This Project Emergency Preparedness and Response Plan has been developed to protect staff
and its employees, visitors, neighbours and the Environment in the event of an undesired event or
emergency situation and to promptly return to normal activities once the emergency has passed.
Emergencies can occur from a number of causes, and therefore it is necessary to implement plans
to respond to different situations.

Main anticipated emergencies that may occur at site, may arise from following categories:
 Medical emergency case; personal injury, electrical shock, confined space, fall, traffic etc.
 Fire and explosion case
 Environmental pollution case, chemical spillage
 Vehicular accident case
 Severe weather case; such as cyclone/heavy rain, flood.
 Earthquake
 Security threat case
 Heat Stress
 Gas Leak

The plan will be reviewed on a regular basis. Should there be any shortfalls and Site Particulars
identified, these will be addressed in consultation with the Project Management team.
The reviewed emergency plan will be made available to all new and existing employees through
their relevant Supervising Managers.
All Incident-Accident shall be reported to HSE department and all relevant authorities like
Ashghal/MOI and government agencies.
It is reminded that each accident must be followed by a Tool Box Talks given by the HSE Officer
on the “Lessons Learned” and must be recorded and filed. All emergencies including Drills must
be debriefed and report to be kept.
A single project emergency contact number has been implemented to centralise all calls and will
be communicated to all people on site during site induction (applicable to all project employees
and visitors).

1.1 Aim and Objective


The purpose of this Emergency Preparedness and Response Procedure is to provide a set of
guidelines for site personnel to facilitate responses to particular site emergencies. It details all
the likely hazardous scenarios and events that may develop and the appropriate responses
and plans that should be enacted relating to Catchment Zone Framework. Details of the
specific responses to be taken, should an emergency occur, are contained within this plan.
It is Velosi Management goal to provide maximum protection for all employees in the event of
an emergency. Also, to develop and maintain an emergency response plan for all employees
and Suppliers and out regular emergency drills to ensure they are familiar with the response
procedure.
The Company will review and revise such procedures periodically, in particular after the
occurrence of any specified incident or emergency situation and as a minimum of

annually. Periodic testing shall ensure whether the procedures are feasible, functional
and practicable.
The objective of the plan is to ensure normal operation after an incident or a disaster will
resume as quickly as possible, identify weaknesses and implement disaster prevention;
thus, reducing the complexity and time constraints of the recovery effort.

1.2 Scope
The scope of this plan specific to Velosi Catchment Zone Framework / subcontractors /
suppliers / other staff involved in this project. This procedure meets the requirement of the
Project HSE Plan AA DOM 2018 M 1867 G-HSP-VELOSI-Rev.08. This Emergency
Response Plan provides a description of the project, planning and management, reaction of
Velosi emergency management personnel to any emergency situation on the construction
site. It outlines the actions to be initiated and notifications to be made before, during and
following any emergency incident, in accordance with QCS 2014 Guidelines.

Specific aims of the plan are:


 To assemble all personnel at specific designed areas.
 To account all Velosi Personnel and Subcontractors on site.
 To ensure that all Velosi personnel and subcontractors remain at assemble points until
instructions are given.
 To set up the guidelines for execution of any medical cases to the appropriate treatment
facilities.
 To ensure an effective emergency response plan with roles and responsibility to key staff clearly
defined at all level of emergency.

ERT Emergency Response Team, comprising members of the project, including
nominated trained personnel from Subcontractors, whom will be co-ordinators at
“Meeting Places” during an emergency. The team is trained to assist in first aid,
firefighting and basic rescue techniques.

Emergency Serious incident conforming to one of the following:


 Event that poses immediate threat to life, health, property, facility and/or
environment
 Event that already caused loss to life, health, property, facility and/or
environment
 Event that has a high probability of escalating to cause immediate threat
to life, health, property, facility and/or environment
Facility Workplace
Impact Physical damage to people, property or the environment from the
consequences of an incident (e.g. property damage, injury, fatality, fish kill).
MSDS Material Safety Data Sheet means a document disclosing the information of the
chemicals and hazardous products.
Local authority In relation to a facility, means the local authority for the local authority area in
which the facility and the surrounding area are located.
Overpressure Pressure developed above atmospheric pressure at any stage or location from
a blast wave or pressure.
Incident “Incident” A work related event(s) in which an injury or ill health (regardless of
severity) has occurred or could have occurred.
An incident where no injury, ill health or fatality occurs may also be referred to
as a “near miss”, “close call” or “dangerous occurrence”.
An emergency / security situation is a particular type of incident that will involve
the different levels of authority.
Near Miss An event or chain of events that could have resulted in fatality, injury, illness,
and / or damage (loss) to assets, the environment, company reputation or third
parties. The situation was avoided or reduced by the circumstances at the time
or under slightly different circumstances would have been more serious.
Accident An undesired event giving rise to death, illness, injury, damage or other loss.
(see Incident)
Fatality A death resulting from an Injury or Illness. Regardless of the time intervening
between Injury and death.
Injury Physical harm or damage to a person resulting from traumatic contact between
the body of the person and a source of energy, or from exposure to
environmental factors.
Occupational Any work – related abnormal condition or disorder, other than injury, which is
Illness mainly caused by exposure to environmental factors associated with the
employment. It includes acute and chronic illness or diseases

First Aid Any one-time treatment and subsequent observation of minor scratches, cut,
burns, splinters, and so forth, which do not ordinarily require medical care by a
physician. Such treatment and observation are considered First Aid even
though provided by physician or registered professional medical personnel. The
following are considered to be first aid treatments:
 application of antiseptics during the first visit to medical personnel,
 treatment of first-degree burns (Characterized by reddening of the skin
only),
 application of bandages during any visit to medical personnel,
 use elastic bandages during the first visit to medical personnel,
 removal of foreign bodies not embedded in the eye, if only irrigation is
required,
 removal of foreign bodies from a wound, if the procedure is
uncomplicated (e.g. using tweezers)
 use of non-prescription medications an administration of a single dose of
prescription medication on the first visit for minor injury or discomfort,
 soaking therapy during an initial visit to medical personnel, or removal of
bandages by soaking,
 application of hot or cold compresses during the first visit to medical
personnel,
 application of ointments to abrasions prevent them from cracking or
drying,
 application of heat therapy during the first visit to medical personnel,
 use whirlpool bath therapy during the first visit to medical personnel,
 negative X-ray diagnosis,
 Observation of injury during a visit to medical personnel
Armed assailant A person who has attacked someone with a weapon or has threatened to do so.
Bomb threat Communication that a bomb has been placed in the site.
Civil Demonstration, site temporary office takeover, confrontations, and other events
disturbance at a level that would disrupt College order and discipline.
Fire/explosion An occurrence where there has been fire or explosion that has or could cause
physical injuries, death or significant environmental damage.
Hazardous An occurrence where there has been a leak or spill of hazardous materials
material release which has or could cause physical injuries, death or significant environmental
damage.
Hostage A situation where one or more persons are being held against their will by one
situation or more individuals.
Mass casualties Multiple deaths and/or injuries by any cause.
Natural disaster A natural occurrence resulting in structural damage that has or could cause
injury, death, property damage, and/or interruption of services.
Public health Accidental or intentionally caused situation in which the health and well-being of
threat the contractor personnel on site is jeopardized.
Severe weather Any weather condition that has or could cause injury, death, major property
situation damage. This includes blizzard, flood, heavy rain, strong winds, electrical storm,
and tornado.
Utility failure Any interruptions of utility service to one or more area project, to include electric
and water.

Serious incident Incident causing one of the following:


 In case of harm to people:
- Fatality

- permanent total disability

- clean-up response requiring specialized external parties,

- spills: large volume (e.g. above 200ltr of hazardous material)

- Extensive damages – in excess of 2,500,000 QAR

2. PROJECT DESCRIPTION
The VELOSI CERTIFICATION LLC shall provide Sewer Repair, Replacement and Rehabilitation of
ASHGHAL operated sewer assets, Condition Assessment of sewer pressure mains, and trade
Effluent Baseline survey in the state of Qatar.
The scope of work will include one or more of the following activities:
 Site preparation
 Traffic management on site
 Sewer Repair
 Sewer Replacement
 Sewer Rehabilitation
 Closing the site safely and remove all site equipment to leave the site clear as it was received

3 ROLE DUTIES AND RESPONSIBILITIES

 Provide / facilitate necessary resources for implementation of the procedure.


 Appoint in writing an Incident Management Team.
 Provide leadership to the Incident Management Team.
 Support training of the Incident Management Team.
 Initiate Incident Management Team activity based on risk assessment of incidents and threats
to the organisation.

HSE Engineer (Emergency Coordinator)

The HSE Engineer shall conduct a risk assessment of potential incidents for the project and
develop specific flowcharts to reflect the potential response strategies available.

 Develop sub-plans and templates for incident management


 Prepare training programs for IMT members and emergency response teams.
 Provide advice with respect to Incident response equipment requirements.
 Assist in the nomination and training of Emergency Response Teams
 Ensure liaison with the authorities
 Ensure the project emergency phone is attended 24hrs/7 by Medical Officer
 Review the present plan.
 Propose additional safeguards if necessary.
 Communicate the procedure to the HSE Officer/ Safety Officer.
 Plan and organize emergency drills as per this procedure and reports to the Client / Consultant.
 Get feedback from HSE Officers about the effectiveness of the procedure and reports to Project
Manager.
 Provide internal training to Fire wardens, Team emergency spill control, fire marshals….
 Responsible of activating of the emergency spill control team, in case of any spillage on site
(Civil works workshop, Warehouses, Stockpile area)
 Regular liaison with the local fire brigade, hospital and any other relevant organization whose
services might be required in the event of

Emergency Coordinator (Emergency Situation)

 Activate call out of appropriate personnel


 Assess and monitor emergency situation

Emergency Response Planner

The Construction Site Manager shall take on the roles and responsibilities of the Emergency
Response Planner.
Duties include:

 Providing forward thinking ideas for the Emergency Coordinator


 Information management for whole team
 Determine response strategy with Emergency Coordinator, Incident Management Team Leader
 Stand in for Emergency Coordinator if absent form office/this may also require the Emergency
Coordinator to facilitate the role of Emergency Response Planner
 Assist in complying incident status summary
 Provide guidance on escalation of emergency response
 Prepare contingency plans for on-going period until incident is confirmed over and team have
stood down

On Scene Controller

The HSE Representative on site or the Area Authority closest to the scene of the incident shall take
up the role of the onsite controller; the main responsibilities of the On-Scene Commander are as
follows:

 Identify and initiate callout of support emergency response resources


 Notify the Emergency Coordinator in the event of an onsite incident
 Maintain direct means of contact with the On-Scene Commander from the scene of the incident
 Provide command and response at the scene of the incident
 Maintain an overview at the scene of the incident
 Report escalation of incident to Emergency Coordinator, Emergency Response Planner
 Coordinate local resources available at scene of incident
 Maintain log of events at the scene of the incident
 Make recommendations to Emergency Coordinator when to demobilise personnel and stand
down the response team

HR Representative

The HR Representative shall immediately attend the Incident Control Room in the event of an
emergency notification

 Obtain relevant information regarding injured/missing/safe persons and implement personnel


tracking
 Act as the focal point for obtaining key information to support the identification and security of
personnel on site
 Liaise with Medical Team for planning of evacuation of personnel
 Liaise with the Emergency Response Planner to deal with onsite resource requirements
including but not limited to toilets, food, accommodation
 Assist in the evaluation of off-site resources

First Aid and Medical staffing requirements

 Medical Staff has been provided as per the below guidelines.


 First Aider and First Aid Kit to be provided for each 25 employees.

 If Above 100 employees, A Registered Male Nurse approved by the Supreme Council of Health-
Qatar will be place in the project.
 If Above 500 employees are appointed, A Registered Doctor approved by the Supreme Council
of Health, Qatar will be place in the project.

First Aiders

 Liaise and assist with Male Nurse


 Act as lead first aider when required to do so
 Apply first aid to casualties where required
 Control and prioritize casualties

Admin/Logger

The Admin/Logger shall:

 Go immediately to the Incident Management Room on notification of an incident


 Start up the computer and commence to register activities as they are relayed from the
Emergency Coordinator, On Site Controller and Emergency Response Planner as they occur
 Provide a detailed analysis of events upon notification of incident stand down

Security

Security shall;

 Co-ordinate with the On-Site Controller


 Provide security controls to the scene of the incident
 Provide logistic support to assist the medical department
 Provide support to the On-Site Controller regarding ongoing security of personnel

Fire Marshall

 Notifying the appropriate authorities in the event of a fire in site temporary offices, warehouses
and storage yards in the site installation
 Protection of the consortium premises and assets from destruction by fire without endangering
his own safety.
 Fighting any outbreaks of fire and any of the emergencies occurring at site.

Employees

Employees will undertake the following duties in the fulfilment of their emergency responsibilities:

 Immediately report all emergency situations to the security department and, where it is safe to
do so, make the area safe.
 Upon being notified of an emergency situation, employees shall proceed to their nominated
emergency meeting place, unless instructed otherwise.
 Ensure, where it is safe to do so, that all equipment is isolated and made safe.
 Immediately report to muster checker on arrival at emergency meeting place.
 Remain at the assembly point until otherwise instructed by meeting place muster checker.
 Return to work when the All Clear is given.

4.1 Fire Fighting Teams


The fire warden’s will be appointed from the watchman’s teams, security guards and labors and
staff offices will be identified and trained to handle a startup fires with fire extinguishers and Sand
materials only.
When fires can be easily extinguished using fire extinguisher by designated employees in their
normal working clothes without compromising their safety due to heat, smoke
The fire team can extinguish both interior and exterior fires.
The only requirements for the startup stage fires are:
 Team must be able to fight in working clothes.
 Team should not have to take excessive evasive action against heat and smoke
 Team must be able to fight the fire effectively with extinguishers available in the work
place.
 Foreman will act as a Fire Coordinator and will control the teams in each designated area
in case of Emergency Only.

The teams should not perform duties for which they have not been trained as follow:

The emergency spill control team will be set up after mobilization. The team will be appointed

Response to Minor Incident cases

For any minor injury, minor fire, minor spillage, the consortium’s first aid room and
emergency teams will handle the emergency. The employee shall report the incident
to the foreman.
The foreman will report to the HSE Officer. The male nurse will report to the scene.
The Male nurse will assess the injury, if the case requires further or advance
treatment, the injured person will be referred to the nearest medical facilities HAMAD
Hospital and will be transported by one of the contractor own vehicle.
Emergency standby vehicle (Ambulance or pick up car) will be used for emergencies
only, driver contact details will be provided to each employee during the time of Site
induction
The project emergency phone number will be displayed at the entrance of site, rest
areas, and on all notice boards on site.
The site engineer will fill the first aid log and the first aid report form.

4.3.2 Emergencies Level Two – Major Emergency


The below listed major emergencies are expected to have a serious impact and
completely disrupts one or more operations of the contractor and may affect life safety
and the external emergency services in this case shall be required
Depending on the situations, the site emergency teams would be required to take part
on the emergency rescue
In the end, for any major emergency, the Police Control Room Emergency Phone
Number: 999 will be called and they will take control of the emergency.
Major Emergencies include, but are not limited to:
 Mass casualties resulted of slab collapse, crane accident, cave-in workers trapped
inside, Illegal connection and discharge of sewage in the sewer line causing
flooding.
 An unconscious person – who doesn’t wake or respond when shaken

phone to the Management Committee.
Velosi organizations will follow their respective crisis Management Process.
Reporting of emergency cases should be done immediately by phone and within one day by e-
mail.

Minor Incidents
Reporting must be via a Monthly HSE Reporting process.

Incident Investigation (Root Cause Analysis)


For Major Incidents or High Potential Incidents (HIPO)
The Management Committee will nominate an external Incident Investigation Team Leader.
Investigation Team Leader pulls together a team comprising of root cause analysis (RCA)
specialist: Project HSE Manager, HSE Engineer, Project Construction Manager and a number
of technical and subject matter specialists
Incident investigation team conducts a root cause analysis, Investigation Team Leader

Project HSE Engineer will lead the investigation team, The Project Manager or his deputy

Liaison with Emergency Services

The project HSE Engineer and his appointed safety officer on duty will be responsible to liaise

5 EMERGENCY FACILITIES & EQUIPMENT’S ON SITE

The following emergency facilities and equipment shall be provided to ensure that all potential
emergency requirements are adequately addressed:

5.1 Incident Command Centre


Velosi has established an Incident Management team to co-ordinate the evacuation of sub-
contractor personnel, provide instructions and information and communicate with the Construction
manager.
All communications to Velosi personnel and Contractors shall be via the Incident Management
Team. Velosi shall conduct all liaisons with local authorities, police, fire brigade, etc and the media.
Velosi shall provide contractors managers with continuous updates of proceedings.
The HSE Engineer shall maintain a contact list and a copy shall be kept in his custody. This list shall
contain contact numbers for emergency services, emergency response team, safety team, and
relevant members of Velosi management and sub-contractor companies if applicable.
HSE Engineer is to assemble team based on knowledge provided at the time of incident
notification
The Incident Management Room in the HSE Manager’s office and/or designated Meeting Room
shall be furnished with the following items of emergency related equipment:
 Satellite and/or Mobile Phone
 White board and pens for listing emergency information.

5.2 Emergency Medical Facility


The site medical facility shall be furnished with the following items of emergency related

 Ambulance/Standby vehicle (vehicles proposed to be used on this contract not be more than
five (5) years old.

5.3 Project Site


The actual project sites shall be furnished with the following items of emergency related
equipment:
 Fire extinguishers.
 Notice boards containing emergency response information.
 Emergency “Meeting Places”.
 Emergency first aid and fire packs.

6 EMERGENCY RESPONSE TRAINING AND EXERCISES


To ensure all company office and site personnel are fully conversant with the company’s emergency
response requirements, the following training and emergency exercises will be undertaken:

6.1 HSE Induction


All visitors and personnel entering the site shall be provided with instructions in what action to
take in the event of a site emergency. Key subject areas will be:
 Overview of emergency plan and procedures.
 Potential emergency threats on the project.
 Action to take in the event of an emergency.
 Location of “Meeting Places”.
 Action to be taken at a “Meeting Place”.
 Medical emergency response requirements
 First aid requirements
 Emergency numbers

6.2 Emergency Exercises/Drill


An in-house emergency exercise will be conducted at least once in three months to test the
efficiency of the emergency response system and emergency response personnel Medical
emergency
The HSE Engineer will prepare a written overview of the planned exercise and will facilitate the
drill.
The On Scene Commander and Incident Management Team shall co-ordinate all emergency
exercises.
Every drill will incorporate a full site muster (where applicable) and will also include a specific
emergency, e.g., fire, motor vehicle accident, medical emergency, etc.
Where practicable, external emergency groups will be requested to participate in these drills.
A report will be prepared following each emergency drill providing an overview of the drill and
making recommendations for improvements.

6.3 First Aid, Fire Fighting and Prevention


Velosi will nominate and have formally trained a number of personnel to address office and site
first aid and firefighting and prevention requirements. The number of personnel trained will be
sufficient to ensure that statutory requirements are addressed and more importantly that potential
site emergency requirements are fully covered.
Additionally, personnel will be nominated according to their work area to ensure that all work
areas are adequately provided with emergency cover.
Up to date list shall be retained for all trained emergency response team members and members
of the Incident Management Team.

6.4 Emergency Response Team


An Emergency Response Team will be formed from HSE personnel and nominated site
personnel to receive sufficient training that will make them proficient to deal with site
emergencies that are beyond the scope of those personnel.
The Emergency Response Team will be headed by a nominated HSE Officer and overseen by
the Engineer.
The Emergency Response team will conduct regular formal emergency drills to test their level of
preparedness and proficiency.
Following each drill, a review will be held, and a report prepared outlining the actual drill and the
findings of the exercise.

6.5 Emergency Desktop Exercises


Every month an emergency desktop exercise will be conducted to assess the level of
preparedness of emergency response personnel and the system. The HSE Engineer, in liaison
with Project Manager or his Department Heads will prepare a written emergency scenario. The
Emergency Response team will then respond to the emergency scenario as if it were a real
emergency.
A HSE team will be held at the end of each desktop exercise to assess the group’s response and
to determine the level of preparedness of the organization.
Where required, this emergency response plan will be modified to address any identified areas of
weakness.

The exercise/drill schedule and the frequency may be revised depending on the requirements of
the project phase.

6.6 Tracking of Personnel Entering the Offices, Premises


At Velosi offices, as enforcement of legislation, tracking of Velosi personnel is facilitated by
fingerprint scanning machines however there is no system available to record the visitors Velosi
Offices and its Contractors at site. Velosi HSE Manager and Admin-HR Manager shall work on a
system to register the visitors who are available in the premises of Velosi.
Emergency Assembly points have been and need to be allocated within Velosi premises.
The system will be capable to identify “Real-time” emergency sheets detailing those persons
allocated to a muster point.
The muster checker, at each muster point, shall conduct a roll call and verify who is absent from
the “Meeting Place”.
Security personnel mainly from Contractors shall be responsible for preparing, issuing and
retrieving identification badges for each person entering and leaving the project site.

6.7 Emergency Alarms and Evacuation


An electronic alarm system will be installed - the alarm will be of a single tone three seconds on /
three seconds off / three seconds on / three seconds off for a period of one minute. Site All Clear
is a continuous tone from Site Alarm System.

6.7.1 Site Related Emergency Alarm

Manual emergency alarm panel is to be situated within the site where applicable.
These alarms will be activated to alert personnel of fire or other site related
emergencies. Upon hearing the alarm, Site Management or Site Office will notify the
Incident Management team who shall respond accordingly. Sirens for fire alarms are
located at the Velosi premises and its Subcontractors.

6.7.2 Actions to be taken on Hearing Site Related Emergency Alarm

Velosi will ensure that all personnel entering its premises and project sites are aware
of the correct procedures to be followed in the event of emergency. Personnel will be
informed of all emergency requirements at their initial induction and at subsequent
toolbox meetings.

On Hearing a Site Related Emergency Alarm All Construction Personnel Shall:


 Stop all work immediately.
 Shutdown and isolate all sources of ignition.
 Crane drivers are to make safe their loads.
 Personnel to proceed to designated meeting places.

 Drivers must park vehicles in a safe location, ensuring that emergency vehicle
access is not blocked, and switch off engines, leave keys in ignition and proceed
to designated assembly point on foot.
 A check of the site by the Project Emergency Response Team will take place to
ensure that all employees have stopped work and evacuated the area.
 All “work permits” are suspended automatically in the event of an “Emergency
Evacuation” and will be re-issued once it is deemed safe to do so.
 Periodic Emergency Drills shall take place to ensure employee familiarity with the
process.

6.7.3 Actions to be taken on Hearing an Office Related Alarm


 Stop work and immediately exit the building via the marked emergency exit doors.
 Once clear of the build go to your designated Meeting Place unless wind direction
or other circumstances prohibit this action.
 If normal meeting place cannot be accessed go to alternative meeting place
 On reporting to Meeting Place check in with meeting place checker.
 Standby until all clear is give/sounded.

6.8 Assembly Points/Muster Points


Assembly Points have been located within the office and on site and may change due
work being performed.
A Green “MEETING PLACE” sign will highlight each assembly point – See Figure 1
for example of “Meeting Place” signs.
Nominated Velosi Senior person shall act as co-coordinators at assembly points
during an emergency.

6.9 All Clear Signal

The “All Clear” signal is a 10-second continuous sounding of the alarm.

Upon hearing the “All Clear Signal” personnel may only return to work if instructed to do so
by their supervisor - who shall have been contacted by the HSE Engineer and/or HSE in
Charge confirming that it is safe to do so.

Personnel who have been working under hot work, confined space or an excavation permit
may not return to work until the “work permit” has been revalidated by the responsible
Engineer.

 Office Fire Warden will test the fire alarm every Thursday morning to determine if it
can be heard by all employees.
 During the planned emergency drills, employees will be made familiar with the sound
of the alarms.
 Will ensure that there are sufficient exits available at all times.
 At least two exits from hazardous areas when possible will be available on every
structure and building.
 All exits will be marked with exit signs and there is sufficient lighting, so people can
safely travel to an exit. Access and egress must be kept clear from obstructions.

7 EMERGENCY RESPONSES

7.1 Introduction
In the event of an emergency the first consideration is the safety of the people and in
attending at such matters, incident, accident and emergencies Velosi will provide suitable
core teams and vehicles to allow the workers to be carried out in a safe and efficient
manner and in relation to accident and such damage Velosi will attend and make safe all
instances of accident damage and clear all damaged components in site and give a
remedy to any such accident damage and maintain suitable records and report all
activities and findings also provide a photo record of all activity including before and after
for all sites.
The Project HSE manager will inform:
 Inform Ashghal HSE department immediately.
 Notify Ashghal HSE department within 24 Hrs. by using Incident notification report
form.
 Carry out an investigation and submit report after 7 days to Ashghal HSE department

7.2 General Case


In case of emergency, employees and visitors are requested to:
 Give the alert and call the project emergency phone number
 Assess and secure the area if safe to do so
 Not expose anyone to further danger
 Refrain from taking action if not trained to do so
 Not interfere with the operations by the emergency teams

7.3 Evacuation Signal


If you hear the evacuation signal:
 Stop work.
 Shut down any source of ignition.
 Evacuate the area and proceed to the nearest Emergency Assembly Point as directed
as quickly as possible (do not run).
 Remain there for head count and await further instructions from Assembly Point
Warden.
 Do not leave the assembly point or re-enter the evacuated area without permission
from Assembly Point Warden.
 Do not resume work until you hear the "all clear " given by Assembly Point Warden

7.4 Medical Emergency Case


In case of medical emergency case:
 Immediately call the project emergency phone number
 Send someone to guide the first aider to the exact location on site
 Assess and secure the location
- to prevent aggravation in case of further immediate danger
- to preserve evidences
 Do not move the person unless exposed to immediate life-threatening danger
 Speak to the person and assure the response team is on the way

7.5 Fire And Explosion Case


In case of fire and explosion emergency case:
 Immediately activate nearest fire alarm device or shout “FIRE, FIRE, FIRE” to make
other personnel inform on the occurrence of fire or explosion
 Call the project emergency phone number
 Send someone to guide the fire warden to the exact location on site
 Assess and secure the location
- to prevent aggravation in case of further immediate danger
- to preserve evidences
 If safe to do so, identify the type of fire and start fighting the fire with adequate
extinguisher
 Use designated emergency exit route and proceed to assembly evacuation point

7.6 Environmental Pollution Case


In case of environmental pollution case (e.g. spillage)
 Immediately call the project emergency phone number
 Send someone to guide the response team to the exact location on site
 Assess and secure the location
- to prevent aggravation in case of further immediate danger
- to preserve evidences
 If spilled materials are flammable; eliminate sources of ignition from near the spill.
 If safe to do so, stop the source of the spill and contain the spill.
 Avoid inhalation of vapors by staying upwind.

7.7 Vehicular Accident Case


In case of vehicular accident case on the project site:
 Immediately call the project emergency phone number
 Send someone to guide the response team to the exact location on site
 Assess and secure the location
- to prevent aggravation in case of further immediate danger
- to preserve evidences
In case of vehicular accident case outside the project site:
 Take care of the medical needs of anyone involved.
 Do not admit fault at the scene. Advise the other party that the accident will be
reported to the local police.
 Contact the police at 999 or Contact traffic and Patrol Department at 4489-0666 if the
accident took place in or out the site.
 Do not leave the scene of the accident (unless you have to seek medical attention)
and until being released by the police with the necessary paperwork.
 Report the accident to your supervisor and call the project emergency phone number

Pedestrian/Traffic Routes, Movement of Plant and People


Access plant other vehicles and equipment, but also for people, therefore segregation is
the safest method. Ideally traffic routes will be organised so that pedestrians are not put
at risk by incorporating such measures as:
 Organise the site entrances and exits so as not to cause a traffic hazard on the public
road
 Segregate the traffic and pedestrian routes and, if practical and necessary,
segregated site entrances.
 One – way systems
 In turning areas to avoid the need to reverse
 Speed limits that must be enforced as necessary
 Reversing vehicles being under the control of a qualified signaller
 The need for published traffic priorities where site vehicles and private traffic both use
site roads
 The need for the provision of traffic control measures (e.g. traffic lights), both on and
off site
 The provision of parking areas for delivery vehicles which are waiting to unload, if
necessary, for example tightly-bound city centre sites, this may need to be off site.
 The provision of a car park for privately owned vehicle which preferably:
- Does not permit access to the construction area by private vehicles
- If work area connected to the site offices and welfare facilities by a safe,
segregated pedestrian route

7.8 Severe Weather Emergency


In case of severe weather case:
 Secure lifting and crane operations
 Secure flammable and diesel storage tanks or drums

 Secure scaffolding and/or excavations works

7.8.1 Rain
 Flooding of Highway including Temporary Traffic Management
Arrangements
The objective of prevention is to reduce or eliminate the risk of being affected
by the flooding to the road users and construction activities.
The roads opened for beneficial use shall be reviewed to identify areas with
potential for flooding. All necessary resources and man power shall be made
available to control the situation.
The gullies and water drain lines shall be cleaned from dust and debris
materials to ensure the water flow will not be blocked.
 Flooding of Woke Site Areas
The construction areas with potential for flooding shall be identified and all
necessary resources and man power shall be made available to remove the
accumulated water from the work areas. Temporary catch-pits shall be made
to accumulate the rain water and water pumps and water shall be used to
remove the water.
An emergency team shall be deployed to act upon any emergency.
 Flooding of Adjacent Properties
VELOSI shall ensure no nuisance is created to the properties adjacent to the
construction areas. If any water accumulation happens in the adjacent
properties due to the construction works, Velosi shall be responsible and
necessary actions shall be taken to reinstate the adjacent properties.

7.10 Confined Space Emergency


 Do not enter the confined space to rescue a victim if you are not trained to do so or
until your safety is ensured.

 Ventilate the area until the victim is rescued.
 Attempt non-entry rescue with rescue and resuscitation equipment to remove the
victim out of the confined space

shall attempt entry rescue with suitable rescue and resuscitation equipment).
 Avail Medical assistance by calling 999.

7.11 Excavation Rescue /Cave-in Emergency


If you have witnessed an excavation falling of person(s) or cave-in incident,
 Alert and clear others in the immediate vicinity of the excavation.
 Eliminate any source of electricity, vibration by shutdown plant, equipment, vehicle
traffic etc. if necessary and say to do so.
 Isolate and restrict access to the incident scene.
 Do not attempt to move an injured person if not in imminent danger (physically
injured) and inform the first aider immediately to assess the IP to further assist for safe
rescue of person(s).
 Make sure for standby rescue equipment (i.e. liftable stretcher, man basket, crane
etc.) for prompt rescue.
 Avail medical assistance by calling 999.
 Identify the job supervisor, note exact time, note number and location of workers
trapped/accident.
 Alert the emergency contact personnel (ERT member) and your supervisor and
communicate accident details.
 Follow the instruction of the emergency contact person.
 Assist workers not trapped out of trench safely, if safe to do so.
 Keep all personnel 50 feet away to prevent further cave in.
 Do not attempt to rescue the trapped personnel if you are not trained to do so.
 Follow excavation evacuation procedure and make sure emergency personnel details
to be displayed and safe excavation rescue procedure to be communicated to all
concerned.

7.12 Biological Emergency

Biological agents are harmful toxins which can harm or even kill people, animals, and
plants.
Bacteria, viruses, and toxins are the three biological agents used most often as weapons.
Biological agents are usually tricky to grow. Anthrax spores are one of the few that will
live for a long time; most break down quickly if they become exposed to the environment.
These toxins can be dispersed in three ways: (1) being sprayed in the air, (2) by
contaminating food and water, and (3) by infecting any animals that could infect humans.
Here are the delivery methods:
 Aerosols – toxins can become spread out in the air and drift as a mist for long
stretches of land. When this mist is inhaled it can cause disease.
 Animals –diseases can be spread by animals and insects to other animals or people.
 Food and Water Contamination –toxins can get into a food or water supply.
However, most of these microbes can be killed by cooking the food and/or by boiling
the water. These microbes must be boiled for at least one minute in order to be killed
but it may require a longer space of time so be sure to always follow instructions.
 Person-to-Person – it is possible to get infected from people. There are several
cases, such as smallpox or the plague, where people have been the source of
infection.

Preventive and control measures may include:


 Elimination of the source of contamination is fundamental to the prevention and
control of biological hazards. Engineering controls such as improvement of ventilation,
partial isolation of the contamination source, installation of negative pressure and
separate ventilation and air- conditioning system (e.g. in medical wards for infectious
diseases) and the use of ultraviolet lamps can help contain the spread of
contaminants. If the contact with biological hazards cannot be prevented, the
employees must use personal protective equipment and adhere strictly to the practice

of personal hygiene. The personal protective equipment includes masks, gloves,


protective clothing, face shields and shoe covers.
 A complete sterilization process should include disinfecting the contaminated
premises and thorough cleaning of any residual toxic substances, to ensure that
employees would not be harmed through exposure in the risk area and the
engineering control including vaccination.
 Washing hands with liquid soap is the simplest and most basic method to avoid
infection. However, it is often neglected but ensures that wash hands before and after
work. Also wash hands immediately before and after wearing protective clothing,
uniforms or gloves to reduce the possibility of infection.

In case of any Bio-hazardous Spill


 Get everyone out of the affected area and warn others not to enter the contaminated
area.
 Remove contaminated clothing and containerize contaminated garments for
autoclaving and thoroughly wash hands and face, or shower when needed.
 Determine the necessity for medical attention for persons exposed to the potentially
hazardous materials.
 Wear protective clothing when entering to clean the spill area (rubber gloves,
autoclavable footwear, outer garment, and appropriate respirator).
 Try to use things that can fit snugly over your mouth and your nose to help filter
contaminants. You may have to improvise to protect your mouth, nose, eyes, skin,
etc.
 For spills on the floor, pour a germicidal solution around the spill and allow to flow into
the spill.
 If splash has occurred, seek medical attention.
 Spray or wipe walls, work surfaces, and equipment with an appropriate disinfectant
detergent.
 Seek out medical assistance and authorities. You may need to be quarantined or
simply advised to stay away from people.

Ecology and Nature Conservation


In some cases of work activities on site, it may often encounter uninvited visitant such as
animals, reptiles and insects. Though many of them are harmless, many can be
dangerous and even lethal. Although someone may be tempted to pet or play with the
cute creature, it is truly not the safe thing to do so. Here are some things you should know
about animals and what to do if you encounter one.

Why animals attack;


Animals attack for various reasons which could include:
 Protecting offspring.
 Protecting “their” territory.
 Feel threatened.
 Injured or ill.
 Disturb eating or sleeping.
 Excited.
 Being teased.

If you are ever confronted by unfamiliar animals, you should use utmost caution
and used following escape danger:
 Remain calm and quiet.
 Move slowly always from the animals.
 Avoid jumping movements or threatening action.
 Face the animal, don’t turn back on it.
 Look just over the animal’s head and avoid directly looking in to eyes.
 Try to put something between you and the animal to shield yourself (like; bag,
wood, board package etc.).
When you reach in safe distance, you should notify your supervisor and ERT member
immediately. Incident or deputy incident controllers shall inform on emergency hotline on
999 or animal control agency to pick up the animal.

7.13 Civil Disorder Emergency


Civil disorders can include picketing, marches, parades, sit-ins, trespassing and riots.
Avoid provoking or obstructing demonstrators. In most cases, since prior warning of
demonstrations can be expected, adequate precautions can be taken prior to the event. A
threatening disturbance should be reported immediately by calling 999. Police will assess
the situation. Participants who refuse to disperse may be arrested for violation of law.

7.14 Security/Armed intruder incidents


In case of security threats/armed intruder incidents;
Try to remain calm. Co-operate with the intruder/s. Observe as much as possible, but
don’t stare. Call 999 when safe to do so.

7.15 Gas Leak


Many of the gases use at construction site can be detected by odour. Gas leaks will
quickly dissipate in naturally ventilated areas and pose a limited risk of fire or explosion.
Despite this, avoid the use of mobile phones (potential ignition source) in the immediate
area.

During Emergency Gas Leak


If the gas leak is detected outside an office structure or building:
 Move away from the gas leaked area.
 Use gas detectors to know the severity of gas leak by the Authorized gas testers on
site.
 Call and inform the site emergency response team.
 Muster at the emergency assembly point.
 Incident controller to inform the authorities, if required.

If the gas leak is detected inside a building or office structures:


 Warn anyone in the imminent danger area.
 Notify the site and local emergency responders.
 Evacuate all to a well-ventilated external area at the emergency assembly point.
 Notify authorities, if required.

7.16 Emergency Response during Night Shift/Extended Working Hours


 Inform emergency hotline by calling 999, if any emergency at night time that cannot
be controlled by the site emergency team.

7.17 Electrical Emergency


Electric Shock
 When someone suffers serious electrical shock, he or she may be knocked
unconscious.
 If the victim is still in contact with the electrical current, do not touch the person and
immediately turn off the electrical power source.
 If you cannot disconnect the power source, try to separate the victim from the power
source with a nonconductive object, such as a wood-handled broom.
 Have someone call for emergency medical assistance immediately.
 Administer first aid, as appropriate.
 Alert the emergency contact personnel (ERT member) and your supervisor and
communicate accident details.
 Call the Medical emergency 999

Electrical Fire
 If an electrical fire occurs, try to disconnect the electrical power source, if possible.
 If the fire is small, you are not in immediate danger, and you have been trained in
fighting fires, use any type of fire extinguisher except water to extinguish the fire.

Power Lines
 Stay away from live power lines and downed power lines.
 Be particularly careful if a live power line is touching a body of water.
 The water could conduct electricity. If a power line falls on your car while you are
inside, remain in the vehicle until help arrives.

Victim support
 Victims need immediate medical response: check for breathing and pulse, and then
apply CPR as necessary to maintain oxygenation.
 If a victim is still conscious after having been shocked, they need to be closely
monitored and cared for until trained emergency response personnel arrive.
 There is danger of physiological shock, so keep the victim warm and comfortable.
 Shock victims may suffer heart trouble up to several hours after being shocked.
 The danger of electric shock does not end after the immediate medical attention.

The ERP will be reviewed and whenever necessary revised and updated every 6 months.
Besides, periodical internal auditing will be carried out to ensure its compliance in not later than 6
months’ interval.

9 COMMUNICATION
This plan shall be communicated to all personnel and stakeholders within the project, through
various communication media, such as induction, notice boards, and tool box talks.
