AFFIDAVIT OF FUNERAL EXPENSES

I, Bernardita C. Raganit of legal age, widow and a resident of Block 3 Lot 27

Kadungganan Village, Brgy 62-A, Tacloban City after having duly sworn to in accordance with
law, do hereby depose and states THAT:
1. I am the wife of the late Ricardo G. Raganit
2. Died on July 27, 2019 in Cebu City
3. That the following are lists of the funeral expenses for the burial of my
husband;
a. Tomb, gravestone and labor – P12,500.00
b. Food and snacks during wake – P8,200.00
c. Mass for burial ceremony – P500.00
d. Food and snacks during padasal – P1,500.00
e. Mass for the 9th day (Pasiyam) – P500.00
4. That I am executing this affidavit for the purpose of attesting the truth of the
foregoing funeral expenses and for whatever legal purpose it may serve.
