSE person will assess the situation and
action will be taken as per ERP scenarios.
• ERC / HSSE Lead will notify COMPANY control Room / onsite HSSE focal Point.

6.2 Actions on General Alarm

Employee shall stop work, make the work area safe and head to the nearest muster point.

6.3 Heavy rain

In case of Heavy Rain, The Project ERC will access situation & declare the
Emergency. In case of such emergency,
• Stop all work activity
• Switch off all electrical appliances
• Cover appliances with plastic material if required.
• Gather at Rest shelter / cabins
• Head count to done
• Do not ON electrical appliances until clear signal from ERC

6.4 Sandstorm / Dust

In case of sandstorm the ERC / HSSE lead shall assess the situation & declare emergency
In case of such emergency,

• Stop all work activity


• Gather at cabins
• Head count
• Do not stand outside in sand storm

6.5 Heavy Wind

In case of Heavy wind, ERC / HSSE lead shall assess the situation & declare emergency
In case of such emergency,

• Stop lifting activity if wind speed exceeds 19.4 Knots


• All activity shall be strictly closed in case of heavy wind. The decision shall be on advice
• of Project ERC during emergency.
• Gather at cabins
• Head count if possible.

6.6 Actions on Discovering a Fire

• Immediately raise alarm and inform on Project Emergency Response number and or contact
project ERC on Radio. If communication means are not available shouting shall be used.
• If employee can extinguish the fire safely, then he/she shall attempt to extinguish the fire
with fire extinguisher and monitor the area until the ERT arrives.
• If not capable to extinguish the fire, he/she shall proceed to the nearest muster point and
wait instruction from the ERT.
• In case of fire incident in the Construction area where fire could not be dealt with using
own resources COMPANY Control room shall be called in for support.

6.7 Action on discovery of a Medical Emergency

• In the event of a casualty, personnel shall immediately notify their Supervisor and other
related parties as per Emergency response Notification Flowchart.
• Personnel shall stay with injured person / personnel until emergency responders arrive.
• One of the persons present at site shall meet emergency responders in the working areas
and guide them to the incident location.
• Witness / Witnesses shall not place themselves at risk by attempting to rescue casualties.
Injured people should not be moved unless they are in an immediate danger (e.g., from a
fire etc.). Only trained personnel will provide treatment/first aid.
• If Medical team decides to examine IP at the site First Aid Room, the first aiders shall assist
to transfer Injured person / personnel by using stretchers.

6.8 MEDEVAC

The purpose of the medical evacuation (medevac) is safe delivery of injured or severely ill patient
from site to the nearest medical facility with appropriate capabilities to provide sufficient
diagnostics and treatment when it is necessary due to medical, operational or logistical
conditions.

Contractor medical team shall agree with COMPANY medical team / HSSE Focal Point before
initiating medevac to the nearest medical facility.
An advanced first aider shall be assigned at site First Aid room initially and once the project
manpower reaches 150, Contractor shall arrange certified Medic at the construction site. The site
initial First Aid room shall be equipped as per contract requirements. The ratio of First Aiders to
man power shall be 1:50.

Contractor has also partnered with International SOS, the world's leading medical and travel
security services Company. Employees have a membership to International SOS with a 24 / 7
access to doctors, security experts and assistance coordinators, who can provide advice in many
different languages.

6.9 Man Overboard Emergency

In case of a ‘Man Overboard’ (MOB), the following steps must be taken depending if the person is
conscious or unconscious.

Man Overboard is conscious
• Standby man by shouting MAN OVERBOARD
• Notify the Supervisor and project Emergency Response Coordinator / HSSE Lead
• Throw the nearest life ring to the person in the water possibly by holding the line.
• Always keep the person in sight.
• During dark hours illuminate the area as fast as possible, don high Visibility vests and use
searchlight.
• Prepare MOB/Rescue boat and team
• Administer first aid until further transfer to approved medical facility if required.
Man Overboard in unconscious
• Standby man by shouting MAN OVERBOARD
• Notify the Supervisor and project Emergency Response Coordinator / HSSE Lead
• Always keep the person in sight.
• During dark hours illuminate the area as fast as possible, don high Visibility vests and use
searchlight.
• Prepare MOB/Rescue boat and team
• Administer first aid until further transfer to approved medical facility if required.

6.10 Chemical Spill (Oil, Paints etc)

All chemicals (Hydraulic oil, engine oil and other lubricants, paints etc. shall be stored and
handled with utmost care. In case of a spill following measures should be taken immediately.

• Check MSDS & COSHH assessments for emergency response actions.


• Wear safety shoes, gloves and safety goggles before dealing with the spill.
• Until the spill is completely controlled and collected, keep all sources of ignition away
from the area.
• Stop or control the spill. Pour absorbent materials like sand or saw dust to darn up and
absorb the chemical. Start on the outer perimeter, surround the spill and work inwards,
scooping oil-soaked material into containers as it becomes saturated.
• For large spills, place-soaked material in doubled garbage bags. Seal the inner bag tightly
and separately from the outer garbage bag.

For spills happening in the Brownfield area COMPANY Oil Spill Team will be contacted.
The response shall be coordinated by project Emergency Response Coordinator. All Level 1 oil
spills will be reported to COMPANY.

CONTRACTOR will provide enough spill kits at Construction field sites. The exact number of the
kits shall be identified after the assessment at the site during construction and commissioning
phases.

6.11 COVID 19 Emergency Response

Possible Case for COVID 19

Any Employee, subcontractor or visitor identified as a possible case for COVID 19


symptoms (as explained in section 9.11) he/she shall be isolated in the specified isolation
facility at site. EMERGENCY CONTACTs shall be immediately notified. Further decision
shall be taken in consultation with identified approved local health authorities for
handling COVID 19 Cases. Please see below flow chart for steps to follow for possible case.

6.12 RESCUE FROM HEIGHT

Accidents may happen when working at height; it is an essential part of the planning and
assessments of such work that an emergency rescue plan is in place in case of suspended traumas.
A dedicated plan shall be in place to perform rescue operations from height, all working at height
activities shall have a rescue plan. In case of any support required COMPANY HSSE Focal Point shall
be contacted for support.

The rescue needs shall be be carried out as soon as possible to prevent the start of the negative
health condition on the fallen person.

Once rescue of a suspended casualty has been successfully performed, the relevant first aid,
treatment shall be provided by the First Aider. If further examination is deemed necessary IP shall
be transferred to site First Aid Room and further to approved medical facility.
Working at height activities shall be carried out with buddy system, minimum two person shall be
working.

When working at height, platforms shall be provided: scaffolding structures, MEWP and ladders.

Safety harness shall be provided when working at height, especially on MEWP, ladder, or
scaffolding structures.

6.13 Electrocution

In case of anybody comes in contact with electric current, switch off the electric supply as quickly
as possible. If not possible to do so, use non-conducting material such as wood to separate the
person from the contact. Do not try to directly touch the person. Depending upon the nature of
the injury initially administer first aid, in case the person is unconscious administer CPR
(Cardiopulmonary Resuscitation) (if you are trained to do so) till the Injured regains consciousness
Or you seek help and simultaneously call Project Emergency response Number explaining the
situation and seek emergency help and follow the instruction of emergency crew head.

6.14 Heat Stress

In case anybody is affected by Heat stress, call the available first aider for assistance and
assessment of IP’s condition. Simultaneously inform the Emergency Response Coordinator / HSSE
Lead.

Shift the person to cool and shaded place and allow for fresh air, loosen the clothes and if the
person is conscious, then provide water to drink. Otherwise, do not attempt to give anything
orally.

6.15 Animal Poisoning (Snakes / Scorpions)

Emergency assistance (Inform Project Emergency Response Coordinator / First Aider / HSSE Lead)
shall be immediately called upon in the case of a snake / scorpion bite. While waiting for the
emergency assistance first aid should be provided to the victim. This is very crucial to casualty.
Assure the affected person that emergency Team / Medical assistance is on the way. Clam IP down
and restrict movement. Keep the body part immobile and lower than the heart.

Monitor on the vital signs such as rate of breathing, temperature, blood pressure and pulse.

What Not To Do:


Medical professionals may not agree on every point of what to do for snake bites, but they are
almost unanimous in their opinions of what not to do. Some of them are.

• Tourniquets should not be applied to affected area. This completely stops blood flow and
can result in loss of limb.
• No cold compressors or ice cubicles should be applied.
• Never try to draw out venom through mouth.
• No application of razor or knife should be on the wound.
• Nothing should be given to the victim through mouth.
Scorpion Bite
• Wash the sting area with soap and water
• Remove all jewellery
• Apply cool compresses.
• DO NOT give Aspirin or Ibuprofen

• Do not CUT into the wound.
• Call on Project Emergency response Number

6.16 Rescue from confined space

Accident may happen within the confined space due to the different factors, i.e.: H2S, hot,
narrow area, lack of oxygen etc.

Personnel who are entering into CS shall be equipped with personal H2S gas detector.
Gas detection and monitoring shall be carried out on the confined space activities. Entry shall not
be permitted into confined space unless safe to do so and gas testing shall be done prior to
rescue.

Rescue equipment, like pulleys, tripods, stretcher, SCBA for rescuer shall be provided at site, it
will be a part of the rescue plan from Confined Space. Rescue from Confined space shall follow
COMPANY Requirements.

6.17 Trench collapse rescue

All trenches shall be barricaded with hard barrier, and proper access egress shall be provided. In
case of a trench collapse Emergency response procedure shall be activated.
COMPANY shall be informed as per Emergency response flow chart.

Make The Rescue Area Safe

• Control all hazards in the area, i.e., utilities, electric, gas, water.
• De water the trench if necessary
• Monitor the atmosphere in the trench
• Ventilate the trench if necessary.
• Identify soil type and condition

Make The Trench Lip Safe

• Approach the trench from the ends if possible


• Look the unidentified hazards
• Assess the soil pile for improper angle of repose
• Remove any tripping hazards
• Make safe or remove unstable trench lips
• Place ingress and egress ladder, at least 2 ladder shall be placed in the trench close to
each other.
• Decide on shoring system to be used
• Crate a safe zone in the un-collapsed portion of the trench, possibly from both end
• Remove the dirt from the collapsed zone. Rescuer shall remain on the safe zone of the
trench while removing the dirt from the collapsed zone.
• Secure unsecured utilities, pipe or any other obstruction in the trench

Victim Removal Accident Cave In

• Create a safe zone


• Begin to remove dirt, operate from a safe zone (bucket, shovel, by hand)
• Continue extending safe zone into collapse zone
• Uncover victim to below the diaphragm
• Begin patient assessment if possible
• Begin ventilation if possible
• Completely uncover the victim
• Properly put victim on the stretcher
• Remove the victim from the trench (vertical haul, horizontal haul)

Treatment Victim

• ABC’s primary survey


• Transfer to site First Aid Room for further survey
• MEDEVAC to approved hospital for further treatment if necessary

6.18 Actions on Radioactivity emergency

Contractor shall follow COMPANY Emergency Response Plan for Radioactivity emergencies.

6.19 Actions on the road traffic emergency

In case of traffic emergency, immediately COMPANY Control room / HSSE Focal point and
Contractor emergency number shall be called as per Emergency response flowchart.
In case there are injured persons, medical team shall be invited to incident scene, and medical
assistance shall be administered.

The vehicle involved in the accident should remain at the scene wait for road police and COMPANY
and CONTRACTOR representative to arrive for initial incident investigation.

6.20 International SOS

Contractor has partnered with International SOS, the world's leading medical and travel security
services Company. Every employee on the contract shall have a membership to International SOS
with a 24 / 7 access to doctors, security experts and assistance coordinators, who can provide
advice in many different languages.

7.0 EMERGENCY EVACUATION DRILLS

Employees and Staff shall be briefed with regards to the project Emergency Plan. Emergency
Evacuation Drills shall be conducted quarterly or according to the site requirements which shall be
conducted at project site as well as office area to ensure awareness of employees and staff with
regards to the project emergency procedure. Detailed report of Emergency Drills shall be
generated and reviewed for any improvement and shared with COMPANY.

• Works as per Instruction of ERTL
• Rescue the trapped persons.
• Ensure that all personnel had vacated the area.
• Carry the victim and secure in stretcher for the medical aid.
• Fight the Fire and ensure that it is extinguished completely.
• Do the gas test for the oxygen in the air, presence of toxic chemicals etc, if any.
8.0 EMERGENCY RESPONSE EQUIPMENT

CONTRACTOR will provide emergency response equipment to deal with emergency situation during
construction phase. Certain amount of oil spill kits will be provided by CONTRACTOR. Fire
extinguishers will be located at laydown / offices, and all hot work activities shall have fire
extinguishers. Rescue equipment from confined space: tripod, Rescue equipment, like pulleys,
stretcher, SCBA for rescuer shall be ready at site. Company’s Muster points shall be followed.
Regular inspection and testing of emergency equipment shall be carried out by responsible
personnel. Equipment’s subject to inspection are (but not limited to):

• Environmental Spill Kits – Monthly by CONTRACTOR HSE


• Eye Wash Stations – Monthly by CONTRACTOR HSE
• First Aid kits - Monthly by CONTRACTOR Doctor
• Fire Extinguishers – Monthly by CONTRACTOR HSE

Personnel H2S gas monitors and BA set in critical areas as required.

9.0 HOSPITALS NEAR HAMRIYA

9.1.1. THUMBAY HOSPITAL AJMAN

This private hospital’s Accident and Emergency Department serves 24 hours a day, 7 days a week.
They have a varied blend of general specialists and junior specialists who pander to patients with
a heap of conditions and also offering quick treatment and evaluation to intense crisis conditions.
The Hospital assesses and treats patients who have suffered a recent injury or accident, or who
have developed a sudden illness. Manned by General Practitioners and Junior Specialists who are
constantly on hand to treat life-threatening illnesses and critically injured patients, the state-of-
the art unit is open 24 hours and seven days a week. To ensure timely treatment and patient flow,
they utilize a triage system where the most seriously ill patients are seen first.

The Main Emergency Department (ED) has 8 ED beds which caters to patients belonging to the less
severe categories of patients. The department also has three beds for trauma and two beds for
resuscitation. Two clinic rooms serve as the central node where doctors can work from. Patients
can also use the spacious waiting area before they can be seen.
