CHCCSM004 Coordinate complex case required

The unit CHCCSM004 Coordinate complex case requirements describes the knowledge and

skills required to coordinate multiple services requirements for clients with complex needs that are

within certain case of management framework [ CITATION Aus18 \l 2057 ]. In accordance to coordinate

complex case requirement, this article shall tackle on a case of three clients that had complex needs.

Ben, John and Tom are people with different personalities, and I will be having a close personal

observation studying on their dairy routines and determine on how I will offer help that meets their

needs.

I will establish coordination function by:

i. Working closely with the clients at the workplace or at the field and try involving

other services that would help determine service provision requirements to them.

ii. Engaging the clients in most of the negotiations collaborations concerning any wok

arrangements, the schedule to follow, what they good at, and asking questions that

would relate to their daily lives.

iii. Associating with the clients into terms that would reach coordinate requirements and

closeness and also come up with a plan that would trigger and identify all

timeframes and expected outcomes.

After establishing a successful coordination function with the three clients, the next essential step

would be to support them to access a variety services, that would help solve their individual

complications by:

i. Trying to identify, implement and maintain duty of care responsibilities to them.

ii. Woking with the clients to establish communication requirements

iii. Assessing and figure out needs of the clients, then arranging for an interpreter according to

clients needs.

iv. Organising and facilitating individual meetings and conferences to coordinate.

v. Working with the client to prioritise needs and communicate these with the service providers

vi. Educating the clients on information about the coordination role

Observing on Tom, he is always on move and rarely settles in one area, he also talks too much

and loud. He is always involved in little fights with his fellow boys. In the case of John, he always into

himself and prefers to staying alone. He rarely starts conversions with strangers and also does he to his

fellow friends and work mates. His face appears to have scars including his arms and the neck. Ben is

well known as a sociable boy, who can get along with anyone. He says he has spent much of his life on

the streets and he doesn’t know where his family is. He recognises himself as a go getter, and despite

his age and shortness, he believes no one can take what belongs to him. After the observation, it is

evident that Tom is a disruptive person, who still a teenage and needs to be disciplined. After engaging

in conversation with John, he opens up about being physically abused by someone from home and he is

hearing impaired. He fears getting back home from job, since he often has to go through physical

harassment, which makes him feel depressed. Ben, on the other hand, is an abandoned child who has

no place to stay and he often says he is hungry; he also has agitation and aggression issues.

To provide on the needed service, resource and support, Tom needs a close assistance, who is to

work and monitor him closely. The assistance need to offer guidance and counselling to Tom regularly

and trying on having tactics that would see Tom always busy, like giving some house chores to do,

Associating with him in games and other field sports. The assistance as well needs financial support

along with help to discipline Tom. John needs to stop being abused and ensure that he is shifted to a

safe environment where he feels protected. The priority is to deal with the current wounds and make

sure that he is shifted to a safe place; also, helping him with hearing gears would of great concern to

him. Another priority that should be taken into account is providing a case manager on an immediate

basis. Ben needs to be taken care of and be provided with basic needs. He also needs someone who can

be closer, and listen to him in patient manner and calm him down by trying to address his anxiety

issues. The priority that needs to be observed is to always attend on him in the most immediate manner

possible.

After the support provided to the clients, it would be essential to provide information about the

most suitable agencies where the clients could attend for protection of their rights. In the first case of

Tom, the key potential referral agencies in regard to his needs is the Beyond blue to Allied

Psychological Services (ATAPS), other needed support could be help from the communication with the

service providers. I would refer John to take the one of the, Older Person Abuse Prevention Referral

and Information Line (APRIL), Advocare INC, NSW Elder Abuse Helpline or Senior Rights Victoria

as the potential referral agencies that would see help him deal with the abuse. The key potential referral

agencies to Ben would be Head space, beyond blue or youth service Australia.

So as to monitor the progress of the three clients, I would achieve this by:

i. Facilitating and ensuring that there is communication between service provider (the referral

agencies) and the client so that we can manage and identify service duplication.

ii. Working closely with the client and service provider to control the progress towards positive

outcomes.

iii. Providing grounds for identifying and implementing further support required to meet needs and

progress towards outcomes.

iv. Obtain feedback from the clients about the services.
