    Please explain in writing within five (5) days upon receipt of this memorandum, with the assistance of
    counsel if you so desire, why no disciplinary action should be taken against you for not following proper
    dress code appearance on September 23, 2019.

    Our Code of Conduct and Discipline states that:

    III. OFFENSES AGAINST COMPANY INTEREST:

    Failure to wear the prescribed office uniform is subject to the following disciplinary action:

    1st offense - Written Warning


    2ndoffense - Three (3) days suspension
    3rd offense - Five (5) days suspension to Ten (10) days suspension
    4th offense - Eleven (11) days suspension to Twenty (20) days suspension
    5th offense - Twenty-One (21) days suspension to Thirty (30) days suspension

    Your failure to submit the written explanation within the prescribe period will be construed as a waiver on
    your part to the same and the Company shall proceed to resolve the complaint against you based on available
    documents/records. However, you will be provided with a copy of the decision.
