Good day.

I am Elisito O. Inciso, resident of Kidapawan, Cotabato City . I am a Professional Driver

(Dumptruck) for the projects of Engr. John Eduard D. Gallego, resident of Chapter 7-A,
Camiguin, Aplaya, Digos City. I was assigned to transport materials for several pending
construction projects within and outside Davao,Digos & Bansalan Davao Del Sur. The following
are the on-going construction projects:

1. Two-Storey Residential House

Owner: Juvy Mabini
Location: Northcrest Subdivision, Cabantian, Davao City

2. Two-Storey Residential House

Owner: Revsan Ethelbert P. Elizalde
Location: Ciudad Verde Subdivision, Maa, Davao City

3. Sea Oil Gas Station Office

Owner: Manuel Alonzo
Location: Bansalan Davao Del Sur

4. Two-Storey Residential House

Owner: Loreta Garcia
Location: Burgos 3rd, Mabini Street, Digos City

In relation to the foregoing, may I request from your good office permission to be allowed entry
to and exit from Davao, Digos & Bansalan Davao Del Sur so that I may attend to these projects?
It is of utmost importance that I personally transport materials for these projects to avoid delay. I
am fully aware of the present situation. Thus, I also undertake that I will strictly observe the
directives of the authorities in relation to my entry and exit, and in my construction projects as
well.
