PARENT CONSENT IN CONDUCTING ACTION RESEARCH

Dear Parent/Guardian:

I am Mrs. Anecita N. Rodriguez, your son/daughter teacher in Wellness Massage. I

want to improve my skill and strategy in teaching about Massage to help students understand
the skills needed for them to be ready in competing globally and to become skillful massage
therapist someday by having action research in using new teaching strategy.

In view of these, I would like to invite your son/daughter to participate in this activity. I
will conduct action research in which the students will have series of activities related in this
study. This will involve keeping track of records with the result of their activities. Comparing the
previous result to the present in which the strategy is being presented. Students might be
concerned to let other students see their output. I will protect and respect their privacy
through data safeguarding and that their reputation will not be compromise specifically during
data analysis and sharing with others in discussion or reports.

Participation in this activity is voluntary. Your son/daughter is free to refuse to be

interviewed, surveyed, and observed. They can change their mind about participation in this
activity at any time. Your son/daughter standing in class will not be influenced by agreeing or
refusing to participate in any portion of this project. If you have any questions about my plans

If you agree that your son/daughter is able to take part in my action research, please
return a signed copy of this form to me as soon as possible you may keep the other copy for
future reference. Thank you in advance for your cooperation and I hope your son/daughter
