Anecdotal Observation Record

Child’s Name: Emma Feiertag Age: 4 (02/06/2016)

Observer: Katie Harder Date: 02/20/20
Domain – Cognitive Development

Anecdotal Note: Describe what you saw the child do and/or heard the child say.

Emma was sitting at the breakfast table, getting ready to eat her waffles. Emma put the fork in her
right hand, in a palmer’s grasp. Emma tried scooping the waffles up on her fork twice. Each time
the waffles piece fell off her fork. Emma then turned the fork, so it was straight up and down and
tried poking into the waffles. Emma tried this three times, trying harder each time. On the third
try, Emma pushed harder while poking the waffle and was able to stick the waffle with fork.
Emma then brought the fork/waffle to her mouth, with the back of her hand facing the table,
Emma successfully got the waffle in her mouth and ate it.
